Drusen are extracellular deposits that accumulate below the retinal pigment epithelium on Bruch membrane. Basal laminar drusen refers to an early adult-onset drusen phenotype that shows a pattern of uniform small, slightly raised yellow subretinal nodules randomly scattered in the macula. In later stages, these drusen often become more numerous, with clustered groups of drusen scattered throughout the retina. Postpartum thrombotic microangiopathy with elevated liver enzymes: Distinguishing postpartum HELLP syndrome from atypical hemolytic uremic syndrome.
A simple bedside technique for managing a non-deflating Foley catheter balloon in a resource-limited setting - a case report.
Urethral catheterization is a frequently performed procedure that can sometimes result in complications such as failure to deflate a Foley catheter balloon. This can present significant challenges in managing a patient. We describe a case involving a 37-year-old woman diagnosed with severe cerebral malaria, eclampsia, and HELLP syndrome whose Foley catheter balloon failed to deflate despite the use of standard techniques. An easy bedside technique was used by cutting the catheter to a short length and using a spinal needle stylet to unblock the existing inflation passage, thus successfully deflating the balloon without complications or the need for anesthesia. This method is safe, minimally invasive, cost-effective, and particularly valuable in low-resource environments.
Epidemiological Risk Factor Analysis for Maternal Mortality Associated with Eclampsia: A Single-Center Study from Tertiary Hospital Garut.
Maternal mortality remains a major global health challenge, particularly in low- and middle-income countries. Eclampsia, a severe complication of hypertensive disorders in pregnancy, continues to contribute substantially to maternal and perinatal deaths despite advances in obstetric care. Previous studies have highlighted the role of clinical and laboratory abnormalities in worsening outcomes; however, evidence from developing settings remains limited. A clearer understanding of maternal mortality risk factors in eclampsia is essential to improve early detection, risk stratification, and clinical management. To identify maternal mortality risk factors associated with eclampsia based on patient characteristics, clinical presentation, and laboratory findings. A retrospective observational study was conducted using medical records of pregnant women diagnosed with eclampsia between January 2022 and March 2024 at a tertiary referral hospital. Demographic data, clinical features, and laboratory parameters were extracted systematically. Statistical analysis included univariate and bivariate analyses using the Chi-square test to assess associations between variables and maternal mortality. Multivariate logistic regression analysis was performed to determine independent risk factors and estimate odds ratios (OR) with corresponding p-values. Statistical significance was set at p ≤ 0.05. A total of 112 patients met the inclusion criteria. The eclampsia case fatality rate was 20.54%. Laboratory parameters demonstrated significant associations with maternal mortality. Severe proteinuria (+3) was associated with increased mortality risk (OR = 1.67; p = 0.05). Anemia, defined as hematocrit <35%, showed a strong association with maternal death (OR = 2.25; p = 0.001). Additionally, HELLP syndrome significantly increased the risk of maternal mortality (OR = 2.82). Laboratory abnormalities are key predictors of maternal mortality in eclampsia. Severe proteinuria, anemia, and HELLP syndrome should be promptly identified and managed to reduce adverse maternal outcomes.
Beyond HELLP: An Unusual Cause of Severe Thrombocytopenia in Pregnancy.
Thrombotic thrombocytopenic purpura (TTP) is a rare yet potentially fatal thrombotic microangiopathy that poses significant risks during pregnancy. It is characterized by microangiopathic hemolytic anemia, profound thrombocytopenia, and widespread microvascular thrombosis. We present a case of pregnancy-associated TTP presenting with isolated severe thrombocytopenia, highlighting the diagnostic challenge. A 27-year-old, G3P1 woman at 31+6 weeks of gestation presented with hypogastric pain and a resolved episode of vomiting. Laboratory evaluation revealed severe thrombocytopenia (platelets: 9 × 10⁹/L), mild anemia, and proteinuria. Differential diagnosis included immune thrombocytopenic purpura (ITP), HELLP (hemolysis, elevated liver enzymes, and low platelet count), and TTP. Initial therapy included corticosteroids, platelet transfusions, primarily suspecting ITP as the first differential diagnosis, and magnesium sulfate later after lactate dehydrogenase levels were suggestive of hemolysis. HELLP syndrome being the main differential, counseling was done about the possible need for preterm delivery, and a neonatologist was involved for discussion with the parents. This was followed by urgent ADAMTS13 testing, which confirmed TTP. The patient was transferred to a tertiary care center for plasma exchange therapy, which improved the platelet count. She subsequently underwent induction of labor at 35 weeks for severe preeclampsia, delivering vaginally with postpartum hemorrhage (1.7 L) managed with transfusions. Both maternal and neonatal outcomes were favorable, and both were discharged on day 10 of delivery. This case illustrates the atypical presentation of TTP, where severe thrombocytopenia may occur without classic neurological or renal symptoms. Timely recognition of the correct diagnosis allowed appropriate treatment and prevented unnecessary very preterm delivery.
Hypocholesterolaemia as an early feature of presumed acute fatty liver of pregnancy.
Differentiating between acute fatty liver of pregnancy (AFLP) and haemolysis, elevated liver enzyme and low platelet (HELLP) syndrome may be difficult. The treatment for both conditions is expedited delivery, with the infrequent exception of expectant management with close monitoring and consideration of dexamethasone with HELLP in the setting of severe prematurity. Correct diagnosis may have implications for maternal morbidity and mortality, risk of recurrence, risk to the child, and the risk of long-term maternal disease. The Swansea criteria lack specificity, a significant proportion of women with HELLP fulfilling criteria. Low antithrombin III, elevated circulating soluble Fms-like tyrosine kinase-1, hypocholesterolaemia, hypofibrinogenaemia, and reduced portal vein blood flow velocity have been proposed as differentiating AFLP from HELLP. A case of presumed AFLP associated with hypocholesterolaemia is presented, and the literature regarding markers in the differentiation of AFLP from HELLP is discussed. Hypocholesterolaemia may be useful in differentiating between AFLP and HELLP syndrome, particularly in resource-limited settings.
