Gelsolin-3: A Novel Myelin Autoantigen in Demyelinating Guillain-Barré Syndrome.
Although immunoglobulin G (IgG) autoantibodies against gangliosides are well-established in axonal Guillain-Barré syndrome (GBS), specific autoantibodies in demyelinating GBS-including acute inflammatory demyelinating polyneuropathy (AIDP) and its variant, bifacial weakness with paresthesias (BFP)-remain unidentified. We aimed to identify novel autoantibodies and clarify the immunological link between AIDP and BFP. Immunoprecipitation and mass spectrometry identified gelsolin from BFP serum IgG targeting non-compact, periaxonal myelin surrounding the nodes of Ranvier. Western blotting confirmed specific recognition of gelsolin-3, with no cross-reactivity to the other 2 isoforms. Anti-gelsolin-3 IgG prevalence was screened in 388 GBS patients and correlated with clinical and electrophysiological phenotypes. In vivo pathogenicity was assessed by intraneural IgG injection. Anti-gelsolin-3 IgG antibodies were detected in 24 GBS patients (6%, p < 0.001 vs both disease and healthy controls), with no positive controls. These autoantibodies were strongly associated with facial weakness (63% vs 18%, p < 0.001) and the AIDP electrophysiological classification (73% vs 31%, p < 0.001). In vivo, these IgG1 autoantibodies bound to the nodal regions, triggering complement-dependent nodal injury, voltage-gated sodium channel cluster disruption, and concomitant demyelination. These early pathogenic events preceded macrophage infiltration. Anti-gelsolin-3 IgG1 represents a novel biomarker linking AIDP and BFP. Our results provide the first evidence that AIDP can be formally classified as a nodo-paranodopathy. This IgG1-mediated, complement-dependent nodal injury establishes a unifying paradigm across GBS: nodal complement activation represents a shared mechanism underlying conduction block despite targeting distinct domains-the axolemma in AMAN and myelin components at the axon-myelin interface in AIDP. Exploring the link between adenovirus infection and Guillain-Barré syndrome in children: a case-based analysis.
Adenovirus infections are commonly associated with respiratory tract infections, conjunctivitis, gastrointestinal disturbances, and in some cases, more severe illnesses such as pneumonia. Guillain-Barré syndrome (GBS), a rare autoimmune disorder that causes progressive peripheral nerve inflammation and paralysis, has been linked to various infections, including viral ones. We describe two newly diagnosed pediatric cases of GBS following adenovirus infection. Both patients were previously healthy boys, aged 3 and 6 years. Adenovirus was detected by nasopharyngeal PCR shortly preceding the neurological manifestations. Both were treated with intravenous immunoglobulin (IVIG); one also required plasma exchange and respiratory support. Both recovered fully during follow-up. Additionally, a relevant case from the literature was reviewed for comparison. Treatment included respiratory support, IVIG and plasma exchange therapy. During the 12-month follow-up no severe complications were observed in our patients. In conclusion, while GBS is a well-established complication of several viral infections, including adenovirus, the association between adenovirus and GBS remains relatively underexplored. Our case reports, along with a review of the literature, highlight the rare but plausible link between adenovirus infection and the development of GBS. Although adenovirus infections are common, the occurrence of GBS following these infections appears to be uncommon, suggesting that additional factors may be at play. However, given the potential severity of GBS and the growing body of evidence, it is important for clinicians to remain vigilant for this rare complication in patients with a history of adenovirus infection.
Postmarketing safety surveillance for GSK's AS01E-adjuvanted respiratory syncytial virus prefusion F protein-based vaccine in the USA: protocol for a non-interventional postauthorisation safety study.
