A group of congenital heart defects involving the outflow tracts. Examples include truncus arteriosus communis, double-outlet right ventricle and transposition of great arteries. Truncus arteriosus communis is characterized by a single outflow tract instead of a separate aorta and pulmonary artery. In transposition of the great arteries, the aorta arises from the right ventricle and the pulmonary artery from the left ventricle. In double outlet of the right ventricle, both the pulmonary artery and aorta arise from the right ventricle.

Percutaneous retrieval of the lattice-tip catheter entrapped in post-surgical atrioventricular valve apparatus: two case reports.
The novel lattice-tip catheter (Sphere-9, Medtronic) enables high-resolution mapping and the ability to switch between radiofrequency and pulsed electric field. Initial experience has shown promising results for treating complex ventricular tachycardias (VTs). However, safety data remain limited, particularly regarding catheter-valve interactions. We report two cases of mechanical entrapment of the lattice-tip catheter in the atrioventricular valves during VT ablation performed via a retrograde transaortic approach. In both cases, the catheter became entangled within the subvalvular apparatus-specifically, in the tricuspid valve of a patient with congenitally corrected transposition of the great arteries and in the mitral valve of a patient who had previously undergone mitral repair. Standard retrieval manoeuvres were unsuccessful, and resolution required percutaneous bailout strategies, specifically transseptal access with bioptomes or forceps. Both procedures were completed without long-term sequelae. These are the first reported cases of large-footprint catheter entrapment in the atrioventricular valve apparatus, highlighting potential mechanical risks associated with its employment in the ventricular setting. Our experience emphasizes the importance of preferring transseptal access over retrograde access where feasible, due to its more favourable trajectory and the ability to support safer rescue techniques. Real-time imaging, particularly intracardiac echocardiography, proved essential both for navigation and complication management. While transaortic access remains a viable option, it should be approached with caution, necessitating a thorough understanding of device behaviour and a readiness to manage complications. These observations add important safety considerations for the expanded use of novel ablation platforms in patients with complex ventricular anatomy.
Deep Learning-Based Automated Echocardiographic Measurements in Pediatric and Congenital Heart Disease.
Echocardiography (echo) is a cornerstone of pediatric cardiology, yet access to expert interpreters is limited worldwide, particularly in low-resource and rural settings. Artificial intelligence (AI) offers a mechanism to broadly deliver expert-level precision and standardize measurements, yet AI for comprehensive automated measurements in pediatric and congenital heart disease (CHD) echo remains underdeveloped. We created EchoFocus-Measure, an AI platform that automatically extracts 18 quantitative parameters and 10 qualitative assessments from full echo studies. The method extends a multi-task, view-agnostic architecture (PanEcho) with a study-level transformer to prioritize diagnostically informative views. Training (80%) and internal testing (20%) were performed on echos from Boston Children's Hospital (BCH), with external evaluation on outside referral studies. Left ventricular ejection fraction (LVEF) was the primary endpoint. The internal cohort included 11.4 million videos from 217,435 echos (60,269 patients; median age 8.5 years; median LVEF 61%), and external validation encompassed 289,613 videos from 3,096 echos (2,506 patients; median age 3.5 years; median LVEF 62%). For LVEF, EchoFocus-Measure exhibited a median absolute error (MAE) of 2.8% internally and 3.8% externally, maintaining accuracy across infants (MAE 3.2%) and complex CHD lesions (e.g., MAE 4.0% for L-loop transposition of the great arteries). EchoFocus-Measure improved upon the PanEcho benchmark (MAE 7.5% for infants; 13.1% for L-loop transposition). Discrepant case (>50th percentile error) adjudication of LVEF demonstrated that model errors (MAE 2.4%) were within human variability (MAE 3.7%). For qualitative measures, EchoFocus-Measure performed well internally (AUROC 0.88-0.95) and modestly externally (AUROC 0.73-0.86). Explainability analyses highlighted model focus on clinically appropriate echo views for LVEF estimation (apical four-chamber, parasternal short/long) and mitral regurgitation assessment (apical four-chamber color Doppler, parasternal short/long color Doppler). EchoFocus-Measure delivers rapid and reliable automated echo measurements across ages and lesions within diverse internal and real-world external cohorts, serving as a step toward scalable, global access to high-quality pediatric cardiovascular care.
