An autosomal recessive lysosomal storage disease due to deficient activity of lysosomal beta-glucocerebrosidase, and characterized by accumulation of glucosylceramide in the reticulo-endothelial system. GD is a multisystem disease historically divided into three main subtypes on the basis of the presence of neurologic involvement, age at onset and progression rate: type 1 is the non-neuropathic form, type 2 is the acute neuropathic form with early onset and rapid neurologic deterioration, type 3 is the chronic neuropathic form with slow progression of neurologic features. GD shows a marked phenotypic diversity ranging from adult asymptomatic forms, at the mild end, to perinatal lethal forms at the severe end of the disease spectrum. Familial Cases of Legg-Calvé-Perthes Disease-Hemostatic and Molecular Markers.
Legg-Calvé-Perthes disease (LCPD) is a rare disease caused by avascular necrosis of the femoral head. Although its etiology is still not fully understood, evidence suggests that heritable prothrombotic and inflammatory factors, as well as environmental factors, may be implicated in its onset and progress. The objective of this study is to describe the genetic, biochemical, and environmental factors that may be associated with the etiology of LCPD. This study was conducted in three families and included seven related patients with an LCPD diagnosis. We evaluated the following gene alterations using real-time PCR: MTHFR, CBS, COL1A1, COL2A1, PT, FVL, FVIII, FIX, PAI-1, eNOS, IL-23R, TNF-α, RANNK, RANNK-L, OPG and IL-6. Additionally, we assessed fourteen thrombophilia-associated biochemical markers, as well as environmental factors that may be associated with the etiology of LCPD in family cases. The results show different hemostatic alterations in every individual analyzed, presenting out-of-range values in one or more parameters. Concentrations of hemoglobin and fibrinogen and the FIX activity percentage showed statistically significant differences (p < 0.001) when compared with healthy controls. All patients presented at least one mutated allele for the MTFHR (rs1801133), IL-23R (rs1569922) and OPG (rs2073618) polymorphisms, as well as isolated cases with other genetic variants. Our results show environmental elements from every family, and hemostatic and inflammatory disorders, may be involved in the development of LCPD. Furthermore, genetic variants could contribute to the onset of the disease. This study highlights the multifactorial nature of this pathology, involving various environmental, genetic, inflammatory, and prothrombotic factors in three families that included seven patients diagnosed with LCPD.
Sclerostin silencing in human umbilical cord mesenchymal stem cells enhances bone regeneration via Wnt pathway activation.
Steroid-induced avascular necrosis of the femoral head is a debilitating condition caused by prolonged glucocorticoid exposure, leading to bone death and disrupted metabolism. Current treatment options are limited, especially in late-stage disease, emphasizing the urgent need for novel regenerative therapies. This study by Lv et al explores the therapeutic potential of human umbilical cord mesenchymal stem cells (MSCs) genetically modified to silence sclerostin, a known inhibitor of bone formation via the Wnt/β-catenin pathway. The authors engineered sh-human umbilical cord MSCs and demonstrated their enhanced osteogenic differentiation and suppression of adipogenesis compared to unmodified MSCs in a steroid-induced rat model of femoral head necrosis. Significant improvements were observed in bone microarchitecture, biochemical markers of bone metabolism, and histological parameters. This investigation suggests that sclerostin silencing enhances MSC efficacy by activating Wnt signaling, thereby offering a promising regenerative strategy for steroid-induced avascular necrosis of the femoral head that targets the core imbalance in bone remodeling.
Prioritising conditions resulting in chronic lower limb musculoskeletal pain in children and adolescents for development of clinical practice guidelines: an Australian Delphi study.
Children and adolescents with chronic lower limb pain may experience delayed diagnosis, misdiagnosis, insufficient or inappropriate treatment initiation or type. Developing condition-specific clinical practice guidelines is one approach to address these healthcare challenges. This research aimed to prioritise the top 10 conditions resulting in chronic lower limb musculoskeletal pain in children that would benefit from the development of Australian clinical practice guidelines. We conducted a modified three round eDelphi. Three multidisciplinary panels were provided a summary of current available evidence and any clinical practice guidelines currently available for 124 conditions identified as resulting in chronic lower limb pain. Panels rated their agreement on the need for the development of clinical practice guidelines and ranked the top 10 conditions. In the final round, parents of children with chronic lower limb musculoskeletal pain rated the appropriateness of the top 10 conditions for guideline development. Forty-four Australian health care professionals (22 allied health, 7 paediatric medical specialists, and 13 general practitioners) participated in Rounds 1 and 2 with a retention rate of 100%. Sixteen parents participated in the confirmatory round. The top 10 conditions prioritised for clinical practice guideline development were generalised joint hypermobility, growing pains, musculoskeletal pain, Juvenile Idiopathic Arthritis, joint instability, avascular necrosis, Legg-Calve-Perthes, flat foot, slipped capital femoral epiphysis and patellofemoral pain syndrome. Healthcare professionals prioritised 10 conditions that require development of clinical practice guidelines. Parents of children and adolescents with chronic lower limb pain provided feedback on the appropriateness of the 10 prioritised conditions for guideline development. These guidelines should be multidisciplinary, co-designed with families and relevant to all health settings. Sickle Cell Disease in Central India: Haematological and Clinical Insights from a Large Cohort of Patients.
