Resolution of penile calciphylaxis in a high-risk dialysis patient with medical therapy and wound care.
Penile calciphylaxis is a rare but life-threatening manifestation of calcific uraemic arteriolopathy, most often seen in patients with end-stage renal disease (ESRD) on dialysis. Its diagnosis is frequently delayed, and its presence carries a poor overall prognosis. Given its rarity, treatment strategies and their efficacy have been understudied and may include observation, medical therapy, radical surgery or hyperbaric oxygen therapy. In this case, we describe a patient with ESRD presenting with a rapidly progressive penile lesion ultimately diagnosed as penile calciphylaxis. Our patient was ineligible for hyperbaric oxygen therapy and declined surgical management. Consequently, unlike many reported cases requiring surgical management nonetheless resulting in poor outcomes, our patient experienced lesional resolution with conservative therapy, specifically intravenous sodium thiosulfate and structured wound care with medical-grade honey, within 9 months after presentation. This case highlights a novel non-surgical approach to a difficult clinical situation and shows the potential efficacy and novelty of non-surgical management in select patients where penile calciphylaxis is diagnosed early in its course.
Lived experiences of patients with calciphylaxis (CALCI-E): a qualitative study.
Calciphylaxis is a rare, severe condition, mainly affecting people undergoing dialysis. It occurs when small arterioles within the skin become calcified causing painful skin lesions. There are currently no diagnostic tests or licensed treatments. This qualitative study explored patient experiences to identify opportunities to improve diagnosis and management of this challenging condition. Participants were recruited between October 2023 and December 2024. A sample of 10-20 people was sought. Participants were eligible to take part if they lived in England or Wales and were over 18 years of age, had capacity to consent, and had a current or previous (<3 years) diagnosis of calciphylaxis, or were a caregiver (or relative) of someone with calciphylaxis. Interviews were recorded, transcribed and anonymised, and analysed using inductive, reflexive thematic analysis. Seventeen participants were recruited (14 patients, median age 68 years, 43% male; 3 relatives). Six overarching themes were identified. 1 Diagnostic Delays and Awareness Gaps; symptoms were often dismissed; 2 Pain and Symptom Experience; pain was frequently poorly managed; 3 Treatment and Medication Burden; severe side effects and very limited treatment explanation; 4 Wound Care Variability; often negative experiences and inconsistent practices; 5 Support and Psychological Impact; lack of emotional and practical support; 6 Patient Recommendations; better communication, education, and knowledge sharing. This first in-depth thematic analysis of the experiences of patients who developed calciphylaxis identified key priorities: improved patient/clinician education, better pain and wound management, and enhanced psychosocial and peer support. These findings should inform clinical pathways, educational tools, and support services, with further co-produced research to assess their impact. The study highlights the value of the patients' voices, their willingness to engage, and the need for patient experience measures in calciphylaxis and other rare diseases.
[Challenges of ongoing VKA use in calciphylaxis: The potential of multimodal treatment including rheopheresis, — a case report].
