Systemic lupus erythematosus 11

A chronic, relapsing, inflammatory, and often febrile multisystemic disorder of connective tissue, characterized principally by involvement of the skin, joints, kidneys and serosal membranes. It is of unknown etiology, but is thought to represent a failure of the regulatory mechanisms of the autoimmune system. The disease is marked by a wide range of system dysfunctions, an elevated erythrocyte sedimentation rate, and the formation of LE cells in the blood or bone marrow.

Clinical, laboratory, and histopathological characteristics of pediatric lupus nephritis: a retrospective study in a national referral center in Mexico.

Pediatric lupus nephritis (LN) remains a major cause of morbidity and mortality, yet data from Latin American populations are limited. This study aimed to describe the clinical, laboratory, and histopathological characteristics of pediatric LN and identify prognostic factors associated with renal replacement therapy (RRT). We conducted a retrospective cross-sectional study including patients <18 years of age with LN diagnosed between 2020 and 2024 at a national referral center in Mexico. Demographic, clinical, immunological, histopathological, and therapeutic variables at diagnosis were analyzed. Multivariable logistic regression was performed to identify predictors of RRT. Eighty patients were included (83% female; mean age 15.1 ± 2.8 years). Median proteinuria was 41 mg/m²/h; hematuria and leukocyturia were present in 46% and 26% of patients, respectively. All patients were ANA positive, with frequent hypocomplementemia and elevated anti-double-stranded DNA titers. Among biopsied patients, class IV was the most common histological subtype (60%). Proliferative forms were associated with reduced glomerular filtration rate (<90 mL/min/1.73 m²; p = 0.012) and higher activity index scores (p = 0.04), while chronicity indices were low. Fifteen patients (18.8%) required RRT, and mortality was 6.25%. In multivariable analysis, hypoalbuminemia (<2.5 g/dL) was independently associated with RRT (OR 6.04; 95% CI 1.33-27.50; p = 0.020). This study represents one of the largest pediatric LN cohorts reported from Mexico. Proliferative forms were associated with greater inflammatory activity and impaired renal function at diagnosis. Hypoalbuminemia emerged as a simple and accessible biomarker for early risk stratification of severe renal outcomes.

Active Pediatric Systemic Lupus Erythematosus in Fars Province, Iran: Associations With Geographical and Meteorological Determinants-A Retrospective Cross‑Sectional Study.

Pediatric systemic lupus erythematosus (SLE) is a complex autoimmune disease influenced by genetic and environmental factors such as sunlight and temperature. This retrospective study investigated the association between meteorological and geographical factors and active pediatric SLE in Fars province, southwest Iran. The residential addresses of pediatric patients with active SLE who were hospitalized at the main referral hospital of southwest Iran in Shiraz City between March 2016 and January 2023 were extracted from their medical records and geographically mapped. The influence of meteorological factors, such as temperature, humidity, evaporation, rainfall, as well as geographical parameters including land cover, slope, and altitude, on active pediatric SLE was evaluated using geographic information system (GIS) analysis. Data were analyzed using univariate and multivariate binary logistic regression analysis. This study included 143 pediatric patients with active SLE from 34 out of a total of 8181 city/village areas. There was a significant positive association between urban setting and active pediatric SLE (OR = 75.949, CI = 9.521-605.846) while mean annual rainfall demonstrated a significant negative association (OR = 0.997, CI = 0.994-1.000) in the univariate analysis. Multivariate analysis revealed that urban setting was the only significant factor positively correlated with active pediatric SLE (OR = 56.567, CI = 6.731-745.372). There was no association between other meteorological and environmental factors and active pediatric SLE. This study found that living in urban areas and lower annual rainfall are significant risk factors for active pediatric SLE in Fars province, probably due to increased exposure to pollutants and ultraviolet radiation. While previous studies have linked SLE activity to temperature and humidity, no such associations were observed here, highlighting the need for further research on regional environmental influences on SLE.

CAR T cells in pediatric systemic lupus erythematosus.

