An autosomal recessive form of ectodermal dysplasia, a heterogeneous group of disorders due to abnormal development of two or more ectodermal structures. It is a syndrome characterized by the association of cleft lip/palate, ectodermal dysplasia (sparse short and dry scalp hair, sparse eyebrows and eyelashes), and partial syndactyly of the fingers and/or toes. Two thirds of the patients do not manifest oral cleft but present with abnormal teeth and nails.

A form of ectodermal dysplasia. Ectodermal dysplasia defines a heterogeneous group of disorders due to abnormal development of two or more ectodermal structures. ADULT syndrome involves ectrodactyly, syndactyly, finger- and toenail dysplasia, hypoplastic breasts and nipples, intensive freckling, lacrimal duct atresia, frontal alopecia, primary hypodontia and loss of permanent teeth. ADULT syndrome differs significantly from EEC3 syndrome by the absence of facial clefting. Inheritance is autosomal dominant.

Comparative Analysis of Airway Volume and Velum Morphology in Primary Furlow Palatoplasty: Conventional vs. Small-Z Design.
Patients with cleft lip and/or palate have a significantly higher prevalence of sleep-disordered breathing (SDB) due to congenital anatomic differences and the sequelae of surgical management. Of note, the velopharyngeal and oropharyngeal airway size plays a significant role. This single-center retrospective study compares the effect of conventional Furlow palatoplasty and small-Z Furlow palatoplasty on airway morphology. Consecutive patients with cleft lip and alveolus (CL/A) and cleft lip and palate (CL/P) presenting for secondary alveolar bone grafting (ABG) between 2017-2019 were enrolled for this study. Exclusion criteria included syndromic patients, previous secondary velopharyngeal surgery, and previous sleep surgery. Patients were divided into the control group (CL/A only), those who received primary conventional Furlow palatoplasty, and those who received primary small-Z palatoplasty. Speech was assessed by 2 experienced speech pathologists and SDB via the OSA-18 questionnaire. Airway and velum morphology was assessed using cone beam CT. Amongst the 95 included patients, 23 were in the control group, 32 had conventional Furlow palatoplasty, and 40 patients had small-Z Furlow palatoplasty. There was no statistically significant difference in speech outcomes or OSA-18 scores between the 3 groups. However, compared to the conventional group, the small-Z group demonstrated increased velopharyngeal airway volume, decreased velar length and increased minimum cross-section area of the velopharyngeal airway. Conventional and small-Z Furlow palatoplasty provided patients with a similar speech and sleep quality outcome. The small-Z palatoplasty, however, demonstrated an increased velopharyngeal airway volume and minimum cross-section area, as well as decreased velar length.
Immediate Orthodontic Repositioning of Traumatically Extruded Incisors in a Patient With Cleft Lip and Alveolus: A Case Report.
This case report describes a 15-year-old boy born with a cleft lip and alveolus who sustained severe extrusive and lateral luxation trauma to multiple incisors during the final phase of orthodontic treatment. The displacement caused incisal interference and prevented normal jaw closure. Deformed beta titanium wires were removed, loosened brackets were rebonded, and immediate orthodontic repositioning was initiated using light forces with 0.014-inch CuNiTi round wires combined with manual repositioning. This approach resolved the incisal interference and restored functional occlusion within 1 week, with complete incisal alignment achieved after 3 months. During follow-up, several affected teeth required endodontic treatment due to pulp necrosis. The patient's cleft-related dental anomalies prolonged the overall treatment process. This case highlights the potential benefit of immediate orthodontic repositioning after extrusive dental trauma, a treatment option currently underrepresented in existing dental trauma guidelines.
Permanent maxillary canine eruption in children with clefts - a retrospective study.
