Case Report: Spinal cord stimulation for neuropathic pain from spinal cysticercosis.
Spinal cord cysticercosis, an exceptionally rare form of neurocysticercosis (NCC), often leads to refractory neuropathic pain and neurological deficits. Current treatments (e.g., antiparasitics or decompressive surgery) may fail to alleviate symptoms, necessitating alternative strategies. Spinal cord stimulation (SCS) is well-established for chronic pain but has rarely been reported for NCC-related pain. A 69-year-old female presented with an 8-year history of progressively worsening trunk and limb pain (burning sensation, VAS 8/10), sensory abnormalities, and lower limb weakness (MMT 3/5). She had undergone laminectomy for spinal cysticercosis in South Korea, but pain persisted post-operatively. MRI revealed residual cystic lesions with arachnoiditis at T12-L2, consistent with inactive spinal cysticercosis. Electromyography confirmed mixed sensorimotor polyneuropathy. After multidisciplinary evaluation, a percutaneous SCS electrode was implanted at T10. Intraoperative testing achieved 80% pain coverage. Parameters were titrated post-operatively (frequency: 40 Hz, pulse width: 300 μs). At the 1-week follow-up, the patient reported significant pain relief, with her visual analog scale score dropping to 2 out of 10. Motor strength improved to grade 4 out of 5 on manual muscle testing, and sensory function returned to normal. These benefits persisted at the 3-month follow-up, accompanied by a 75% reduction in opioid requirements, measured in morphine milligram equivalents. SCS may be a viable option for spinal cysticercosis-induced central neuropathic pain when conventional therapies fail. Its dual benefits (pain relief + functional recovery) warrant further study in NCC-related complications. Early SCS intervention could prevent chronic disability in similar cases.
Taenia solium neurocysticercosis: Its current epidemiological, diagnostic, therapeutic, and control landscapes.
Neurocysticercosis is the most common helminthic parasitic disease affecting the human central nervous system and is pleomorphic in its presentation. It is frequently encountered in daily practice in most parts of the world, and also commonly seen in industrialized countries in immigrant populations. In the past decade, new treatment (combined anti-parasitic drugs, increased attention to reducing treatment-associated inflammation and damage, new surgical strategies), and diagnostic (more specific antigen and antibody detection concepts and tools, more sensitive magnetic resonance imaging sequences) approaches, new animal models, and data on control of transmission have emerged. Still, diagnostic challenges persist and treatment approaches for some types of disease may differ, affecting clinical practice. This review provides clinicians in endemic and non-endemic countries with a comprehensive and practical reference to understand the variabilities in clinical expression of the disease and the optimal diagnostic and treatment approaches.

Neurocysticercosis is the result of the interplay between parasites, hormones, and inflammation.
Parasitic infections of the central nervous system (CNS) are still a health problem in low- and middle-income countries and due to international travels and migration, can affect even opulent countries. Parasites can modulate host immunity to ensure their long-term survival without inflammatory symptoms, therefore infection of the brain parenchyma by Taenia solium cysticerci (parenchymal neurocysticercosis) may be asymptomatic, or on the other hand lead to severe inflammation that initiates glial activation with production of pro-inflammatory cytokines, with mild to severe neurological symptoms. Besides, when cysticerci are localized in the ventricles or in the subarachnoid space (extraparenchymal neurocysticercosis) severe symptoms and treatment difficulties occur. Here we review how the nervous, the immune and the endocrine systems interact with Taenia solium and crassiceps cystcerci and influence the course and individualities of the disease that the parasites triggers in humans and animals. In example, a clear sexual dimorphism in cysticercosis has been found in experimental models of the disease, but the influence of gender in human patients needs further investigation. The role of neurosteroids in parenchymal NCC is intriguing and could influence parasite survival and in the inflammatory reaction they cause. The synthesis of steroids by the brain cells and the cysticerci might influence the local immune response elicited by the presence of the parasites. Remarkably, cultured cysticerci can synthesize sex steroids and deoxycorticosterone (DOC) suggesting that these parasites can influence the local brain microenvironment, especially by producing dehydroepiandrosterone (DHEA), estrogens and androgens which could modify the immune response. Treatment of neurocysticercosis includes cysticidal drugs and corticosteroids intended to control the inflammation caused by the parasite, however, these steroids represent a double-edged tool due to their side effects during prolonged administration and also because of some beneficial effects for the parasite itself.
Global burden shift from infectious to non-communicable neurological disorders in adolescents and young adults (10-24 years): findings from the global burden of disease study 1990-2021.
Neurological disorders are a leading cause of disability and death worldwide; however, their age- and sex-specific patterns among adolescents and young adults remain poorly characterized. Understanding these trends is essential for developing targeted health strategies and optimizing pharmacotherapy in this population. This study aimed to assess global trends and sex-specific patterns in the burden of neurological and neurodevelopmental disorders among individuals aged 10-24 years from 1990 to 2021, providing evidence to inform clinical pharmacy practice and global health policies. Data for 34 neurological conditions among individuals aged 10-24 years were extracted from the 2021 Global Burden of Disease database. Age-standardized prevalence and disability-adjusted life-year (DALY) rates were analyzed according to sex and disorders. Estimated annual percentage changes were derived using log-linear regression models. Joinpoint regression identified significant shifts in the temporal trends. Heatmaps were generated to rank disorders by prevalence and DALYs across global and specific regions. Analyses were conducted using R, version 4.4.3. Between 1990 and 2021, infectious neurological diseases, including malaria, cysticercosis, rabies, meningitis, encephalitis, and tetanus, have declined substantially in both prevalence and DALYs. Conversely, perinatal and developmental disorders showed marked increases, particularly neonatal encephalopathy, preterm birth complications, and neonatal sepsis in both measures. Migraine and tension-type headaches remain the most prevalent conditions in 2021 with minimal temporal changes. Pronounced sex disparities emerged: multiple sclerosis increased more rapidly in females, while alcohol use disorders declined faster in males. Regional inequalities persisted, with infectious burdens concentrated in low- SDI areas, and chronic disorders predominant in high-SDI regions. Neurological disease burden among adolescents and young adults has transitioned from infectious to chronic, neonatal, and behavioral conditions, accompanied by significant sex-specific and regional differences. These results highlight the importance for youth-centered prevention, early diagnosis, and equitable access to care. Integrating pharmacists into multidisciplinary neurological care teams can enhance medication optimization, adherence support, and the long-term management of chronic neurological conditions, ultimately improving health outcomes in this vulnerable population.
