Disseminated Mucormycosis with Characteristic Gastric Ulcers and Extensive Intravascular Invasion after ABO-Incompatible Living-Donor Liver Transplantation: A Case Report.
Mucormycosis is a life-threatening invasive fungal infection that occurs mainly in patients who are immunocompromised. Here, we report a case of mucormycosis with characteristic gastric ulcers and severe vascular invasion after ABO-incompatible living-donor liver transplantation. A male patient in his 50s with decompensated cirrhosis due to metabolic dysfunction-associated steatohepatitis underwent an ABO-incompatible living-donor liver transplantation. Rituximab (375 mg/m2) was administered 14 days before transplantation, and tacrolimus (1 mg/day) and mycophenolate mofetil (500 mg/day) were initiated 7 days before surgery. Plasma exchange was performed the day before transplantation. Postoperatively, tacrolimus, mycophenolate mofetil, and methylprednisolone were continued as immunosuppressants, and micafungin was administered as antifungal prophylaxis. The postoperative course was complicated by sepsis due to infectious enteritis, which required broad-spectrum antibiotics. Two episodes of T cell-mediated rejection occurred, both treated with steroid pulse therapy. On POD 39, esophagogastroduodenoscopy was performed for persistent anemia and bloody drainage from the nasogastric tube. Multiple extensive gastric ulcers surrounded by dark brown mucosa were observed. Biopsy specimens revealed broad aseptate hyphae with wide-angle branching, consistent with mucormycosis. Rhizopus microsporus, the causative organism of mucormycosis, was isolated from gastric juice culture, confirming the diagnosis. Contrast-enhanced CT demonstrated poor enhancement of the gastric wall and multiple pulmonary nodules, suggesting disseminated fungal infection. Intravenous liposomal amphotericin B was initiated; however, on POD 56, the patient developed septic shock and died of multiple organ failure on POD 63. Autopsy revealed mucormycosis in the stomach, lungs, adrenal glands, thyroid gland, and kidneys. Cobblestone-like fungal masses were identified along the thoracic aortic intima, indicating extensive vascular invasion. This case represents a breakthrough mucormycosis infection that developed despite antifungal prophylaxis. The disease manifested as characteristic gastric ulcerations and extensive vascular invasion, leading to fatal outcomes despite liposomal amphotericin B therapy. Necrotic gastric ulcers and positive gastric juice cultures provide critical diagnostic clues. In highly immunosuppressed liver transplant recipients, atypical gastric findings should raise suspicion for mucormycosis, and early systemic evaluation is essential to improve otherwise poor prognoses.
Conidiobolomycosis: apparent yet overlooked. A report of two cases.
Conidiobolomycosis is a rare, chronic subcutaneous fungal infection caused primarily by Conidiobolus coronatus and Conidiobolus incongruus. It predominantly affects immunocompetent individuals and presents as slowly progressive rhino-facial swelling. Due to its indolent nature and rarity, it is often misdiagnosed, leading to delayed treatment and potentially disfiguring outcomes. We report two successfully treated cases of conidiobolomycosis from Sri Lanka. The first patient, a 27-year-old immunocompetent male, presented with a six-month history of left-sided nasal obstruction, rhinorrhea, and gradual, painless swelling that initially affected the left nasal cavity and later extended to the left cheek, infraorbital region, and right nasal side. Histopathological examination of excision biopsy specimens revealed broad, aseptate fungal hyphae with associated necrotizing granulomatous inflammation and dense eosinophilic infiltrates. He was treated with oral itraconazole 200 mg twice daily and saturated potassium iodide solution (1 ml three times daily). Clinical symptoms completely resolved after six weeks, and therapy was continued for an additional month before cessation. The second patient, a 59-year-old previously healthy female, presented with a five-month history of progressive left-sided nasal congestion and disfiguring swelling over the nose. Clinical examination and radiological assessment revealed soft tissue thickening in the nasolabial fold extending into the nasal septum. Biopsy samples showed granulomatous inflammation with broad, sparsely septate fungal hyphae, confirmed on Grocott-Gomori methenamine silver (GMS) staining. Direct microscopy with 10% potassium hydroxide (KOH) demonstrated thin-walled, hyaline hyphae with irregular branching and few septations. She also recovered completely with itraconazole treatment. Both patients achieved complete resolution with medical management, emphasizing that early recognition and targeted antifungal therapy can lead to excellent outcomes in conidiobolomycosis. Despite its rarity, this infection should be considered in patients presenting with persistent rhino-facial swelling, especially in endemic areas. While the prognosis is generally good, prolonged therapy may be necessary, and cosmetic deformities may persist. Increased clinical awareness and timely diagnosis are essential to prevent complications and disfigurement associated with this overlooked fungal infection.
