The role of mediterranean diet and highly processed foods in inflammatory bowel diseases.
The Mediterranean Diet (MD) is considered a balanced dietary model, whereas high consumption of highly processed foods (HPF) may worsen metabolic health. We aimed to compare adherence to the MD and HPF consumption between patients with Crohn's Disease (CD) and Ulcerative Colitis (UC), and to assess their relationship with disease activity. A secondary objective was to explore the association between dietary patterns and metabolic parameters. Adherence to the MD was assessed using the Pyramid Mediterranean Diet Score (PyrMDS), while HPF intake was evaluated with the short questionnaire on highly processed foods (sQ-HPF). 193 consecutive IBD patients were included. CD patients showed significantly lower adherence to the MD compared with UC patients (median PyrMDS 9.3 vs 9.8, p = 0.02), while HPF consumption was similar in both groups (median sQ-HPF 3). In CD patients, a trend toward an inverse correlation was observed between MD adherence and disease activity (p = 0.07). Higher HPF consumption was positively associated with metabolic syndrome (median sQ-HPF 4 vs 3, p = 0.01). CD patients exhibit lower adherence to the MD, particularly during periods of higher disease activity, while greater HPF intake is linked to adverse metabolic profiles. Tailored nutritional counseling is important for supporting healthier dietary patterns in individuals with IBD.
Teduglutide use in haemodialysis-dependent renal failure from secondary hyperoxaluria in short bowel syndrome and type 3 intestinal failure - A case report.
Renal impairment is common in patients receiving long-term home parenteral nutrition (HPN) for short bowel syndrome-associated intestinal failure (SBS-IF). While glucagon-like peptide-2 (GLP-2) analogues like teduglutide are effective in reducing parenteral support (PS) requirements, data on their use in advanced stage chronic kidney disease (CKD) or haemodialysis are scarce. We describe a 66-year-old female with SBS-IF on HPN, complicated by hyperoxaluria and nephrocalcinosis contributing to stage 4 CKD. Initial renally-adjusted teduglutide dosing (0.025 mg/kg/day) enabled enteral autonomy, but was interrupted by intercurrent illness. As a result of progressive renal dysfunction, the patient was commenced on haemodialysis. Teduglutide was introduced post-dialysis at full dose (0.05 mg/kg), resulting in a sustained 36 % reduction in weekly PS requirements without adverse effects, despite recurrent sepsis. This case demonstrates a clinically significant teduglutide response in a haemodialysis-dependent SBS-IF patient, supporting post-dialysis dosing as a feasible strategy in this HPN cohort.
Spontaneous transanal evisceration of the small bowel through perforation of the sigmoid colon: a case report and new surgical approach.
Transanal evisceration of the small intestine is a rare surgical emergency that requires immediate intervention. Since its initial description in 1827, only three cases of evisceration secondary to perforation of the sigmoid colon have been documented. This case report describes the fourth case worldwide. An 85-year-old woman presented with transanal small bowel evisceration after straining during defecation. Examination revealed prolapsed, congested but viable small bowel loops through the anal canal, without uterine or rectal prolapse. Emergency laparotomy identified sigmoid perforation likely due to chronic diverticulitis. Initial surgery involved sigmoid segment resection and blind stapling, followed by ICU care. A second-look laparotomy required 290 cm small bowel resection, sigmoidectomy, colorectal anastomosis, and loop ileostomy. Recovery was prolonged due to age and short bowel syndrome. The patient was discharged to a nursing home 2 months postoperatively. Transanal evisceration involves small bowel herniation via rectal or sigmoid perforation, commonly due to rectal prolapse and sudden increase in intra-abdominal pressure. Surgical management includes emergency laparotomy, bowel viability assessment, resection, and repair. Hartmann's procedure is preferred in contaminated fields. Mortality has decreased significantly with improved techniques. Definitive care should address underlying pathology to prevent recurrence. Immediate surgical intervention is the mainstay of treatment in transanal evisceration of the small bowel. The choice of surgical approach depends on intraoperative findings. A staged surgical approach including second or even third look surgeries may improve small bowel preservation and improve clinical outcome.
Case report: minimally invasive management of two major complications of colonic perforation and pseudoaneurysm formation following nephrostomy tube placement.
We present a case of two major complications following insertion of a nephrostomy managed in a minimally invasive way. Our case is the first in the literature to describe this minimally invasive treatment technique for colon perforation in a completely asymptomatic patient. A 75-year-old female patient with a history of bilateral obstructive uropathy secondary to bilateral radiation-induced ureteric strictures attended for bilateral nephrostomy placement. The patient had a history of endometrial cancer, treated previously by total hysterectomy and bilateral salpingo-oophorectomy with adjuvant chemo-radiotherapy and brachytherapy. Her recovery had been further complicated by the development of radiation cystitis, small bowel resection, and short gut syndrome. The strictures had previously been managed with bilateral double J stents, which have now failed. A left-sided nephrostomy insertion was performed. A CT-scan prompted by a repeat deterioration in renal function revealed a right-sided hydronephrosis and suggested transcolic passage of the previously placed left-sided nephrostomy. She had remained clinically well and apyretic. A right-sided nephrostomy was inserted, and the left re-sited following a colonoscopy-guided nephrostomy removal and clip occlusion of the nephrostomy tract. The patient then developed a pseudoaneurysm, which was managed with embolization. The patient was able to return home with corrected kidney function. The three-monthly checks for changes in nephrostomy catheters did not reveal any complications. The patient therefore presented with two major complications of nephrostomy placement: colonic perforation and pseudoaneurysm, classified as Clavien IIIb and IIIa, respectively. Multidisciplinary management, including urologists, gastroenterologists, general surgeons, and radiologists, enabled a minimally invasive management. Minimally invasive management with endoscopic clip placement appears to be a safe alternative to directed fistula of colonic perforation.
Pilot trial: 8.4% sodium bicarbonate locks for infection prevention in pediatric intestinal failure.
Pediatric patients with intestinal failure (IF) require long-term central vascular catheters (CVCs) for parenteral nutrition, placing them at high risk for catheter-related bloodstream infections (CRBSIs). Sodium bicarbonate (NaHCO₃) locks have shown potential in reducing CRBSIs in adult populations, but data in pediatric patients with IF remain limited. We conducted an open-label prospective cohort study at a tertiary medical center to evaluate the impact of 8.4% NaHCO₃ locks on CRBSI and CVC replacement rates in pediatric patients with IF (ClinicalTrials.gov NCT05927484). The baseline period (heparin locks) was assessed retrospectively, whereas the intervention period (NaHCO₃ locks) was evaluated prospectively. Primary outcome was CRBSI rate; secondary outcomes included CVC replacement because of CRBSI and infection-related hospitalizations. In the per‑protocol analysis, six patients contributed 5733 total catheter days (3244 on heparin, 2489 on NaHCO₃). CRBSI rates decreased from a median of 8.2 to 3.0 per 1000 catheter days after switching to NaHCO₃ locks (P = 0.11). A similar downward trend was observed in the intent‑to‑treat analysis. A statistically significant reduction in CVC replacement because of CRBSI was observed (2.3 vs 0 per 1000 catheter days; P = 0.03). Trends toward reduced hospital and intensive care unit admissions were also noted in the intervention group. No serious adverse events were attributed to NaHCO₃ lock use. NaHCO₃ locks appear to be a safe and promising strategy for reducing CRBSI-related complications in pediatric patients with IF, especially where access to more effective lock solutions is limited. Larger prospective studies are needed to confirm these findings.
