The Metacarpophalangeal Pattern Profile: An Old Method With New Insights Into the Evaluation of Short Stature.
To characterize the metacarpophalangeal pattern profile (MCPP) of healthy children and adolescents from São Paulo, Brazil, and to establish percentile curves by chronological age (CA), bone age (BA), and sex using the LMS method. Additionally, to compare these findings with previous population-based data and to apply the derived standards to patients with skeletal dysplasias. Left hand and wrist radiographs were obtained from healthy individuals and age-matched patients with confirmed skeletal dysplasias. Tubular bone lengths were compared across CA and BA, against prior normative studies, and with dysplastic cohorts using Student's t-test. Patient Z-scores were calculated from LMS parameters generated from the healthy population. We analyzed 974 radiographs from healthy subjects and 83 from patients (18 hypochondroplasia, 27 achondroplasia, 14 osteogenesis imperfecta, 24 Turner syndrome). In healthy participants, hand bone lengths correlated significantly with CA and BA. Compared with international reference data, differences in mean metacarpal and phalangeal lengths were noted. Patients with achondroplasia and hypochondroplasia exhibited markedly reduced Z-scores relative to controls, whereas those with Turner syndrome showed reductions of up to 1.8 SD in the fourth metacarpal. Patients with osteogenesis imperfecta demonstrated no significant deviations. This study established MCPP reference percentiles for Brazilian children and adolescents using the LMS method. Bone measurements showed consistent associations with CA and BA. Although differences were observed relative to international cohorts, the generated standards effectively discriminated dysplastic phenotypes, particularly achondroplasia and hypochondroplasia, supporting the use of MCPP analysis as an adjunct tool for evaluating short stature and suspected skeletal dysplasias.
Quantifying the risks: a systematic review and proportional meta-analysis of the perioperative complications of posterior cranial vault distraction osteogenesis in patients with craniosynostosis.
Posterior cranial vault distraction osteogenesis (PCVDO) is a well-established surgical technique for increasing intracranial volume and correcting cranial deformities in children with craniosynostosis. Despite its efficacy, PCVDO is associated with the risk for several perioperative complications. In this systematic review and proportional meta-analysis, the authors aimed to estimate the pooled prevalence of complications associated with PCVDO, including overall complications, infection, CSF leakage, device-related complications, premature device removal, and unplanned returns to the operating room. Additionally, they sought to identify sources of heterogeneity among the studies and evaluate the influence of key clinical and surgical moderators-such as patient age, syndromic status, distraction parameters, elevated intracranial pressure, hydrocephalus, Chiari malformation, syrinx, and prior surgical interventions-on complication rates. A systematic review and proportional meta-analysis were conducted, following the PRISMA guidelines. The MEDLINE/PubMed database was reviewed for English-language, peer-reviewed studies published between 2009 and 2025 that reported perioperative complications in pediatric patients (age < 18 years) with craniosynostosis who had undergone PCVDO and included at least 2 patients. Excluded publications were single case reports, editorials, technical notes, reviews, preclinical or animal studies, and studies not meeting these criteria. Data from the included studies were compiled into a comprehensive spreadsheet to facilitate qualitative and quantitative analyses. A quality assessment of all studies was conducted utilizing the Newcastle-Ottawa Scale. The primary outcome was the proportion of patients experiencing any complication. Secondary outcomes comprised rates of infection and wound-related events, CSF leakage, device-related complications, premature device removal, and unplanned returns to the operating room. Thirty-three studies were included in this analysis. The pooled overall complication rate was 26.3%. Infection and wound-related complications were most common (10.4%), followed by CSF leakage (6.7%) and device-related complications (6.0%). Premature device removal and return to the operating room occurred in 4.0% and 9.9% of patients, respectively. Meta-regression analysis revealed that younger age was significantly associated with an increased risk of CSF leakage and premature device removal. Shorter latency periods were associated with a higher risk of CSF leakage and return to the operating room. While PCVDO is effective in managing craniosynostosis, it is associated with a considerable risk of complications. Age and latency period are significant predictors of adverse outcomes. Innovations in device design and surgical protocols are warranted to optimize the safety and efficacy of the procedure.
Application of the Gross Motor Function Measure in children with conditions other than cerebral palsy: A systematic review.
To investigate the application and evaluate the measurement properties of the Gross Motor Function Measure (GMFM) in children with conditions other than cerebral palsy (CP). A systematic review was conducted using five electronic databases to identify studies that used the GMFM in children with conditions other than CP. Methodological quality and measurement properties were evaluated using established standards for assessing outcome measures. We identified 210 studies across various paediatric conditions. Measurement property studies examined eight conditions: acquired brain injury, spinal muscular atrophy, Fukuyama congenital muscular dystrophy, Down syndrome, osteogenesis imperfecta, acute lymphoblastic leukaemia (ALL), leukodystrophy, and Pompe disease. Evidence quality was generally low to very low owing to small sample sizes and methodological limitations. Reliability showed sufficient ratings across most conditions. Content validity was examined only for ALL and demonstrated sufficient ratings. Responsiveness and construct validity showed variable results across conditions. Clinical application analysis revealed inadequate methodological reporting and widespread use without appropriate validation. GMFM validation for conditions other than CP remains insufficient despite widespread use. Content validity verification and enhanced methodological rigor are critically needed. Clinicians should interpret results cautiously until robust validation is established.
Humeroradial Synostosis: An Updated Classification and Differential Diagnosis Based on Genetic Aetiology.
Humeroradial synostosis (HRS) is a rare congenital limb malformation, characterised by fusion of the humeral and radial bones, leading to functional disability of the elbow joint. HRS may be reported in familial or sporadic cases and observed either isolated or as part of a syndromic condition. According to an extensive review of the literature, a dozen known conditions may comprise an HRS. The present review aims to propose an updated classification based on molecular pathways (chondrogenesis and osteogenesis; limb development and patterning; genome regulation), combined with a concise overview of the conditions associated with HRS. This knowledge could guide molecular analyses, patient management and genetic counselling. As some cases remain unexplained, further genetic and epidemiological studies are required to evaluate the contribution of genetic and environmental factors in HRS physiopathology.
P38α MAPK-induced senescence in cranial suture progenitor cells promotes craniosynostosis.
Craniosynostosis is a congenital cranial developmental disorder that frequently leads to craniofacial deformities and even neurological dysfunction. The abnormalities in cranial suture progenitor cells (SPC) are considered a key event in craniosynostosis; however, the specific mechanism remains unclear. Using a syndromic craniosynostosis mouse model, we found that hyperactivation of p38α mitogen-activated protein kinase (MAPK) induced senescence in SPC of craniosynostosis mice. Integrated analysis of datasets from human patients and murine models, combined with cellular validation, revealed that p38/p53 activation and cellular senescence were prevalent across multiple forms of craniosynostosis and corresponding experimental models. Additionally, senescent cells significantly promoted osteogenic differentiation of SPC by paracrine Tgf-β1. Through in vivo and in vitro experiments, our evidence demonstrates that pharmacological inhibition of p38 MAPK, conditional knockout of Mapk14, and scAAV-mediated shRNA knockdown differentially attenuate SPC senescence, suture fusion, and elevated intracranial pressure, while ameliorating behavioral abnormalities in craniosynostosis mouse model. The present study supports p38α MAPK as potential therapeutic target for craniosynostosis.
