A paralisia de Bell é um tipo de paralisia facial que resulta em uma incapacidade temporária de controlar os músculos faciais no lado afetado do rosto. Os sintomas podem variar de leve a grave. Eles podem incluir contração muscular, fraqueza ou perda total da capacidade de mover um ou, em casos raros, ambos os lados do rosto. Outros sintomas incluem a queda da pálpebra, alteração do paladar e dor ao redor da orelha. Normalmente, os sintomas aparecem em 48 horas. Psychosocial impact of chronic oral mucosal pain disorders: Associations with oral health related quality of life, anxiety, and depression.
Chronic oral mucosal pain disorders, including Burning Mouth Syndrome (BMS), Oral Lichen Planus (OLP), Atypical Odontalgia, and Persistent Idiopathic Facial Pain, are debilitating conditions that significantly impair oral health-related quality of life (OHRQoL). These disorders are frequently associated with psychological distress, particularly anxiety and depression; however, studies integrating assessments of OHRQoL with psychological morbidity remain limited. To evaluate the impact of chronic oral mucosal pain disorders on OHRQoL, anxiety, and depression, and to identify psychological predictors of impaired quality of life. A case-control study was conducted involving 52 patients with chronic oral mucosal pain disorders and 52 age- and sex-matched healthy controls. OHRQoL was assessed using the Oral Health Impact Profile-14 (OHIP-14), and psychological distress was measured using the Hospital Anxiety and Depression Scale (HADS). Group comparisons were performed using independent t-tests and chi-square tests. Multivariable linear regression identified independent predictors of OHIP-14 scores, and receiver operating characteristic (ROC) analysis evaluated the ability of OHIP-14 to detect depression. Patients showed significantly higher OHIP-14 scores than controls (28.3 ± 6.0 vs. 11.7 ± 5.0; p < 0.0001). Clinically relevant anxiety and depression were present in 84.6% and 80.8% of patients, respectively. Anxiety and depression independently predicted poorer OHRQoL, while demographic factors and symptom duration were not significant. OHIP-14 demonstrated good diagnostic accuracy for depression (Area under the curve (AUC) = 0.85). Chronic oral mucosal pain disorders (COMPD) are associated with marked impairment in quality of life and substantial psychological burden, underscoring the need for routine psychological screening in oral medicine practice.
Neuralgia-inducing cavitational osteonecrosis (NICO): A scoping review of a controversial concept in orofacial pain.
Neuralgia-Inducing Cavitational Osteonecrosis (NICO) is a debated diagnostic entity proposed as a cause of chronic orofacial pain. Despite ongoing clinical use, systematic evaluations integrating historical foundations and methodological gaps remain limited. A comprehensive search of PubMed/MEDLINE, Scopus, and Web of Science mapped NICO's development from 1979 to 2024. The search yielded 603 records; 41 studies were included after screening. Data extraction categorized studies by type, methodology, diagnostic criteria, and limitations. Methodological quality was assessed using Joanna Briggs Institute critical appraisal tools. The 41 studies revealed recurrent methodological limitations: absence of control groups, non-standardized diagnostic criteria, limited histological evidence, and interpretive bias. Proposed mechanisms such as RANTES/CCL5 overexpression lack independent validation. Diagnostic tools remain unvalidated, and surgical interventions continue based on unclear criteria. Evidence consists predominantly of low to moderate quality retrospective case series, with no randomized controlled trials. Many patients diagnosed with NICO may be more appropriately classified as having persistent idiopathic facial pain or trigeminal neuropathic pain according to International Classification of Orofacial Pain criteria. Current evidence is insufficient to support NICO as a distinct, validated clinical entity. The substantial physical and psychological risks of surgical intervention underscore the ethical imperative to base treatment decisions on validated evidence. Rigorous, prospective research with standardized diagnostic frameworks and appropriate controls is urgently needed. Clinicians should exercise caution, prioritizing evidence-based non-surgical management and avoiding irreversible surgical interventions without clear justification.
When persistent idiopathic facial pain is not idiopathic: Facial pain attributed to lung cancer and comparative review of classification systems.
Facial pain attributed to lung cancer (FPLC) is frequently misclassified as persistent idiopathic facial pain (PIFP). This study reviewed published cases, retrospectively applied existing criteria for PIFP to evaluate misclassification, characterized clinical features of FPLC, and proposed a diagnostic framework. A structured search of MEDLINE, Embase, and Web of Science through December 13, 2025, identified 51 English-language case reports of orofacial or pharyngeal pain secondary to lung cancer. Thirty-three cases contained sufficient detail to allow retrospective application of PIFP criteria from leading classification systems. Twenty-seven cases, 81.8%, met criteria for PIFP despite an underlying malignancy. Adding a requirement to exclude systemic disease substantially reduced misclassification. The proposed diagnostic criteria for FPLC demonstrated an 87.9% diagnostic accuracy. Idiopathic orofacial pain phenotypes overlap with FPLC. Excluding systemic causes improves the diagnostic validity of classification systems. The consistent clinical profile of FPLC supports the use of defined diagnostic criteria.
