Long-Term Outcomes and Natural Course of Giant Cerebral Arteriovenous Malformations (> 6 cm): Insights From a National Multicenter Propensity Score-Matched Cohort.
This study evaluated the natural history of giant cerebral arteriovenous malformations (AVMs) over 6 cm and compared long-term outcomes of interventional treatment versus conservative management. Patients with AVMs > 6 cm were identified from a national multicenter prospective registry (MATCH). Rupture risk factors were analyzed with uni- and multivariate models. Propensity score matching balanced intervention and conservative groups. The primary outcome was long-term hemorrhagic stroke or death; secondary outcomes included obliteration rates and neurological status. Subgroup and sensitivity analyses assessed robustness. From August 2011 to December 2021, 380 patients with giant AVMs were enrolled. Annual rupture risk was 2.4% for unruptured lesions, 9.4% for previously ruptured, and 3.6% overall. Ventricular involvement (OR = 3.61) and draining vein stenosis (OR = 2.61) were independent hemorrhage risk factors. Over a mean follow-up of 7.3 years, intervention did not significantly reduce hemorrhagic stroke or death compared to conservative management but was associated with higher hemorrhagic stroke risk (HR = 2.04) and neurological deterioration, despite higher obliteration rates (39.1%). Stratified analysis suggested microsurgery and embolization alone were less favorable, while embolization plus radiosurgery more effectively reduced hemorrhagic stroke or death. Subgroup analysis indicated conservative management was preferable for higher S-M grades, unruptured AVMs, and eloquent brain regions. The annual rupture rate of giant AVMs (> 6 cm) is approximately 3.6%. Interventional treatment for giant AVMs is not superior to conservative management, and the risks of hemorrhagic stroke and neurological deterioration remain substantial-particularly for unruptured, high-grade, or eloquently located AVMs.
Surgical management of a lenticulostriate artery aneurysm and an associated initially less-appreciated arteriovenous malformation: two-dimensional video.
Surgical management of the basal ganglia vascular lesions needs a personalized surgical strategy due to intricate neurovascular anatomy and eloquent surrounding structures. This report describes the successful resection of a perforator aneurysm and an initially less-appreciated arteriovenous malformation (AVM) at the basal ganglia in a 15-year-old male. Preoperative digital subtraction angiography (DSA) and high-resolution magnetic resonance imaging (MRI) showed a deep-seated basal ganglia aneurysm on a lenticulostriate artery (LSA) perforator with a distal suspicious early drainage vein adjacent to a hematoma. After a thorough discussion, a trans-Sylvian/trans-insular microsurgical obliteration of the aneurysm and exploration of the suspected micro-AVM utilizing the hybrid operating room was planned for this patient, and informed consent was obtained [1-5]. After craniotomy, the Sylvian fissure was sharply dissected to expose the insular lobe. Subsequently, the hematoma cavity was opened to expose and clip the aneurysm under the guidance of the neuronavigation system and intraoperative neurophysiological monitoring (IONM). After isolating the aneurysm, the micro-AVM distal to the aneurysm was clearly revealed in the intraoperative DSA review. With stable IONM over a 30-minute clip occlusion of the aneurysm and the parent artery, both the AVM and the aneurysm were resected. Repeat intraoperative DSA confirmed complete obliteration of both lesions, and no IONM changes (motor/somatosensory evoked potentials) occurred. Postoperative monitoring addressed hemorrhage, vasospasm, and neurological deficits. At 6-month follow-up, DSA and MRI showed sustained obliteration and sequela-free recovery. We hope that this case can underscore the surgical nuances and tricks in rare LSA lesions within the basal ganglia.
Translational relevance of animal models available on brain arteriovenous malformations, a systematic review.
We conducted a systematic review to identify and evaluate animal models of true brain arteriovenous malformations (bAVMs), focusing on how they replicate human disease. Outcomes assessed included bAVM growth, rupture, seizures, and survival. The review adhered to PRISMA guidelines and SYRCLE's protocol for animal studies. A search on PubMed and Embase was conducted (April 19, 2023; updated October 24, 2024). Exclusion criteria were; (1) Not an original, peer-reviewed full length research article, (2) not an in vivo study, (3) no bAVM induction, (4) no appropriate control group, (5) no histological or anatomical assessment during the bAVM follow-up, (6) outcome of the bAVM not reported. Meta analyses were planned for all key parameters. Risk of bias and image duplication assessments were conducted. Forty-one studies could be included in this SR. Models primarily involved mice and targeted mutations in MAPK, TGFbeta and Notch related signaling. Study designs varied significantly, limiting meta-analysis and direct comparison. We often noted high risk of bias in studies' reporting. Many studies had high risk of bias and focused on HHT-related mutations, which represent only a minority of clinical bAVM cases. KRAS-based models may offer better clinical relevance, but overall, current bAVM models show substantial variability.