In May 2023, the US Food and Drug Administration (FDA) initially approved an AS01E-adjuvanted respiratory syncytial virus (RSV) prefusion F protein-based vaccine (adjuvanted RSVPreF3) for adults aged ≥60 years. The approval was expanded in June 2024 to include adults 50-59 years of age at increased risk for RSV-associated lower respiratory tract disease. In this paper, we describe the protocol of a postmarketing safety study evaluating the association between adjuvanted RSVPreF3 and new-onset Guillain-Barré syndrome (GBS), acute disseminated encephalomyelitis (ADEM) and atrial fibrillation (AF) among adults ≥50 years of age in the USA and provide our rationale for key methodological decisions. The potential associations between adjuvanted RSVPreF3 and GBS, ADEM and AF will be evaluated using secondary healthcare data and the self-controlled risk interval (SCRI) design. Data from five research partners in the USA spanning August 2023 through June 2030 will be used for the conduct of yearly monitoring queries and, sample size permitting, SCRI analyses. Claims-based definitions for new-onset outcomes (first diagnosis in 365 days) are: ≥1 inpatient diagnosis for GBS and ADEM; ≥1 inpatient or ≥2 ambulatory/emergency diagnoses for AF. The primary risk and control windows are 1-42 and 43-84 days, respectively, for GBS and ADEM; and 1-8 and 9-16 days for AF. SCRI analyses for GBS and ADEM will include chart-confirmed cases. SCRI analyses for AF will adjust for the positive predictive value obtained from validation against charts. Conditional Poisson regression will be used to calculate incidence rate ratios. This study was approved by the Institutional Review Boards (IRB) of Harvard Pilgrim Health Care Institute; WIRB-Copernicus Group, Inc and its affiliates (collectively, 'WCG'); WCG IRB, Inc; and Sterling IRB, with Federal Wide Assurance (FWA) numbers FWA00000100, FWA00033319 and FWA00025632, respectively, for all participating research partners. Study results will be shared with the US FDA and publicly disseminated through national or international clinical or scientific conferences and peer-reviewed publications. This protocol has been registered in the Heads of Medicines Agencies-European Medicines Agency Real World Data Catalogues (EUPAS1000000486).
A case-control neuroimaging investigation of chronic Zika virus-infected adults.
Systemic viral infections with neurotropic potential pose significant global health challenges. The Zika virus (ZIKV) is known for its pronounced neurotropism, with recent infectious clusters raising renewed public health concerns. While research has predominantly focused on congenital populations, growing evidence suggests that the mature central nervous system (CNS) is also vulnerable. However, no study has examined the long-term impact of ZIKV infection on the adult human brain. To address this gap, we studied a rare group of adult ZIKV patients presenting with both peripheral (Guillain-Barré Syndrome; GBS) and CNS-related neurological symptoms. We compared these patients at the chronic stage (5 to 12 months post-infection) to healthy controls and to patients with GBS of non-ZIKV etiology (total N = 43). Structural and functional measures included cortical thickness, white matter hyperintensities, diffusion metrics, and resting-state functional connectivity. Despite the rarity of both patient populations, power analyses indicated that our sample size could detect large group differences-effect sizes deemed reasonable given the severity of neurological symptoms in the ZIKV group. Nonetheless, our multimodal analyses yielded null results, with Bayesian statistics (where applicable) providing evidence for a lack of effects. The null findings suggest that chronic ZIKV infection in adults is not associated with brain changes of large magnitude. Importantly, this study offers detailed clinical characterization of a heavily understudied group. In light of recent ZIKV outbreaks, this characterization underscores the need to monitor, study, and provide longitudinal care to adult survivors of severe ZIKV infections.
Deciphering the Breathless Future: A Novel Approach to Predicting Respiratory Failure in Children With Guillain-Barré Syndrome.
The applicability and utility of clinical predictive models for respiratory failure in the pediatric Guillain-Barré syndrome (GBS) and Asian population are significantly constrained. Therefore, we aim to develop and validate a clinical prediction model to predict respiratory failure risk in pediatric GBS patients in China, alongside the economic immunological biomarkers in decision-making, and evaluate the Erasmus GBS Respiratory Insufficiency Score (EGRIS)-Kids score's efficacy. The retrospective study originated from our pediatric GBS cohort at Children's Hospital of Chongqing Medical University during 2014-2022. We utilized logistic regression to identify predictors and construct a nomogram and web-based dynamic nomogram. The net reclassification index and integrated discriminant improvement index were used to compare models after incorporating new indices, with bootstrapping validation. Our study included 175 children, among which 23 (13%) patients have developed respiratory insufficiency. Variables included in our score were: age (odds ratio [OR] 1.23), bulbar palsy (OR 23.17), bilateral hip flexion (OR 0.75), and platelet-to-lymphocyte ratio (PLR; OR 2.47). The area under the receiver operating characteristic curve of the nomogram was 0.899 (95% confidence interval 0.839-0.960). The calibration plots showed an adequate consistency between the reported and predicted occurrence. The EGRIS-Kids model yielded an area under the receiver operating characteristic curve of 0.849 (95% confidence interval 0.754-0.944) in our cohort and the incorporation of PLR exhibited an incremental value of 12.51% (P = 0.03). We performed the first external validation of the EGRIS-Kids score in Chinese children with GBS. Furthermore, we developed a new predictive model incorporating the PLR, which shows promise and additional value but requires further external validation.