Pediatric systemic lupus erythematosus (SLE) is associated with high disease severity and significantly higher morbidity and mortality compared to adult-onset SLE, making it a particularly challenging condition to manage. Chimeric antigen receptor (CAR) T-cell therapy holds significant promise for improving outcomes in refractory pediatric SLE. However, the use of CAR T-cell therapy in pediatric patients presents unique challenges, including difficulty in collecting sufficient T cells. Pediatric patients may experience distinct side effects, such as age-specific toxicities or more pronounced neurotoxic events, requiring tailored monitoring and management. Additionally, the psychological impact on children and their families, facing potentially life-threatening treatment protocols, must be carefully addressed. To date, only four children with SLE have been treated with CAR T cells, highlighting the early stage of research in this area. The clinical outcomes provide hope that CAR T-cell therapy could offer a breakthrough in treating pediatric SLE; however, main limitations remain the small number of patients and the relatively short follow-up. More data are needed to fully assess the safety, efficacy, and long-term outcomes in this vulnerable population. Key Points • Pediatric SLE is characterized by more severe manifestations, higher morbidity, and increased mortality compared to adult-onset SLE. • CAR T-cell therapy shows promise as a potential treatment for refractory pediatric SLE aiming ideally towards long-term, drug-free remission. • CAR T-cell therapy in pediatric SLE presents unique challenges, including age-related toxicities, technical difficulties, and ethical considerations. • To date, results are available from only a handful of children with SLE treated with CAR T cells. Expected advancements in CAR T-cell technology, along with data from ongoing clinical trials involving children with SLE, will determine the pace and extent of CAR T-cell therapy application in pediatric SLE.

Case Report: Rare aplastic anemia in pediatric systemic lupus erythematosus: a case series and systematic literature review.

Systemic lupus erythematosus (SLE) is a complex autoimmune disease in which the immune system mistakenly attacks healthy cells and tissues. This condition can impact various organs and systems, including the hematological system, where aplastic anemia (AA) emerges as a particularly severe and rare complication. Relevant clinical manifestations and treatment experiences must be shared and updated urgently. This manuscript presents the clinical features, examination, and treatment of two pediatric SLE patients with AA. And a systemic search of MEDLINE, EMBASE, LILACS, SciELO, and Scopus, using MeSH headings and keywords for "Aplastic Anaemia" and "Systemic Lupus Erythematosus". Both of our cases were severe, resulting in one death. Our systematic literature review identified 32 eligible studies. We found a total of 38 patients, and along with the two patients from our case series, there were 9 out of 40 SLE patients who experienced alopecia areata as children. In all 40 patients, 52% (21 out of 40) developed AA at the onset of SLE. The infection rate was concerning, with 16 out of 40 patients affected. While 83% (33 out of 40) of the patients showed improved outcomes, all seven patients who died had contracted an infection. SLE with AA is rare in both children and adults. Although the prognosis of this dangerous disease is optimistic in terms of data, it is still necessary to be vigilant, and early bone marrow examination, infection prevention, and on-time treatment are crucial.

Association of Mitochondrial DNA Haplogroups with Pediatric Systemic Lupus Erythematosus Disease Activity, Damage Scores, and Lupus Nephritis.

Mitochondria, which have critical roles in energy metabolism and oxidative regulation, also have a role in immune regulation including T cell activation, NET formation, inflammation, and apoptosis. More than 50% of those with systemic lupus erythematosus (SLE) have lupus nephritis due to kidney damage from immune complex deposition. Disease severity is reported to be greater in certain lineages. Mitochondrial DNA (mtDNA) haplogroups, which reflect maternal lineages, may modulate immune balance and disease outcomes in SLE. Methods: DNA was extracted from 25 consecutive, consenting pediatric patients that fulfilled the 1997 criteria for SLE and their maternal mitochondrial DNA (mtDNA) haplogroups were determined through next-generation sequencing (NGS). Results: This study evaluated the associations between mtDNA haplogroups, lupus nephritis, and organ damage in four mtDNA haplogroups: African (n = 5), Amerindian (n = 12), Asian (n = 4), and Caucasian (n = 4). Clinical data, SLE Disease Activity Index (SLEDAI-2K), SLICC Damage Index (SDI), and renal biopsy findings were analyzed. Median SLEDAI-2K scores were higher in Amerindian (10) and African (8) patients than in the Caucasian (5.5) and Asian (3) groups, with significant differences between Amerindian vs. Caucasian (p = 0.045) and Amerindian vs. Asian (p = 0.008). Irreversible organ damage (SDI > 1) was more frequent in Amerindian (54%) and African (40%) patients. Lupus nephritis occurred most often and most severely (Class III-IV, CKD) in the Amerindian (85%) and African (80%) groups, while Caucasian and Asian patients more often showed milder, membranous disease without CKD. Conclusion: Although limited by the small sample size, pediatric SLE severity and renal involvement were found to be greater in Amerindian and African mtDNA haplogroups, suggesting that mitochondrial lineage may contribute to ethnic disparities in SLE.
