A Critical Four-Hour Therapeutic Window Predicts Delayed Encephalopathy Risk After Carbon Monoxide Poisoning: A Multicenter Retrospective Cohort Study.
The therapeutic window for preventing delayed encephalopathy after carbon monoxide poisoning (DEACMP) remains unclear. We aimed to define this temporal risk relationship and establish an intervention threshold using machine learning. In this multicenter retrospective cohort study (n = 1755), a gradient boosting model for predicting DEACMP was developed (n = 1654) and externally validated (n = 101). Performance was assessed using the area under the receiver operating characteristic curve (AUC) and interpreted using Shapley Additive exPlanations (SHAP). The exposure-to-treatment interval was the most powerful predictor of DEACMP risk. Intervention within four hours emerged as the most critical variable influencing risk (SHAP analysis). The model demonstrated robust discrimination in the training (AUC = 0.944, 95% CI, 0.926-0.960), internal validation (AUC = 0.849, 95% CI, 0.785-0.905), and external validation (AUC = 0.872, 95% CI, 0.772-0.946) sets. Treatment delay is the primary modifiable risk factor for DEACMP following CO poisoning. The identified critical four-hour therapeutic window provides the first quantitative, evidence-based benchmark to inform clinical guidelines and optimize emergency response strategies aimed at preventing delayed neurological sequelae.
Study on the construction of a predictive model for delayed encephalopathy after acute carbon monoxide poisoning.
Acute carbon monoxide poisoning (ACOP) commonly results in delayed encephalopathy (DE). This study aimed to develop a predictive model to estimate the odds of DE in patients with ACOP. Clinical data were retrospectively collected from 102 ACOP patients admitted to a single institution between January 1, 2020, and June 30, 2024. The least absolute shrinkage and selection operator regression was used to identify influencing factors, and then a prediction model based on logistic regression was constructed. The discrimination, calibration and clinical practicality of the model were respectively evaluated according to the area under the receiver operating characteristic (ROC) curve (AUC), calibration curve, decision curve analysis (DCA) and clinical impact curve (CIC). A total of 102 ACOP patients were included in the study, of who 15 developed DE. Length of stay, comorbidities (e.g., hypertension, diabetes), carboxyhemoglobin, and hyperbaric oxygen therapy after discharge were influencing factors. The four-variable model displayed high prediction ability with the AUC of 0.933 [95% confidence interval (CI): 0.877–0.991]. The calibration curve (bootstraps = 1,000) showed good calibration with a brier score of 0.066 (95%CI: 0.030–0.102). In addition, the DCA and CIC also showed good clinical practicality. The prediction model constructed in this study showed good discrimination, calibration and clinical practicality, which could help medical personnel identify DE in patients with ACOP. Predictive value of acute MRI-detected brain lesions for clinical outcome in patients with acute carbon monoxide poisoning.
To investigate the predictive value of acute brain lesions (ABLs) for the clinical outcomes and delayed encephalopathy after acute carbon monoxide poisoning (DEACMP). This is a retrospective cohort study, patients with acute carbon monoxide (CO) poisoning were divided into the ABLs group and non-ABLs group. Outcome measures included clinical cure, favorable prognosis, and the incidence of DEACMP. Multivariate logistic regression analysis was used to identify independent predictors of clinical outcomes, and subgroup analyses were performed to clarify the impact of ABLs on clinical outcomes. Multivariate analysis showed that ABLs (clinical cure: adjusted odds ratio [aOR] 0.35, 95% confidence interval [CI] 0.17-0.75; DEACMP [DEACMP: aOR 2.54, 95% CI 1.08-6.00) and time from onset to first hyperbaric oxygen therapy (HBOT) > 6 h (clinical cure: aOR 0.38, 95% CI 0.18-0.82; DEACMP: aOR 3.50, 95% CI 1.58-7.74) were associated with a lower clinical cure rate and a higher incidence of DEACMP. A Glasgow Coma Scale (GCS) score < 9 (aOR 7.50, 95% CI 3.33-16.89) was an independent predictor of DEACMP, but was not associated with clinical cure. Regarding ABLs in different locations, white matter lesions were significantly associated with lower clinical cure (adjusted OR = 0.23, 95% CI 0.10-0.55), lower favorable outcome (adjusted OR = 0.34, 95% CI 0.17-0.68), and higher DEACMP development (adjusted OR = 3.17, 95% CI 1.59-6.32). In contrast, basal ganglia lesions were only significantly correlated with lower clinical cure (adjusted OR = 0.45, 95% CI: 0.21-0.96), but not significant with favorable outcome or DEACMP occurrence. ABLs and time from onset to first HBOT were significant predictors of clinical cure and DEACMP in patients. A GCS score < 9 was significantly associated with DEACMP but showed no definitive correlation with clinical cure.
Optimizing hyperbaric oxygen initiation time in carbon monoxide poisoning: a 3-hour window enhances neurological recovery via lactate clearance.
To explore the optimal initiation time of hyperbaric oxygen therapy (HBOT) for moderate-to-severe acute carbon monoxide poisoning (ACOP) and to assess the prognostic value of lactate clearance and early MRI findings. This single-center retrospective study included 12 ACOP patients (2020-2023) treated with HBOT within 6 h after admission. Patients were categorized by HBOT initiation time: early (≤3 h, n=8) and delayed (>3 h, n=4). Clinical, biochemical, and imaging data were analyzed. Primary outcomes were time to regain consciousness and Barthel Index at 6 months. Median HBOT initiation was 112 min. Early treatment was associated with faster organ function recovery (greater SOFA score reduction, p<0.05). Lactate normalized within a median of 17.5 h, and clearance >50 % in 24 h correlated with better neurological outcomes (p=0.002). MRI detected early globus pallidus injury more sensitively than CT. At 6 months, 83.3 % recovered functionally; one developed delayed encephalopathy. Early HBOT initiation (≤3 h) may facilitate metabolic and neurological recovery in ACOP. Early lactate clearance and MRI findings may serve as prognostic markers. Given the small sample size and absence of a non-HBOT control group, these results are exploratory and require confirmation in larger studies.
Delayed-onset parkinsonism after carbon monoxide poisoning: neuropsychological perspectives from a case report.
This case report presents the clinical evolution of a patient who developed delayed-onset parkinsonism and cognitive deficits following carbon monoxide poisoning (COP). After a symptom-free interval of approximately one month, the patient exhibited progressive neurological and cognitive impairments that significantly interfered with daily and occupational functioning. A combined therapeutic approach was implemented, involving pharmacological treatment, cycles of hyperbaric oxygen therapy (HBOT) and a tailored neuropsychological rehabilitation program. Longitudinal assessments documented progressive improvements, particularly in sustained attention, memory, and executive functioning, although residual difficulties persisted. This case highlights the relevance of individualized neuropsychological interventions in conjunction with medical treatment in facilitating functional recovery in patients with delayed neurological sequelae after COP.
