Conotruncal heart malformations

A group of congenital heart defects involving the outflow tracts. Examples include truncus arteriosus communis, double-outlet right ventricle and transposition of great arteries. Truncus arteriosus communis is characterized by a single outflow tract instead of a separate aorta and pulmonary artery. In transposition of the great arteries, the aorta arises from the right ventricle and the pulmonary artery from the left ventricle. In double outlet of the right ventricle, both the pulmonary artery and aorta arise from the right ventricle.

Common trunk branching from the renal artery to the diaphragm, adrenal gland, and testis: a case report with embryological hypotheses by observation.

The vascular systems of the kidneys and gonads are highly susceptible to variations because of the degeneration of the mesonephros and mesonephric artery and the progressive ascending development of the metanephros. In this report, we present a rare case of a common arterial trunk originating from the left renal artery and supplying the left diaphragm, adrenal gland, and testis. Anatomical dissection of a 95-year-old male was conducted during the 2023 academic year at Tokai University Medical School. The left renal artery had a common trunk that bifurcated to superior and inferior branches, forming a T-shape. The superior branch was distributed in the left adrenal gland and diaphragm, whereas the inferior branch was distributed in the left testis. This anatomical variation likely results from the remnant of the mesonephric artery network structure, particularly the rete arteriosum urogenitale. Understanding such variations is crucial for radiologists and surgeons to avoid potential complications during diagnostic procedures and surgical interventions.

Coronary artery anomalies in common arterial trunk: proposal of a new anatomical classification.

Coronary artery abnormalities (CAAs) are frequently encountered in common arterial trunk (CAT), with an estimated incidence of 5-20%. However, their prognostic implications remain unclear. Surgical challenges potentially arise due to coronary arteries crossing the right ventricular outflow tract (RVOT), close proximity of the coronary and pulmonary orifices, and distortion to the proximal coronary segments and ostia during arch reconstruction or truncal valve replacement. Some studies have demonstrated that CAAs confer worse outcomes after CAT repair, while others have reported no significant prognostic impact. Both the number and subtype of CAAs may influence outcomes, but heterogeneous categorisation limits the conclusions that can be drawn from existing studies. A uniform classification of CAAs in CAT is warranted to better ascertain the prognostic impact of CAA burden and morphology. This may enable more focused decision-making in clinical scenarios where a high-risk CAA pattern is suspected. For example, it may help inform the intraoperative trade-off between probing the coronaries to define their precise morphology against the risk of causing damage. We propose a classification consisting of six abnormalities: A) single coronary artery, B) ostial stenosis, C) intramural course, D) juxtacommissural origin, E) coronary crossing RVOT, and F) close proximity of coronary and pulmonary orifices.

Right Ventricular Outflow Tract Reconstruction in Truncus Arteriosus-With or Without a Conduit? A Systematic Review and Meta-Analysis.

Background: Surgical repair of truncus arteriosus (TA) requires right ventricle-to-pulmonary artery continuity, achieved through either conduit-based (CB) reconstruction or a nonconduit-based (NCB) approach. As the optimal right ventricular outflow tract (RVOT) strategy is still debatable, we conducted a meta-analysis to compare outcomes. Methods: A systematic literature search was performed focusing on studies that compared CB and NCB RVOT reconstruction techniques for the primary correction of TA. Statistical analyses were conducted using RevMan 8.13.0, with effect estimates reported as odds ratios (ORs) and mean differences, along with 95% confidence intervals (CIs). Results: We included seven studies comprising 360 patients, with 174 neonates (48.3%) allocated to the CB group. No significant difference was observed in early mortality [OR 0.61; 95% CI 0.13-2.92; P = .43; I² = 43%] or long-term mortality [OR 1.22; 95% CI 0.09-17.23; P = .83; I² = 78%] between CB and NCB RVOT reconstruction techniques. However, the incidence of RVOT reoperation [OR 3.70; 95% CI 2.02-6.76; P = .003; I² = 0%] and the overall reintervention rate, including both surgical and catheter-based interventions, were higher in the CB cohort [OR 2.45; 95% CI 1.33-4.52; P = .01; I² = 0%]. Conclusion: While CB and NCB approaches yield similar early and long-term survival outcomes, CB reconstruction is associated with a significantly higher risk of RVOT reoperation and overall reintervention. These findings suggest that NCB approaches may offer superior long-term durability, reducing the need for subsequent interventions in neonates undergoing primary repair of TA.

Direct Anastomosis Versus Conduit Repair for Right Ventricular Outflow Tract Reconstruction in Common Arterial Trunk: A Meta-Analysis of Reconstructed Time-to-Event Data.

Repair of common arterial trunk (CAT) involves establishing the right ventricular outflow tract (RVOT) using either a conduit or a direct right ventricle-pulmonary artery (RVPA) anastomosis (DA). Conduits offer a valved pathway but are limited by durability and availability. The comparative outcomes of these 2 techniques remain uncertain. This work assessed whether DA improves survival, reduces complications and reintervention outcomes compared to conduit repair. PubMed, Web of Science, EMBASE, and Cochrane Central were searched for studies comparing conduit versus DA for RVOT reconstruction from February 20, 2025 to March 30 30, 2025. The primary outcome was early mortality; secondary outcomes included haemodynamics, recovery, and complications. Time-to-event data were reconstructed from Kaplan-Meier curves. Pooled hazard ratios (HRs), risk ratios (RRs), or mean differences (MDs) with 95% confidence intervals (CIs) were calculated using random-effects models. Eleven studies (767 patients; 419 conduit, 348 DA) were included. Early mortality (RR = 0.61, 95% CI, 0.26-1.44, P = .220) and long-term survival (HR = 1.11, 95% CI, 0.61-2.02, P = .738) were similar. Reoperation was more frequent in the conduit group (HR = 1.77, 95% CI, 1.05-3.01, P = .034). Conduit repair required longer ventilation (MD = 3.44 days, P = .010) and hospitalization (MD = 4.77 days, P = .030), with comparable ICU stay and RVOT growth. Truncal valve insufficiency (RR = 0.13, P = .130 for truncal valve vs conduit) was similar in incidence following DA. Conduit and DA repairs yield similar survival and postoperative complications in CAT, while DA offers fewer reoperations and faster recovery. Data from future prospective multicentre trials will support decision-making.

Anomalous systemic venous drainage in truncus arteriosus - An unusual association.

Truncus arteriosus (TA) is a major cyanotic heart malformation infrequently associated with aortic arch interruption and isolation of the branch pulmonary artery. Anomalous superior vena caval drainage to the left atrium (LA) is hitherto unreported in TA. In a 45-day-old infant diagnosed with TA and cyanosis, anomalous drainage of the right superior vena cava into the LA was not identified preoperatively or intraoperatively. It was detected postoperatively due to persistent hypoxemia. The anomaly posed unique challenges in the subsequent management. This case report highlights the unusual association of systemic venous drainage in TA.
