Optimising DTwP-containing vaccine infant immunisation schedules in Uganda and Nepal (OptImms): two open-label, non-inferiority, randomised controlled trials.
Few data are available comparing the WHO diphtheria-tetanus-pertussis (DTP)-containing vaccine schedule with reduced-dose or delayed three-dose schedules in infants. We aimed to identify an alternative schedule that is non-inferior or superior to the WHO schedule in maintaining early pertussis antibody levels before booster, while creating space for integration of new vaccines. In two parallel, open-label, randomised, non-inferiority trials in Uganda and Nepal, healthy infants aged 42-50 days were randomised in a 4:4:4:3:3 ratio via an online system to five diphtheria-tetanus-whole-cell pertussis-Haemophilus influenzae type b-hepatitis B (DTwP-Hib-HepB) vaccination schedules (WHO schedule [ages 6, 10, and 14 weeks]; reduced two-dose schedules at ages 6 and 14 weeks or ages 2 and 4 months; or delayed three-dose schedules at ages 2, 3, and 4 months or ages 2, 4, and 6 months), using site-stratified block randomisation with a block size of 18. The primary outcome was the pre-booster IgG antibody response against pertussis antigens (pertussis toxin; filamentous haemagglutinin [FHA]; pertactin; fimbriae 2 and 3 [Fim 2 and 3]). Antibody responses during and 1 month after the primary series were assessed as secondary endpoints. The two-dose schedules were compared with the WHO schedule as primary analyses, and the exploratory analyses were to compare delayed three-dose schedules with the WHO schedule. All these analyses were conducted in the per-protocol population. For the primary endpoint, non-inferiority margins for geometric mean ratios (GMRs) between two-dose and WHO schedules were calculated for each pertussis antigen separately as 35% of the standard deviation of the geometric mean concentrations of the WHO group, and non-inferiority was concluded if the lower bound of the 95% CI of the GMR exceeded the non-inferiority margin. A non-inferiority margin of 0·67 was used for all other non-inferiority comparisons. The trials were registered with ISRCTN (Uganda [ISRCTN60356654] and Nepal [ISRCTN12240140]) and are complete. Between Oct 1, 2021, and July 12, 2022 (Uganda) and Dec 5, 2021, and Feb 26, 2023 (Nepal), 956 infants were recruited per country and randomly allocated across the five study groups; pre-booster analyses were conducted in 876 participants in Uganda and 851 participants in Nepal. Two-dose schedules at ages 6 and 14 weeks and ages 2 and 4 months did not meet non-inferiority criteria for pertussis antibodies pre-booster, except FHA (non-inferiority margin 0·59) in Nepal (6 and 14 weeks GMR 0·83 [95% CI 0·61-1·13]; 2 and 4 months 1·20 [0·88-1·64]). Delayed three-dose schedules at ages 2, 3, and 4 months and ages 2, 4, and 6 months produced similar or higher post-primary series and pre-booster responses compared with the WHO schedule, but the WHO schedule achieved higher antibody responses against all pertussis antigens, except FHA in Nepal, at age 3 months. The WHO DTP schedule is the preferred schedule in high-pertussis-burden settings, as it elicits the greatest antibody responses in the first 3 months of life when infants are particularly vulnerable. Although delayed three-dose schedules may be considered in low-risk contexts and in settings with maternal immunisation programmes, early infant protection should guide policy decisions. The diagnostic void: how typhoid-by-default drives antimicrobial resistance in Ethiopia.
The empirical diagnosis of typhoid-by-default represents a severe failure in Ethiopia's management of febrile illnesses. This practice of diagnosing undifferentiated fever as typhoid without confirmation drives antimicrobial resistance (AMR) through a unique diagnostic ecosystem shaped by historical precedent, systemic constraints, community demand, and reliance on the outdated Widal test. This diagnostic void promotes empirical use of broad-spectrum antibiotics, selecting for resistant Salmonella enterica serotype Typhi, while contributing to the spread of AMR in commensal organisms and other pathogens. This practice contributes to a broader AMR crisis that extends beyond enteric fever, compromising the treatment of urinary, respiratory, and other common infections. The result is a self-perpetuating cycle of multisystem treatment failures, excess mortality, and economic loss. In this Personal View, we discuss the steps required to break this cycle: eliminating the Widal test, implementing validated diagnostic algorithms for febrile presentation, expanding diagnostic access, introducing typhoid vaccines, and establishing antimicrobial stewardship programmes. Addressing this issue is an urgent public health priority for both national and global AMR containment.