Liposomal amphotericin B in the treatment of pediatric gastrointestinal basidiobolomycosis.
Gastrointestinal basidiobolomycosis is a rare fungal infection caused by Basidiobolus ranarum, increasingly recognized as an intestinal pathogen in children. Its clinical features closely mimic malignancy or inflammatory bowel disease, leading to frequent diagnostic delays. Although endemic in regions such as Saudi Arabia, Iran, and Oman, sporadic cases occur worldwide. Amphotericin B-liposomal remains a cornerstone therapy for invasive fungal disease, but no standardized treatment protocol for pediatric gastrointestinal basidiobolomycosis has been established. We report a 6-year-old Asian girl, born preterm with very low birth weight, who presented with persistent abdominal pain without organomegaly, alternating diarrhea and constipation, fever, and anorexia. Initial ultrasonography revealed colonic wall thickening with aneurysmal dilation at the splenic flexure. Colon biopsy confirmed Basidiobolus ranarum. The patient received intravenous liposomal amphotericin B (90 mg daily), with partial radiologic improvement but persistent symptoms, prompting exploratory laparotomy. Adhesiolysis, left hemicolectomy, and resection of jejunal and colonic masses were performed. Histopathology demonstrated dense eosinophil-rich granulomatous inflammation with multinucleated giant cells and a solitary fungal hypha, without classic Splendore-Hoeppli phenomenon, likely due to prior antifungal therapy. Postoperatively, the patient was discharged on oral itraconazole for 28 days with adjunctive cotrimoxazole. At 1-year follow-up, she remained asymptomatic with no evidence of recurrence. This case highlights the diagnostic challenges of pediatric gastrointestinal basidiobolomycosis and demonstrates successful management with liposomal amphotericin B followed by itraconazole. It underscores the ongoing gap in establishing standardized treatment and supports further prospective studies to define optimal antifungal and surgical strategies for this rare entity.
Treatment of pulmonary mucormycosis: current concepts.
Pulmonary mucormycosis (PM) is a rapidly progressive angioinvasive fungal infection with high mortality. Despite therapeutic advances, optimal management remains uncertain. We review the evidence on managing PM. We emphasize aggressive host factor optimization (glycemic control, immunosuppression reduction) and outline the evidence-based antifungal strategy: liposomal amphotericin B (L-AMB) for induction, followed by oral maintenance (posaconazole or isavuconazole). We discuss critical nuances, including LAMB dosing, timing, and duration of therapy, drug interactions, and therapeutic drug monitoring. We review the role of combination antifungals, newer agents, and adjunctive modalities (inhaled antifungals, immunomodulation, bronchoscopic interventions, and iron chelation), assessing their appropriate use. The timing, indications, feasibility, and perioperative risk of surgery are discussed. We provide practical guidance for PM in special populations, including children, pregnant women, individuals with chronic liver or renal disease, and transplant recipients. Optimal care of PM requires early recognition, prompt LAMB induction with rigorous host factor optimization, multidisciplinary surgical assessment, followed by oral triazole maintenance. Combination antifungal therapy lacks definite evidence and should be reserved for severe or refractory cases. Future research priorities include prospective evaluation of treatment strategies, host-directed therapies, emerging antifungals (oral nanocrystal amphotericin, fosmanogepix, etc.), and systematic assessment of adjunctive modalities.
An outbreak of ovine conidiobolomycosis in the Brazilian Amazon.
Conidiobolus sp. causes conidiobolomycosis, an emerging invasive fungal disease that affects humans and animals, mainly in tropical regions. In sheep, the disease has a major economic impact. We report an outbreak of conidiobolomycosis on a sheep farm in the Brazilian Amazon, as well as give a brief review of the subject. Veterinary care was requested on a rural property on which 3 of 35 sheep had developed prostration, facial edema, and exophthalmos. Two of the sick animals died, and a third was referred to the Veterinary Hospital of the Federal University of Acre (Rio Branco, Acre, Brazil). After imaging tests, the animal was euthanized given the advanced clinical stage of the disease. The main autopsy findings were rhinosinusitis with caseous necrosis, destruction of the nasal turbinates, and pulmonary granulomas. Based on our histologic, immunologic, microbiologic, and molecular tests, the outbreak was confirmed to be caused by Conidiobolus lamprauges, a saprozoonotic agent that has not been reported previously in northern Brazil, to our knowledge.
