[A case of allergic bronchopulmonary aspergillosis complicated by pinworm infection in a child: immune interaction mechanism and stepwise treatment strategy].
This article reports a rare pediatric case of allergic bronchopulmonary aspergillosis (ABPA) complicated by concurrent pinworm infection, highlighting immune interaction and individualized, stepwise management. A 10-year-7-month-old girl presented with recurrent cough and wheezing; laboratory testing showed marked eosinophilia (1.23×109/L) and elevated total immunoglobulin E (1 196 IU/mL). Aspergillus fumigatus IgG was 88.61 AU/mL, and Aspergillus fumigatus allergen m3-specific IgE was 0.37 IU/mL. Stool parasite examination confirmed pinworm infection. Chest computed tomography demonstrated atypical segmental pulmonary consolidation. Through multidisciplinary collaboration, stepwise anti-infective therapy (cefoperazone-sulbactam followed by imipenem and then voriconazole), pulsed anthelminthic therapy (albendazole), and immunomodulatory treatment were implemented, resulting in complete symptom resolution and sustained remission over 30 months of follow-up. Establishment of a novel murine model of allergic bronchopulmonary aspergillosis using an eosinophilic mutant mouse.
Allergic bronchopulmonary aspergillosis (ABPA) is a hypersensitivity lung disorder caused by chronic exposure to Aspergillus fumigatus (Af). Existing mouse models do not fully capture the severity of ABPA. This study developed a novel murine model of ABPA using the spontaneously eosinophilic Yama mouse. Female Yama and BALB/c mice were sensitized and challenged with Asp f1, a major allergen derived from Af. Peripheral eosinophil counts, serum IgE levels, and histopathological changes in the lungs, including goblet cell hyperplasia, mucus plugging, collagen deposition, eosinophil cationic protein (ECP), and CD117/c-kit expression, were assessed. Cytokine and fibrosis-related gene mRNA expression of interleukin-4 (IL-4), IL-5, IL-6, IL-13, transforming growth factor-β (TGF-β), and Col1a1 was also measured. Compared to BALB/c mice, Asp f1-challenged Yama mice exhibited sustained eosinophilia, increased IgE, pronounced goblet cell hyperplasia with mucus plugging, collagen deposition, and elevated eosinophil cationic protein (ECP) expression. Yama mice also showed more significant peribronchial inflammation and pulmonary fibrosis. RT-PCR analysis demonstrated substantial increases in IL-4, IL-5, IL-6, IL-13, and Col1a1 transcripts, indicating a Th2-dominant immune profile and enhanced fibrotic activity. The minimal differences in TGF-β expression between Asp f1- and saline-treated mice indicate that fibrosis in Yama mice may proceed via a TGF-β-independent pathway. These findings establish the Yama mouse as a novel model for ABPA research, closely replicating the immunopathological features of severe human ABPA. This model provides a reliable platform to investigate eosinophil-driven lung fibrosis mechanisms and evaluate hypersensitivity responses contributing to human ABPA onset and progression.
Iron induces Aspergillus proteases in cystic fibrosis allergic bronchopulmonary aspergillosis.
Allergic bronchopulmonary aspergillosis (ABPA) is a hypersensitivity disease triggered by inhaled Aspergillus -fumigatus (Af) spores (i.e., conidia), in persons with cystic fibrosis (CF). High iron levels have been commonly described in CF respiratory secretions. Af proteases cause a severe allergic immune response, leading to pulmonary exacerbations and progressive lung function decline. The stimuli that promote Af proteases in ABPA are poorly understood. Determine if airway iron is a critical factor in CF-ABPA by regulating fungal protease production. We used transcriptomics and proteomics to investigate the role of iron in stimulating the production of Af proteases and evaluated the impact of these iron-induced Af proteases on the Th2 immune response in CF bronchial epithelial (CFBE) cells and in a CF-ABPA mouse model. Transcriptional analysis showed a significant upregulation of Af proteases with exposure to iron and a downregulation with iron chelation. This finding was validated in secretome studies showing that Af-culture filtrates (Af-cf) from iron-exposed conidia contained significantly higher levels of 14 proteases compared to controls. Exposure to iron-primed Af-cf increased the production of Th2 promoting cytokines (IL-33, IL-25 and TSLP) in CFBE cells compared to the wild type cells. In vivo, inoculation of CF mice with iron-primed Af conidia led to higher levels of serum IgE and Th2 cytokines (IL-4, IL-5, IL-9, IL-13) in lung tissue. These findings suggest that airway iron is a potent stimulus for Af proteases and represents a modifiable risk factor and potential therapeutic target for CF-ABPA.

Pharmacist-Led Full-Course Pharmaceutical Care Model in Allergic Bronchopulmonary Aspergillosis: A Case Report-Based Approach to Optimizing Multidisciplinary Management.
The management of allergic bronchopulmonary aspergillosis (ABPA) is complicated by prolonged, multi-drug regimens that carry a high risk of multi-system adverse drug reactions (ADRs). This complexity underscores the need for integrated, continuous pharmaceutical care to optimize safety and efficacy, yet standardized models for such care are lacking. We report the case of a patient with ABPA and comorbid chronic hepatitis B, who had a history of failed ABPA therapy. During the current therapeutic course, the patient not only sequentially developed steroid-induced diabetes, drug-induced liver injury, hyperlactatemia, visual disturbances, phototoxicity, and hypokalemia but also progressed to treatment-dependent ABPA. The patient successfully completed the over-two-year protocol through a pharmacist-led full-course pharmaceutical care model that integrated regimen evaluation, efficacy monitoring, comprehensive adverse reaction management, discharge education, and follow-up. This case demonstrates that a full-course pharmaceutical care model enables timely detection and management of complex ADRs, optimizes therapeutic outcomes, and supports multidisciplinary decision-making. The model illustrates the pivotal role of the clinical pharmacist in improving medication safety and efficacy in ABPA, offering a replicable framework for promoting integrated, patient-centered care in pulmonary fungal diseases.
