Towards achieving immune reset in autoimmune cytopenias: Insights from clinical experiences with B cell depleting therapies.
Autoimmune cytopenias (AICs) arise from pathogenic autoantibody-mediated destruction of blood cells. Current treatments often fail to achieve durable remission, necessitating long-term treatment including immunosuppression and exposure to treatment-related toxicities. Insights into B cell biology demonstrate the central role of autoreactive B cells and plasma cells in sustaining disease activity and relapse, providing a rationale to achieve sustained, treatment-free remissions through "immune reset". This review summarizes the current understanding of immune reset in AICs and examines clinical data for depletion strategies targeting distinct stages of B cell maturation. We compare therapeutic modalities across monoclonal antibodies, bispecific T cell engagers (TCE), and chimeric antigen receptor (CAR) T cells. The depth and breadth of depletion, kinetics of immune reconstitution, and treatment-related risks are critical determinants of long-term outcome potential and individualized risk-benefit assessment. Collectively, these advances support a potential paradigm shift from chronic immunosuppression towards durable remissions off therapy with time-limited therapeutic interventions.
Measurement of haemolytic anaemia in a novel treatment response measure for SLE clinical trials.
To define a clinically meaningful treatment response definition for haemolytic anaemia, that will form part of a novel multidomain outcome measure for SLE clinical trials. An international working group comprising 12 clinicians and 4 patients completed a multistep consensus process. First, the scope of haemolytic anaemia for the SLE clinical trial context was defined. Candidate response measures were then nominated via an online survey and shortlisted for evaluation via a systematic literature review (SLR) focussing on relevant measurement properties and clinical trial utility. Informed by the SLR results, the working group used nominal group technique (NGT) to reach consensus on a response measure, as well as entry, clinically meaningful response and complete response thresholds. The domain scope was unanimously agreed on as 'immune-mediated haemolytic anaemia attributed to active lupus that impacts the patient and is modifiable by therapy to reduce or control disease activity', including Coombs' positive autoimmune haemolytic anaemia and rarer subtypes. Six laboratory measures to fit this scope were shortlisted for the SLR, which screened 1519 articles, of which 10 were included. Consensus via NGT was reached on haemoglobin (Hb) improvement to measure haemolytic anaemia treatment response. Thresholds were defined for entry (Hb <100 g/L with laboratory confirmation of haemolysis), minimum clinically meaningful response (Hb improvement >20 g/L or Hb normalised with ongoing haemolysis) and complete response (Hb normalised without haemolysis). Hb, with meaningful thresholds for entry and improvement, will be adopted to classify haemolytic anaemia treatment response in a novel multi-domain outcome measure for SLE clinical trials.
Autoimmune hemolytic anemia: New frontiers in diagnosis and therapy.
Autoimmune hemolytic anemias comprise warm AIHA (wAIHA), due to IgG autoantibodies reacting at body temperatures, and cold agglutinin disease (CAD), due to IgM reacting in cold and strongly activating complement. The diagnosis, based on the direct antiglobulin test (DAT), is sometimes difficult since the DAT is performed with various methods with different sensitivity and specificity. Novel investigation includes DAT by flow-cytometry that helps the diagnosis of DAT-negative forms and in-deep bone marrow examination that aims to understand AIHA pathogenesis. Therapy is different in wAIHA and CAD: in the former, steroids represent the first-line, followed by rituximab, splenectomy and immunosuppressors, while in the latter rituximab is recommended frontline, followed by rituximab plus chemotherapy or the complement inhibitor sutimlimab. Plasmacell-directed therapies (bortezomib and daratumumab) may be administered in refractory patients. Several new therapies are under investigation, including inhibitors of Bruton's and spleen tyrosine kinases, neonatal Fc receptor, and cytokines, and bi-specific monoclonal antibodies.
Warm autoimmune hemolytic anemia in adults in Latin America: A scoping review.
Autoimmune hemolytic anemia is a rare disorder characterized by the autoimmune-mediated destruction of red blood cells. The condition is classified into subtypes based on the thermal reactivity of the autoantibodies, with warm autoimmune hemolytic anemia representing the most common form in adults. This review aimed to summarize the clinical evidence regarding warm autoimmune hemolytic anemia generated in adult patients within the Latin American region. A literature search was conducted in Embase, Medline, Lilacs, Cochrane Library, Epistemonikos, and Value in Health in April 2023. Gray literature was also consulted. Records were eligible for inclusion if they presented data on the epidemiology, diagnosis, treatment, or healthcare resource utilization among adult warm autoimmune hemolytic anemia patients in Latin American countries. Clinical practice guidelines on the condition issued in Latin American were also eligible. Nine records were included: seven retrospective studies (six single-center), one national clinical guidelines (Mexico), and one therapeutic protocol (Brazil). Corticosteroids are the cornerstone of first-line treatment, reported in 97.7 % of 354 patients enrolled in the studies, primarily as monotherapy (91 %). While response rates were high (69-84 %), a considerable proportion of patients (15-40 %) required second-line therapies. The use of splenectomy varied widely between studies, from being the preferred second-line option to not being performed in any patient. Access barriers contribute to a limited use of rituximab. Compared to other hemolytic disorders, warm autoimmune hemolytic anemia studies are lacking in Latin American and consist primarily of single-center, retrospective case series. Further research is warranted to understand management strategies in refractory disease.
Therapeutic plasma exchange as a rescue and bridging therapy for life-threatening autoimmune hemolytic anemia associated with angioimmunoblastic T-cell lymphoma.
Autoimmune hemolytic anemia (AIHA) is a potentially life-threatening complication of lymphoid malignancies, and transfusion management can be particularly challenging in refractory cases. We report a case of angioimmunoblastic T-cell lymphoma complicated by severe, transfusion-refractory warm AIHA, in which red blood cell transfusion was paradoxically associated with a hyperhemolysis-like process and overt disseminated intravascular coagulation. Despite corticosteroid therapy and rituximab, repeated transfusions failed to produce sustained hemoglobin increments and were followed by worsening hemolysis and coagulopathy. Detailed immunohematologic evaluation demonstrated panreactive IgG autoantibodies with persistently positive direct antiglobulin testing and elevated polyethylene glycol-enhanced indirect antiglobulin test reactivity. Therapeutic plasma exchange was initiated as a rescue intervention, resulting in a progressive reduction in circulating autoantibody reactivity, restoration of transfusion efficiency, and stabilization of hemoglobin levels, despite persistent direct antiglobulin test positivity. Following effective lymphoma-directed chemotherapy, hemolysis resolved completely, and transfusion independence was achieved. This case highlights post-transfusion deterioration resembling hyperhemolysis in lymphoma-associated AIHA and underscores the potential role of therapeutic plasma exchange as a rescue and bridging therapy in life-threatening, transfusion-refractory disease.