Percutaneous treatment with the 'valve-in-valve' technique in congenitally corrected transposition of the great arteries and systemic atrioventricular valve with severe double lesion: case report.
Congenitally corrected transposition of the great arteries (CCTGA) is a rare congenital heart disease characterized by atrioventricular (AV) and ventriculoarterial discordance, allowing near-normal circulation without early surgical correction. However, long-term complications such as valvular dysfunction, systemic right ventricular (RV) failure, and pulmonary hypertension are common. Management becomes particularly challenging in patients with prior interventions, where anatomical complexity and elevated surgical risk limit therapeutic options. We present the case of a 47-year-old Chinese female diagnosed with CCTGA who had previously undergone surgical implantation of a bioprosthetic valve in the systemic AV position in her home country. She presented with significant deterioration in functional capacity, and imaging revealed severe dysfunction of the surgical prosthesis, characterized by severe mixed valvular disease, aneurysmal dilation of the left atrium, and severe systemic RV dysfunction. A multidisciplinary team assessment determined that the patient was at high surgical risk, leading to the decision to perform a percutaneous valve-in-valve implantation. An Edwards SAPIEN® 26 mm bioprosthetic valve was successfully implanted, yielding excellent haemodynamic results without complications. The patient was discharged 24 h post-procedure with significant clinical improvement. Although percutaneous interventions for systemic tricuspid valve disease have been described, they typically involve native valves and remain anecdotal. To date, no published reports have documented valve-in-valve implantation in a systemic AV prosthesis in a patient with CCTGA. This case illustrates the feasibility and safety of such an approach in a high-risk patient, offering a valuable alternative to surgical replacement and potentially delaying the need for heart transplantation.
Anaesthetic management of a paediatric patient with situs inversus totalis and congenitally corrected transposition of the great arteries undergoing adenotonsillectomy.
Situs inversus totalis (SIT) with congenitally corrected transposition of the great arteries (ccTGA) represents a rare and complex congenital combination that significantly complicates anaesthetic management. We report the anaesthetic management of an early childhood (2-5 years), 13 kg child diagnosed with SIT and ccTGA undergoing elective adenotonsillectomy. Individualised perioperative strategies were implemented, including modified ECG monitoring, arrhythmia preparedness and explicit mg/kg drug dosing. Intraoperative management was uneventful, and the patient had a stable recovery. This case highlights anaesthetic considerations and monitoring adaptations in non-cardiac paediatric surgery involving mirror-image congenital cardiac anatomy.
Postoperative cerebral oxygen availability and neurodevelopment in children with d-transposition of the great arteries.
The etiology of neurodevelopmental impairments in children with d-transposition of the great arteries (TGA) is incomplete. We aimed to assess the association between postoperative cerebral oxygen availability and neurodevelopment in these children. Neonates with TGA and a ventricular septal defect (TGA-VSD, n = 11) or an intact ventricular septum (TGA-IVS, n = 19) and postmenstrual age-matched healthy controls (n = 19) underwent bedside optical neuromonitoring within 72 h post-surgery and 72 h post-birth, respectively. Indices of cerebral blood flow (CBFi) and oxygen delivery (CDO2i) were derived from neuromonitoring signals. 2-year neurodevelopmental outcome was evaluated using the Bayley Scales of Infant and Toddler Development 3rd/4th Ed. Neonates with TGA had lower CDO2i than controls. In children with TGA, lower postoperative CBFi and CDO2i were associated with lower motor composite, receptive communication and gross motor scores. When adjusted for sex and Bayley version, associations with receptive communication were stronger. In the TGA-IVS subgroup, CBFi and CDO2i were associated with all motor scales and most of these associations were stronger when adjusted. No associations were significant in the TGA-VSD subgroup. Although the modest cohort size and exploratory nature of subgroup results, postoperative CBFi and CDO2i may act as early biomarkers of brain maturity and future neurodevelopmental risk in children with TGA.
