Differences in Right Hepatic Duct Length Among Left-Sided Hepatectomies: Conventional Hepatectomy Versus Extended Hepatectomy Versus Trisectionectomy.
Left-sided hepatectomy for perihilar cholangiocarcinoma includes conventional left hepatectomy (C-LH; H1234-B), extended left hepatectomy (E-LH; H12345'8'-B-MHV), and left trisectionectomy (LT; H123458-B). The anatomical characteristics of the resected length of the right hepatic duct (RHD) in E-LH remain unclear. This study aimed to characterize the length of the RHD across procedures. Patients who underwent left-sided hepatectomy for perihilar tumors between 2015 and 2023 were retrospectively reviewed. The shortest distance between the proximal bile duct stump and left hepatic duct orifice was measured on the resected specimens. The lengths and clinicopathological features of the procedures were compared. In total, 205 patients were included: C-LH (n = 80), E-LH (n = 53), and LT (n = 72). The length of the right anterior hepatic duct was longer in E-LH than that in C-LH (15.2 vs. 13.0 mm, p = 0.006). Similarly, the length of the right posterior hepatic duct increased stepwise from C-LH to E-LH and LT (13.3, 16.6, and 20.0 mm, respectively). E-LH is an intermediate procedure between C-LH and LT with respect to the resected length of the RHD. The additional bile duct length achieved by E-LH is minimal, and this procedure should be selected primarily to secure the parenchymal margin rather than ductal clearance.
Long-term survival after multidisciplinary treatment including durvalumab for recurrent perihilar cholangiocarcinoma.
Perihilar cholangiocarcinoma is an aggressive malignancy with a poor prognosis even after curative resection, and recurrence is common. Systemic chemotherapy has long been the standard treatment for recurrent disease, but durable survival is rare. Recently, immune checkpoint inhibitors (ICIs) have been incorporated into first-line regimens for advanced biliary tract cancer, showing promising efficacy. We report a 61-year-old man who underwent extended left hepatectomy with caudate lobectomy, bile duct resection, and portal vein reconstruction for stage IVA perihilar cholangiocarcinoma. Twelve months later, para-aortic lymph node recurrence was detected and treated with gemcitabine plus cisplatin (GC), followed by S-1 and proton beam therapy for local recurrence. At 41 months post‑diagnosis, new intrahepatic metastases developed, and GC‑durvalumab (GCD) therapy was initiated. All lesions achieved radiological complete response within 2 years, with only grade 1 skin toxicity. The patient remains alive and well at 65 months after surgery and 53 months after recurrence, with sustained disease control. This case highlights the potential role of ICIs within multidisciplinary treatment strategies, demonstrating that even recurrent perihilar cholangiocarcinoma may achieve clinical complete response and long-term survival.
The rise of transplant oncology in primary liver cancer and metastatic disease.
Liver transplantation has evolved from a treatment restricted to patients with end-stage liver disease to a therapeutic option for selected patients with primary and metastatic liver malignancies. This review explores the rapidly expanding field of transplant oncology, highlighting its role in hepatocellular carcinoma, colorectal liver metastasis, neuroendocrine liver metastasis, and intra-hepatic and perihilar cholangiocarcinoma. Emphasis is placed on strategies to broaden eligibility, optimize donor organ use, and improve oncologic outcomes. We synthesized current evidence from clinical series, registries, and experimental protocols to evaluate patient selection criteria, outcomes, and peri-transplant management. Key topics include downstaging approaches to meet transplant criteria such as locoregional therapies and systemic regimens, and their prognostic implications. Advances in donor utilization have been analyzed for their capacity to expand the graft pool. Additionally, the review addresses the integration of oncologic principles into immunosuppression regimens, balancing graft protection with cancer control. Collectively, the reported studies demonstrate improved survival and reduced recurrence when stringent selection and multimodal therapy were applied. Transplant oncology reshapes the therapeutic landscape of liver malignancies, with growing evidence supporting liver transplantation in carefully selected patients beyond traditional indications. Optimized downstaging, innovative donor strategies, and tailored immunosuppression are pivotal for safe expansion. Continued collaboration between the transplant and oncology disciplines, along with prospective trials, is essential to further define standardized protocols and solidify transplantation as a cornerstone of multidisciplinary cancer care.
Accuracy of automated 3D biliary tract reconstruction compared to ERCP to assess Bismuth-Corlette classification in patients with perihilar cholangiocarcinoma.
Accurate descriptions of the extent of disease in intrahepatic bile ducts are essential for managing patients with perihilar cholangiocarcinoma (pCCA). This study describes and assesses a new, automated Computed Tomography (CT)-scan-based 3D reconstruction process (3DR) of biliary tree in these patients compared with magnetic resonance cholangiopancreatography (MRCP) and endoscopic retrograde cholangiopancreatography (ERCP). We studied consecutive patients with histologically proven hilar cholangiocarcinoma, who were referred for biliary drainage by ERCP in our center between 2015 and 2023. Automated reconstruction of bile ducts from CT images was performed using an artificial intelligence software (Synapse 3D 6.7, Fujifilm, Tokyo, Japan). We compared the Bismuth-Corlette classification assessed by ERCP (gold-standard), MRCP, CT-scan, and 3DR by computing exact percentage and weighted Kappa agreements. We included 62 patients. As compared with ERCP, CT-scan without 3DR had an exact agreement of 48% and had a moderate weighted Kappa agreement (weighted Kappa 0.47; 95%CI: 0.27-0.67). The MRCP had an exact agreement of 52% and a moderate weighted Kappa agreement (weighted Kappa 0.57; 95%CI: 0.39-0.76). The 3DR had an exact agreement of 60%, and a strong weighted Kappa agreement (weighted Kappa 0.72; 95%CI: 0.60-0.85). 3DR is highly concordant with ERCP in assessing the extent of pCCA.
Gross Intrahepatic Mass Formation Predicts the Primary Site of Perihilar Cholangiocarcinoma Based on Molecular Pathologic Studies.
Intrahepatic cholangiocarcinoma (iCCA) and extrahepatic cholangiocarcinoma (eCCA) are clinically and genetically distinct. However, the classification of perihilar cholangiocarcinoma (phCCA) with an intrahepatic tumor mass remains unclear. This study aimed to position phCCA near the hilar plate (hCCA) within an extrahepatic-intrahepatic framework using pathological and molecular analyses. Among 357 resected invasive CCAs, 100 hCCAs were histologically classified as either hCCA with (hCCA-M) or hCCA without (hCCA-NM) a grossly evident intrahepatic mass. Transcriptomic comparison of 9 typical eCCAs and 39 mass-forming iCCAs identified three contextual markers, which were examined by immunohistochemistry in 309 additional cases. Among 100 hCCAs, 85 were hCCA-NM and 15 hCCA-M. Claudin 18 (CLDN18) and mesothelin (MSLN) were identified as extrahepatic contextual markers, and serpin family A member 1 (SERPINA1) as an intrahepatic contextual marker. SERPINA1 was more highly expressed in hCCA-M than in hCCA-NM, regardless of microscopic liver parenchymal invasion, whereas CLDN18 and MSLN were similarly expressed in both. Cluster analysis revealed that hCCA-NM clustered with eCCA, whereas hCCA-M clustered with iCCA. Gross intrahepatic mass formation indicates an intrahepatic contextual profile and provides a useful criterion for subclassifying hCCA. This contextual framework shows that hCCA-M and hCCA-NM represent biologically distinct tumor groups.