We report two cases of complete remission of uremic calciphylaxis in hemodialysis patients despite continued use of vitamin K antagonists (VKAs). These cases highlight the potential of a multimodal treatment approach including rheopheresis as a promising therapy when VKA discontinuation is not feasible. Both patients presented with severe calciphylaxis while on VKAs for mechanical mitral valve replacement. They received intensive multimodal treatment, including daily hemodialysis, intravenous sodium thiosulfate, calcimimetics, phosphate binders, opioid analgesics, and plastic surgery evaluation. In addition, rheopheresis was initiated twice weekly for four months, during which VKAs were maintained. Rheopheresis frequency was then gradually reduced. Complete wound healing was achieved within seven months. Both patients subsequently underwent parathyroidectomy for tertiary hyperparathyroidism. To our knowledge, these are the first reported cases of full remission of calciphylaxis without VKA discontinuation. A multimodal, multidisciplinary treatment strategy may represent an effective approach for high-risk anticoagulated patients. Nous rapportons deux cas de rémission complète de calciphylaxie urémique chez des patients hémodialysés chroniques, malgré la poursuite d’un traitement par antivitamines K (AVK). Ces cas soulignent l’intérêt d’un traitement multimodal, incluant la rhéophérèse comme thérapie prometteuse lorsque l’arrêt des AVK est impossible. Les deux patients ont présenté une calciphylaxie urémique sévère sous AVK en raison d’une valve mécanique mitrale. Ils ont bénéficié d’un traitement multimodal intensif, incluant une hémodialyse quotidienne, du thiosulfate de sodium intraveineux, des calcimimétiques, des chélateurs du phosphate, des analgésiques opioïdes et une évaluation en chirurgie plastique. Un traitement par rhéophérèse a été initié deux fois par semaine pendant quatre mois, période pendant laquelle les AVK ont été maintenus. La fréquence des séances de rhéophérèse a ensuite été progressivement réduite. La guérison complète des plaies a été obtenue en sept mois. Les deux patients ont ensuite bénéficié d’une parathyroïdectomie devant une hyperparathyroïdie tertiaire. À notre connaissance, il s’agit des premiers cas rapportés de rémission complète de calciphylaxie urémique sans interruption des AVK. Cette stratégie de traitement multimodal et multidisciplinaire pourrait représenter une approche efficace chez les patients anticoagulés à haut risque.
Wound Management and Surgery for Calcific Uremic Arteriolopathy (Calciphylaxis): A Retrospective Observational Cohort Study.
Wound care in calciphylaxis remains poorly defined without evidence-based consensus on timing and technique of surgical intervention. We demonstrate that surgical debridement and subsequent wound closure are safe and effective in calciphylaxis and describe a systematic multidisciplinary approach to intervention. We retrospectively reviewed a cohort of patients with calciphylaxis at our institution. Those who underwent surgical debridement and wound closure were analysed with emphasis on wound healing, progression to amputation and mortality. Sixty-two patients with calciphylaxis underwent surgical debridement. Twenty patients had wound closure by skin grafting, five were excised with primary wound closure, and 37 were debrided and allowed to heal by secondary intention. There were excellent rates of healing in all groups, and no patients demonstrated wound progression or new lesions following operative intervention. Surgical debridement and wound closure are safe and effective in treating wounds related to calciphylaxis.
Postparathyroidectomy calciphylaxis in a patient with primary hyperparathyroidism: a rare complication.
Calciphylaxis, a rare and life-threatening condition involving calcification of small to medium-sized arterioles in subcutaneous tissue, leads to ischemia and necrosis. While typically seen in chronic kidney disease (CKD), a small proportion of cases can present without CKD, referred to as nonuremic calciphylaxis (NUC). NUC associated with primary hyperparathyroidism is rare with limited evidence-based management. A 41-year-old woman with no prior medical history presented with cardiac arrest due to severe hypercalcemia of 17.87 mg/dL (4.46 mmol/L) (normal reference range, 8.6-10.3 mg/dL; 2.2-2.6 mmol/L) and markedly elevated parathyroid hormone (PTH) levels. She required intensive care and extracorporeal membrane oxygenation (ECMO). Following stabilization, she underwent emergency parathyroidectomy for a parathyroid adenoma. During her prolonged recovery over 100 days, she developed painful necrotic skin lesions. Histopathology confirmed calciphylaxis. Management was conservative, involving wound care and analgesia, without surgical debridement. Her skin lesions resolved, and she has remained free of recurrence with stable calcium and PTH levels for more than 10 years. NUC following parathyroidectomy for primary hyperparathyroidism is rare. Multifactorial causes could contribute to the pathogenesis of calciphylaxis in our case, involving acute kidney injury, ECMO, and prolonged immobility. Early recognition and multidisciplinary management are crucial given the high mortality rate in calciphylaxis.