The purpose of this study was to investigate the effect of alveolar bone grafting (ABG) on the eruption timing and the need for deliberation of the permanent maxillary canine in cleft lip and/or palate patients. The study group included 45 cleft sites from a total of 39 unilateral cleft lip and alveolus (CLA, n = 5) and unilateral (n = 22) cleft lip and palate (CLP) and bilateral (n = 12) CLP patients who had undergone treatment in the Oulu Cleft Center (OCC). A split-mouth approach and linear mixed-effects regression analysis was used to compare findings between single individual grafted and non-grafted alveolar sites. The maxillary canine angulation had a significant role in the timing of the canine eruption (estimate 0.06, 95% CI 0.04 to 0.08). While ABG seemed to delay the canine eruption, this result was not statistically significant. The eruption of the maxillary canine tooth on the bone grafted side was delayed compared to the eruption of the maxillary canine on the non-grafted side (estimate -0.55, 95% CI -0.99 to -0.11). Another factor affecting the canine eruption was surgical exposure of the canine, and it typically emerged about a year after it had been surgically exposed and deliberated (estimate 1.05, 95% CI 0.40 to 1.70). The permanent maxillary canine eruption was slightly delayed after the ABG. Also, the maxillary canine was found to typically emerge about a year after it has been deliberated. The early bone graft had no benefit in terms of the timing of canine eruption. The angulation of the maxillary canine should be taken into consideration when planning ABG to facilitate its eruption. Careful planning and timing of the ABG and multidisciplinary approach have an important role in cleft patients' treatment.
How Much Pain Medication Do Patients Require After Posterior Iliac Crest Bone Graft to the Alveolar Cleft?
ObjectiveThe purpose of this study was to quantify analgesic use following alveolar cleft bone grafting (ABG) utilizing a posterior iliac crest (PIC) donor site.DesignThis is a prospective cohort study of consecutive patients that underwent ABG with PIC in a 10 month period from November 2022 to September 2023.SettingTertiary care free-standing pediatric hospital.ParticipantsPatients with cleft lip and alveolus with or without cleft of the secondary palate undergoing alveolar bone graft with cancellous marrow from the PIC.InterventionsAll patients underwent ABG and recorded analgesic use afterwards.Main Outcome MeasureThe main outcome variable was total pain medication utilization during the first 10 postoperative days (PODs).Results22 subjects (72.7% male, 54.5% Caucasian 72.7% unilateral and 27.3% bilateral clefts) recorded pain medication use. Total duration of analgesic use (any type) was 3.86 ± 1.7 postoperative days. 10 subjects (45.5%) did not use opioid medication postoperatively. The remaining 12 subjects (54.5%) used a median of 12 (0, 19) morphine milli-equivalents (MME), equal to 1.6 (0, 2.53) doses of 5 mg oxycodone. An average of 9.9 ± 5.6 doses of acetaminophen and 9.1 ± 4.7 doses of a non-steroidal anti-inflammatory drug (NSAID) were administered. No subject used opioids after POD3. Patients with unilateral clefts used more acetaminophen (p = 0.02); otherwise, there were no significant differences in outcomes based on any predictor variable (p > 0.05).ConclusionsAnalgesic requirements after ABG with PIC were minimal, with nearly half of patients taking no postoperative opioids. As such, judicious opioid prescribing is prudent. .
Unveiling Oral Health Differences: Gingival Health and Hygiene in Children With and Without Cleft Lip and Palate.
ObjectiveTo compare the oral hygiene and gingival health of children with and without cleft lip and palate.DesignCross-sectional comparative study.SettingInstitutional tertiary referral hospital.Patients/ParticipantsThis study was conducted on 108 children aged 6 to 12 years, divided into 3 groups: Group 1 = children with cleft lip and alveolus with or without cleft palate, unilateral or bilateral (n = 36, 22 boys and 14 girls, aged 8, 39 ± 1.61 years), Group 2 = children with isolated cleft palate (n = 36, 8 boys and 28 girls, 8.11 ± 1.88 years); and Group 3 = children without clefts (n = 36, 18 boys and 18 girls, 8.53 ± 1.33 years).Main Outcome MeasuresThe clinical parameters evaluated were Gingival Index (GI) and Patient Hygiene Performance (PHP) Index. analysis of variance and Kruskal-Wallis tests were used to evaluate intergroup differences. Correlations between the variables age, PHP Index and GI were assessed by the Spearman correlation (P < .05).ResultsThe results demonstrated no statistically significant difference between the three groups for the PHP index (P = .126). There was also no intergroup difference for GI (P = .418).ConclusionsNo difference for the gingival status was observed between children with cleft lip and palate and with isolated cleft palate. Children with oral clefts present adequate gingival status compared to noncleft children.