Ceftriaxone-resistant multidrug-resistant Salmonella Typhi sepsis in pregnancy.
A primigravida at 26 weeks' gestation visiting from Pakistan presented with fever, abdominal pain and hypotension suggestive of sepsis and required admission to the intensive care unit. Blood culture grew a multidrug-resistant (MDR) Salmonella enterica serovar Typhi isolate that was resistant to cephalosporins but susceptible to carbapenems. Intravenous meropenem was commenced, resulting in a rapid clinical improvement within 48 hours. She completed a 14-day course of meropenem without maternal or fetal adverse effects, and repeat blood cultures were sterile. The remainder of her pregnancy was uncomplicated, and she delivered a healthy term infant. This case highlights the diagnostic and therapeutic challenges of MDR, cephalosporin-resistant Salmonella Typhi infection during pregnancy and supports meropenem as an effective and potentially life-saving treatment associated with favourable maternal and fetal outcomes.
Long-term mortality and cause of death in people with tuberculosis compared with matched controls with influenza or non-typhoid salmonellosis in Australia: a retrospective cohort study.
Tuberculosis (TB) patients are thought to experience excess long-term morbidity and mortality. However, the long-term mortality of people with TB has not been compared with other common infections to assess their overall and disease-specific causes of death. All people diagnosed with TB between 2000 and 2015 in New South Wales, Australia, were matched by age, sex and year of infection to notified cases of influenza or non-typhoid salmonellosis. Patients were linked to the Australian mortality register to assess date and cause of death, up until the end of 2022. The comparative mortality ratio (CMR) for each cohort was calculated and Kaplan-Meier survival curves constructed. The standardised mortality rate (SMR) was estimated using national mortality data. The CMR of 7386 people with past TB was marginally increased (1.16 with 95% CI 1.10 to 1.22) compared with non-typhoid salmonellosis but reduced compared with influenza (0.88 with 95% CI 0.84 to 0.93). Most deaths occurred within 10 years of the notified infection. For TB patients, 1162 of 1331 people (87.3%) died in the 10 years after TB diagnosis. Risk factors for long-term mortality among TB patients included age over 40 years, male sex, sputum smear-positive and intracranial disease. Respiratory causes of death, including chronic obstructive pulmonary disease, lung cancer and pneumonia, were increased for TB patients, while influenza patients suffered more cardiovascular deaths. The SMR for TB patients (compared with the Australian general population) was increased by 18.7 times for infectious causes of death, 2.8 times for respiratory causes and 1.4 times for cancer-related mortality. Despite successful treatment, people with TB often experience post-TB sequelae and increased long-term mortality, with increased risk of respiratory causes of death. This highlights the need for appropriate post-TB respiratory follow-up, particularly in the first 10 years after TB diagnosis.
Country-level perspectives and priorities to guide the development of Invasive non-Typhoidal Salmonella (iNTS) vaccines: Insights from seven countries.
Invasive non-typhoidal Salmonella (iNTS) disease typically presents as a non-specific febrile illness that can progress to bloodstream infections and carries a high case fatality rate. Early detection improves patient outcomes, however, in resource-constrained settings, limited diagnostic capacity leads to underdiagnosis, insufficient incidence data, and incomplete epidemiological and socio-economic research on iNTS. Although no licensed vaccines currently exist, several candidates are in pre-clinical and clinical development. In the early stages of vaccine development, it is essential to consider country-level perspectives and priorities to guide the development of novel vaccines. To support this, the World Health Organization's Vaccine Innovation Framework was applied to conduct a consultation with 40 stakeholders representing the national immunization programmes in Burkina Faso, Ghana the Gambia, the Democratic Republic of the Congo (DRC), Kenya, Malawi and Mozambique. The consultation aimed to assess the stakeholder perceptions on the need for an iNTS vaccine and to evaluate its desirability, suitability, and evidence requirements from a low- and middle-income country (LMIC) perspective. Stakeholders argued that although an iNTS vaccines is desirable, the lack of nationally available epidemiological data is a barrier to accurately understanding the disease burden. A novel vaccine would be more desirable if it aligns with the preferred product characteristics specified, and helps mitigate, rather than exacerbate, current immunization programme challenges. They also expressed a preference for combination vaccines that align programmatically, including formulations combining iNTS with typhoid conjugate vaccine (TCV), to reduce the number of injections and simplify delivery logistics. To make evidence-based decisions on potential introduction, stakeholders emphasized the need for robust data on the value and impact of iNTS vaccines.