Sickle cell disease (SCD) is an inherited blood disorder that affects haemoglobin. Understanding the variability in the presentation of SCD is important for effective disease management. The present study is a cross-sectional analysis of a cohort of 752 SCD patients during 2016-2020, collected as routine institutional data from the Vidarbha region of central India, undertaken to explore the haematological, molecular and clinical features. Among the 752 SCD patients, 85.8% were SS, and 13.8% were S-β thal while the remaining 0.7% were the HbS-D Punjab variant. The median age of the patients was 17 (IQR: 9-27) years. Among them, 41.4% belonged to the scheduled caste and 17.8% belonged to the scheduled tribe. Common clinical presentations showed hospitalizations (51.3%), blood transfusions (32.4%), febrile episodes (50.7%), acute chest syndrome (29.5%), painful severe crisis (7.5%), and avascular necrosis (AVN) (4.2%). Haematological findings revealed that S-β thal has lower MCV, MCH and MCHC and higher RBC counts compared to SS. Only 24.5% of patients reported consumption of hydroxyurea (HU) regularly. Haemoglobin levels were found to be significantly (P < 0.05) higher in patients receiving HU. Additionally, higher HbF (%) and lower HbS (%) were observed in patients consuming HU. The Xmn1 polymorphism was determined in 342 SCD patients. Out of that 298 (86.6%) patients were homozygous (+/+) and 44 (13.4%) were heterozygous (+/-). The HbF% levels were significantly higher (P = 0.04) in those patients with Xmn1 homozygous (+/+) as compared to the patients with heterozygous (+/-) in S-β thal. During this study course, 21 SS disease patients lost their lives. Our study offers realistic insights into the haematological, clinical and molecular spectrum of a cohort of SCD from the Vidarbha region. The clinical course of the disease is influenced by many factors, such as HbF levels, Xmn polymorphism and HU consumption etc., The study highlights the suboptimal use of HU among the patients. HU has shown an improved clinical course, and its coverage needs to be increased. The comparison of age group-wise distribution of SCD patients and the general population in younger and older age groups reflects early mortality in SCD patients. Induction treatment of lupus nephritis: to pulse or not to pulse? Whether 'tis nobler to universally or selectively prescribe.
Although treatment guidelines recommend pulse steroids, induction treatment of lupus nephritis (LN) varies significantly among providers. This paper aims to explore evidence that intravenous pulse provides pharmacological benefits along with improved clinical efficacy without greater toxicity compared with high-dose oral glucocorticoids justifying inclusion for all active LN. We conducted a systematic literature review (SLR) using the term 'pulse glucocorticoids in LN' in order to identify studies that summarise the pharmacologic mechanisms of glucocorticoids, reviewed the historical use of glucocorticoids in SLE, and compared pulse therapy with high-dose oral treatment related to their efficacy and toxicities. SLR demonstrated that non-genomic mechanisms of action are more associated with pulse than oral steroids. Some observational studies reported improved renal responses with pulse steroids but in exchange for more adverse metabolic bone disease effects (eg, osteoporosis and avascular necrosis) as well as infections and, importantly, mortality. Current guidelines promoting pulse therapy for all forms of proliferative LN (and in the case of American College of Rheumatology 2024 treatment guidelines, even isolated class V membranous LN) rely on structured evidence grading processes, including expert consensus and observational data, but are not based on head-to-head randomised controlled trial comparisons. Therefore, there can be an ongoing debate regarding the best approach. The SLR identifies a distinct pharmacological benefit unique to pulse treatment, though without direct evidence necessary for the treatment of LN; includes observational studies with evidence of superior efficacy, but not consistently; and identifies a higher risk of adverse effects. In our opinion, the data advocates for a more selective approach to treatment, foregoing universal treatment with pulse steroids, based on a toxicity versus benefit assessment that patients with mild disease do not require the additional risks.
