Pellagra in Contemporary Clinical Practice (2000-2023): A Systematic Review.
Pellagra's clinical presentation, etiological factors, and treatment in the early 21st century remain underexplored. This study aimed to update the understanding of pellagra's clinical and etiological aspects with a reappraisal of its treatment and prevention. We searched PubMed and Scopus up to January 9, 2023, to identify relevant studies. The reference lists of all included articles were also carefully screened to identify any additional studies. We included case reports, case series, and case-control studies documenting confirmed cases of pellagra between 2000 and 2023. The review protocol was registered with PROSPERO (CRD42022381416). The search identified 918 studies, of which 133 met the inclusion criteria, comprising 1212 pellagra cases. Cutaneous manifestations were the most common (98.63%), followed by gastrointestinal (59.4%) and neurological (54.2%) signs. Complete symptom resolution occurred on average within 27.42 days of vitamin B3 supplementation. Dietary deficiency remained the predominant cause of pellagra (85%), and it was particularly common in populations affected by food insecurity or humanitarian crises, accounting for 93% of cases. Alcohol misuse was reported in 9.6% of cases. Isoniazid was the leading drug-related cause (14.3%). Other contributing factors included gastrointestinal disorders and prior surgeries associated with malabsorption. These findings indicate an evolving etiological pattern in pellagra: dietary deficiency remains the primary driver, yet medication-induced and malabsorption-related cases are increasingly reported. Targeted niacin supplementation strategies for high-risk populations may improve outcomes and enable more efficient, lower-dose therapeutic approaches once underlying causes are addressed.
Marchiafava-Bignami Disease and Pellagra in a Chronic Alcohol User: A Reversible Case of Cognitive Impairment.
Marchiafava-Bignami disease (MBD) is a rare complication of chronic alcoholism characterized by demyelination of the corpus callosum. Pellagra, resulting from niacin (vitamin B3) deficiency, typically presents with dermatitis, diarrhea, and dementia. Both MBD and pellagra are often associated with severe nutritional deficiencies. We describe the case of a 56-year-old man with alcohol use disorder admitted after a syncopal episode, presenting with disorientation, apathy, and photosensitive desquamative lesions on sun-exposed skin. Brain MRI revealed T2 hyperintensity in the splenium and body of the corpus callosum, compatible with subacute MBS. Laboratory tests confirmed niacin deficiency, establishing concomitant pellagra. The patient received B-complex vitamin supplementation and was maintained in alcohol abstinence. His cognitive and dermatologic condition improved markedly, with near-complete recovery after six months. This case underlines the importance of recognizing nutritional deficiencies in alcohol-related neuropsychiatric presentations.
Epidermal NAD+ deficiency induces IL-36-mediated skin inflammation and acanthosis.
Nicotinamide adenine dinucleotide (NAD+) is essential for cellular metabolism, DNA repair, and stress responses. NAD+ is synthesized from nicotinamide, nicotinic acid (collectively termed niacin), and tryptophan. In humans, deficiencies in these nutrients result in pellagra, marked by dermatitis, diarrhea, and dementia. The dermatitis associated with pellagra typically manifests as photodermatosis in sun-exposed areas. This study examined the effects of NAD+ deficiency on skin homeostasis using epidermis-specific Nampt-conditional KO (Nampt-cKO) mice. These mice displayed substantial NAD+ depletion, reduced poly(ADP-ribose) polymerase (PARP) activity, and increased DNA damage. Consequently, Nampt-cKO mice developed spontaneous skin inflammation and epidermal hyperplasia. RNA-seq and IHC analyses demonstrated increased IL-36 cytokine expression, suggesting that DNA repair-related genomic stress triggers keratinocyte-driven IL-36 production, which promotes inflammation. Furthermore, reduced COL17A1 expression and elevated thymic stromal lymphopoietin (TSLP) levels were observed. NAD+ repletion by transdermal supplementation of nicotinamide mononucleotide (NMN) suppressed the rise of IL-36 levels and skin inflammation. These findings underscore the importance of Nampt-mediated NAD+ metabolism for epidermal stability and indicate that NAD+ depletion may contribute to IL-36-mediated skin inflammation, offering insights for therapeutic strategies in inflammatory skin disorders.
Pellagra-Fine arts and literature: A short journey from Pietro Belloti and Johann Wolfgang Goethe to John Wesley Caroll and Amedeo John Engel Terzi.
Pellagra is a non-hereditary disorder of the niacin and tryptophan metabolism. Cutaneous and gastrointestinal signs and symptoms are early signs of the disease, but neuropsychiatric findings follow, if the disease is not recognized and treated soon enough. Although pellagra had not been identified as a separate entity before 1735, artists noticed the characteristic cutaneous manifestations. An example is Pietro Bellotti's masterpiece Vecchia popolana con un ragazzo (Old Peasant Woman with a Boy; ca 1680-1690) with a most realistic style. In 1786, the German writer and scientist Goethe traveled to Northern Italy. In his book Italienische Reise (Italian Journey), he made remarks about cutaneous findings in the rural population suggesting pellagra. He missed the scaling and fissuring of skin, as he traveled in the autumn and not spring, when the disease has its seasonal peak. The association of pellagra with art is further illustrated by two examples of the early 20th century-painter John Wesley Carroll from Kansas and medical illustrator Amedeo John Engel Terzi from Sicily. Their multicolored illustrations supported education about the disease before high-quality color photography became available.
[Pellagra secondary to chronic alcoholism in Costa Rica].
Introduction: pellagra is a disease caused by a deficiency of niacin (vitamin B3) or tryptophan, its precursor. Case report: we present the case of a 67-year-old Costa Rican male patient, alcoholic and homeless, with chronic ulcers on the lower limbs, photosensitive dermatitis, gait disturbance, mild cognitive impairment, and dysthymia. A diagnosis of pellagra was made based on clinical findings, which was later confirmed by a positive response to vitamin supplementation. Discussion: a review of pellagra, a condition that is increasingly underdiagnosed, was conducted; in this case, it presented with 2 of the 3 classic manifestations described in the literature. Introducción: la pelagra es una enfermedad carencial causada por déficit de niacina (vitamina B3) o de su precursor, el triptófano. Caso clínico: se presenta el caso de un paciente masculino costarricense de 67 años, alcohólico y habitante de la calle, con úlceras crónicas en los miembros inferiores, dermatitis fotosensible, trastorno de la marcha, deterioro cognitivo leve y distimia, en quien se diagnosticó la pelagra con base en los hallazgos clínicos, que posteriormente se confirmó ante la respuesta positiva a la suplementación vitamínica. Discusión: se realizó una revisión sobre la pelagra, una patología cada vez menos diagnosticada, que en esta ocasión se presentó con 2 de las 3 manifestaciones clásicas descritas en la literatura.