Mutant KRAS in brain endothelial cells promotes vascular inflammation and impairs vascular integrity in brain arteriovenous malformation.
Somatic KRAS (KRASG12V) mutation in endothelial cells (EC) induces brain arteriovenous malformation (bAVM) that could lead to vascular instability and ultimately bleeding. However, the causes of bAVM instability remain unclear. Here we demonstrate that KRASG12V expressing cultured ECs (KRAS-G12V-EC) have increased expression of pro-inflammatory mediators and reduced expression of blood-brain-barrier (BBB) junction constituents. The conditioned medium from KRAS-G12V-EC can activate BV2-microglia (BV2-MG) and conditioned media from this primed BV2-MG can compromise the expression of EC-junction constituents when added to wild-type ECs. In an in vitro BBB model, KRAS-G12V-EC form a dysfunctional EC barrier that is further disrupted, leading to lower transendothelial electrical resistance and increased FITC-dextran leakage when blood-derived macrophages (Mφ) are included. KRAS-G12V-EC potently stimulates BV2-MG chemotaxis. In vitro BBB leakage and BV2-MG chemotaxis were inhibited by treatment with the anti-inflammatory drug minocycline in the KRAS-G12V-EC. In our bAVM mouse model that uses AAV-BR1-KRASG12V injection to produce EC expression of KRASG12V in the brain, minocycline injection reduced production of inflammatory cytokines by bAVM nidus, reduced leakage of BSA-647, and restored VE-cadherin expression on malformed vessels. Our findings suggest that KRAS-G12V-EC can activate local MG/Mφ that causes vascular inflammation and instability of malformed vessels in bAVM.
Microsurgical resection of brain arteriovenous malformations with or without preoperative embolization: a 10 year observational cohort study.
The role of preoperative embolization as an adjunct to microsurgical resection of brain arteriovenous malformations (AVMs) remains uncertain. We evaluated clinical and angiographic outcomes of microsurgical resection performed with or without preoperative embolization and examined factors associated with selection for embolization. We conducted a retrospective observational cohort study of consecutive patients undergoing microsurgical resection of AVMs at a tertiary centre between July 2015 and June 2025. Patients treated with microsurgery alone or with preoperative embolization were included. Baseline differences were addressed using propensity score methods, including 1:3 matching and inverse probability of treatment weighting. The primary outcome was treatment related morbidity at 90 days, defined as an increase of at least 1 point on the modified Rankin Scale (mRS). Secondary outcomes included functional dependence (mRS greater than 2), postoperative haemorrhage, parenchymal infarction, angiographic obliteration, operative duration and composite outcome of mRS deterioration of at least 2 points or death at 90 days. Factors associated with embolization use were evaluated with Firth penalized logistic regression, and discriminative performance of AVM size was assessed using receiver operating characteristic analysis. Among 104 patients, 91 (87.5%) underwent microsurgical resection alone and 13 (12.5%) underwent preoperative embolization followed by microsurgery. Embolized AVMs were larger (mean 3.8 vs 2.5 cm), more frequently demonstrated deep perforator supply, and were of higher Spetzler-Martin grade. After propensity score adjustment, no clear differences were observed in treatment related morbidity at 90 days (risk difference + 23.1%, 95% CI - 3.3 to 51.9), functional dependence (risk difference + 20.5%, 95% CI - 8.5 to 49.7), the composite outcome of mRS deterioration of at least 2 points or death, or mortality. AVM size was independently associated with embolization use (odds ratio 1.77 per cm increase), with moderate discriminative performance (AUC 0.76). A 3 cm threshold demonstrated the strongest association with embolization selection. In this single centre observational cohort, microsurgical resection with or without preoperative embolization was associated with similar short term functional and angiographic outcomes after adjustment for baseline differences, although estimates were imprecise. AVM size was the primary factor associated with selection for embolization, but this did not correspond to a demonstrable outcome advantage. These findings reflect contemporary practice patterns and underscore the need for prospective multicentre studies to clarify whether embolization confers incremental benefit in selected patients.
