Marchiafava-Bignami disease mimicking acute corpus callosum infarction associated with chronic shochu consumption.
Marchiafava-Bignami disease (MBD) is a rare alcohol-related disorder that predominantly affects the corpus callosum. Because MBD frequently demonstrates diffusion restriction on magnetic resonance imaging (MRI), it can closely mimic acute ischemic stroke (AIS), posing a diagnostic challenge in image-driven acute stroke evaluation. Early recognition is crucial, as timely thiamine therapy may result in complete clinical and radiologic recovery. A 38-year-old man with long-standing daily consumption of shochu, a Japanese distilled spirit, developed vertigo and vomiting on awakening, followed by mild right lower extremity weakness on day 5. MRI performed at a local clinic on day 6 revealed a diffusion-restricted lesion confined to the splenium of the corpus callosum, and corpus callosum infarction was initially suspected. On admission, the patient was fully conscious and exhibited horizontal gaze-evoked nystagmus with very mild right-sided weakness. MRI identified a splenial lesion of the corpus callosum exhibiting high signal intensity on diffusion-weighted imaging with corresponding low apparent diffusion coefficient values, accompanied by mild hyperintensity on T2-weighted and fluid-attenuated inversion recovery images. Given the patient's young age, absence of vascular risk factors, and the splenial distribution of the lesion, AIS was considered unlikely. Following consultation with a neurologist, MBD was suspected based on the patient's history of chronic alcohol consumption, and intravenous vitamin supplementation, including thiamine, was initiated. Neurological symptoms improved rapidly, and follow-up MRI on hospital day 8 demonstrated complete resolution of the splenial lesion. The patient was discharged without neurological sequelae on hospital day 10. A diffusion-restricted splenial lesion in patients with a history of chronic alcohol consumption and without vascular risk factors should prompt consideration of MBD rather than a reflexive diagnosis of AIS. Careful integration of imaging findings with clinical context is essential for identifying reversible stroke mimics and ensuring timely, appropriate treatment.
Marchiafava-Bignami Disease with Hyperperfusion on Arterial Spin Labeling and Complete Recovery.
Marchiafava-Bignami disease (MBD) is a rare demyelinating disease, which mostly diagnosed among alcoholics. Previous studies on cerebral perfusion of patients with MBD mostly found hypoperfusion of corpus callosum and extra-callosal lesions. Nevertheless, hyperperfusion of corpus callosum in MBD has rarely been described.The aim of this study was to report a case of MBD with hyperperfusion on arterial spin labeling and completely recovered. We described a case of MBD with hyperperfusion in the corpus callosum on arterial spin labeling perfusion magnetic resonance imaging, presented with altered mental state. This MBD patient eventually achieved a complete recovery. We suppose that the hyperperfusion of corpus callosum in this patient may be associated with the vasodilation caused by the inflammatory response in the lesions. Future researches are needed to confirm our finding.
Marchiafava-Bignami Disease and Pellagra in a Chronic Alcohol User: A Reversible Case of Cognitive Impairment.
Marchiafava-Bignami disease (MBD) is a rare complication of chronic alcoholism characterized by demyelination of the corpus callosum. Pellagra, resulting from niacin (vitamin B3) deficiency, typically presents with dermatitis, diarrhea, and dementia. Both MBD and pellagra are often associated with severe nutritional deficiencies. We describe the case of a 56-year-old man with alcohol use disorder admitted after a syncopal episode, presenting with disorientation, apathy, and photosensitive desquamative lesions on sun-exposed skin. Brain MRI revealed T2 hyperintensity in the splenium and body of the corpus callosum, compatible with subacute MBS. Laboratory tests confirmed niacin deficiency, establishing concomitant pellagra. The patient received B-complex vitamin supplementation and was maintained in alcohol abstinence. His cognitive and dermatologic condition improved markedly, with near-complete recovery after six months. This case underlines the importance of recognizing nutritional deficiencies in alcohol-related neuropsychiatric presentations.
Acute Marchiafava-Bignami Disease Linked to Tea and Toast Diet: A Case Report.
Marchiafava-Bignami disease (MBD) is a rare demyelinating disorder primarily associated with chronic alcoholism. We present a case of a nonalcoholic patient who developed acute cognitive decline secondary to severe dietary deficiency. Magnetic resonance imaging (MRI) demonstrated an isolated, symmetric lesion in the central corpus callosum, differentiating MBD from acute infarction. Rapid improvement followed Vitamin B supplementation, emphasizing the need for early recognition and treatment. This case expands the clinical spectrum of MBD and underscores the crucial role of MRI in diagnosis and prognosis.

Marchiafava-Bignami disease (MBD) may show normal findings on early MRI. In patients with alcohol use disorder or risk of thiamine deficiency, repeat imaging is important because splenial lesions can develop later. Recognizing this pattern is key to avoiding delayed diagnosis.
