Eosinophilic Fasciitis in Pediatric Patients: A Rare but Distinct Autoimmune Fibrosing Disorder.
Eosinophilic fasciitis (EF), or Shulman syndrome, first described in 1974, is a rare fibrosing disorder characterized by painful, symmetric swelling and progressive woody induration of skin and subcutaneous tissues. Although the pathogenesis remains unclear, EF is considered immune-mediated, often triggered by physical exertion, infections, or medications. Pediatric EF, a particularly uncommon subset, can exhibit distinct clinical features, including pronounced extracutaneous manifestations, unpredictable disease progression, and variable therapeutic responses. Clinical presentations range from rapidly advancing fibrosis leading to joint contractures to fluctuating inflammatory episodes. Diagnosis is challenging because of the absence of universal criteria, although peripheral eosinophilia, elevated inflammatory markers, and imaging findings support clinical suspicion. Definitive diagnosis depends on deep skin and fascial biopsy, revealing eosinophil-rich lymphoplasmacytic infiltrates and fibrosis. Management primarily relies on systemic corticosteroids, supplemented by steroid-sparing immunosuppressive medications in refractory cases. Early diagnosis and treatment are critical because untreated EF can cause irreversible fibrosis and significant functional impairment. The disease's rarity, heterogeneous presentations, and unclear etiology further complicate clinical management. Recent insights suggest EF may involve intricate interactions among environmental triggers, immune dysregulation, and fibrotic remodeling. This review aims to provide an updated overview of pediatric EF, highlighting current knowledge on clinical manifestations, diagnosis, differential diagnosis, therapeutic approaches, and outcomes, supported by an illustrative case, with emphasis on areas needing further research.

Two case reports of eosinophilic fasciitis with onset after immune checkpoint inhibitor cessation.
Immune checkpoint inhibitors (ICIs) are known to cause a wide spectrum of immune-related adverse events (irAEs). Among these, eosinophilic fasciitis (EF) is a rare, fibrosing disorder causing inflammatory infiltration of subcutaneous fat and fascia. It is characterized clinically by edema and subsequent induration and tightening of the skin and subcutaneous tissues. Several case reports have documented EF secondary to ICIs in patients on active treatment. Herein, we present two cases of delayed EF following treatment cessation with avelumab for metastatic urothelial carcinoma (Case 1) and following adjuvant nivolumab completion for stage IIIC melanoma (Case 2). Both patients had typical exam findings including erythema/edema of the extremities and trunk and diffuse thickening of subcutaneous fat and fascia, leading to severe respiratory restriction in Case 1. Both patients were diagnosed with EF by full-thickness skin biopsy showing sclerosis and lymphocytic infiltration of the subcutaneous fat and/or fascia. Only one of the two patients presented with definite eosinophilia. Both cases were treated with glucocorticoids and had early recurrence of symptoms post steroid taper, necessitating subsequent protracted steroid and steroid sparing agent use. Overall, we demonstrate the importance of considering delayed irAEs, specifically autoimmune fibrotic skin diseases even when ICI therapy has been discontinued. We underscore the need for collaboration between oncology and rheumatology as the scope of ICI treatments for cancer continues to expand.
[Shulman syndrome: An atypical presentation of a rare disease].
Eosinophilic fasciitis is a rare scleroderma-like disorder. Most of the available information comes from case reports or small case series. It is characterized by fascial thickening, acute pain, skin induration, joint contractures, and, anecdotally, visceral involvement. Diagnosis is confirmed through deep biopsy, although magnetic resonance imaging may be useful in inconclusive cases. We describe the case of a previously healthy 32-year-old man male with a history of recurrent minor trauma to the lumbar and gluteal regions. He developed acute-onset pain and progressive deep induration in the affected regions, which later extended symmetrically to all four extremities. Initial magnetic resonance imaging suggested lymphedema; however, given the high clinical suspicion, images were re-evaluated, revealing findings consistent with fasciitis. Treatment with corticosteroids and methotrexate was initiated, resulting in rapid improvement of residual pain and gradual reduction of deep induration. This case highlights the heterogeneous presentation of eosinophilic fasciitis and underscores the importance of including it in the differential diagnosis of scleroderma-like disorders, as well as the role of magnetic resonance imaging in ambiguous cases. la fascitis eosinofílica es un trastorno esclerodermiforme poco frecuente.
Rituximab and Intravenous Immunoglobulin (IVIG) for Refractory Eosinophilic Fasciitis: A Case Report.
Eosinophilic fasciitis (EF) is a rare condition with an unknown cause. This case study showed that a 40-year-old man with EF did not respond to standard treatments but improved after receiving rituximab and intravenous immunoglobulin (IVIG). Further studies are needed to confirm rituximab's effectiveness and long-term safety for EF.
