Severe Headache and Acute Blindness: A Case of Pituitary Apoplexy.
Pituitary apoplexy is a rare, life-threatening syndrome that commonly occurs in the adenomatous pituitary gland. It presents with a myriad of severe neuroendocrine and ophthalmological signs and symptoms. Early diagnosis and prompt treatment are vital in the management of pituitary apoplexy. This case report describes a 56-year-old woman who reported to the neurology clinic with 1 week's history of visual loss, and a severe headache, on the background of a 2-year history of pituitary adenoma with poor follow-up. The patient underwent transsphenoidal surgery. She regained vision in the right eye, but no perception of light in the left eye. Patients with headaches and impaired visual field should be screened for pituitary adenoma. Signs and symptoms may be reversible with early diagnosis and prompt treatment of pituitary apoplexy.
Cabergoline-Resistant Recurrent Macroprolactinoma During Pregnancy: A Case Report and Review of the Literature.
We report a rare case of cabergoline-resistant recurrent macroprolactinoma during pregnancy. A 34-year-old woman, 17 weeks pregnant, presented with a four-week history of headache and a one-day history of visual disturbance. The medical history was significant for a macroprolactinoma with pituitary apoplexy, which was treated with an emergent pituitary resection approximately two years and four months before this presentation. At that time, postoperative prolactin levels improved but remained elevated despite gradually escalating cabergoline doses up to 6 mg per week. Our patient met the established criteria for cabergoline resistance, with persistently elevated prolactin levels despite high-dose cabergoline therapy. She was dealing with resistant hyperprolactinemia, causing infertility. The patient decided to conceive, and pre-pregnancy counseling was done by reproductive endocrinology, maternal-fetal medicine, and a genetic counselor. Before conception, magnetic resonance imaging (MRI) of the pituitary showed postoperative changes from transsphenoidal hypophysectomy and expanded partial sella, with no clear tumor, and the prolactin level was 228 ng/mL (non-pregnant normal reference range: 3-30 ng/mL). She had in vitro fertilization done, and cabergoline was discontinued on confirmation of pregnancy. On presentation, MRI of the pituitary revealed a recurrent pituitary macroadenoma (29 × 29 × 27 mm). She developed symptomatic tumor recurrence within three months of stopping cabergoline, and her prolactin level was 1,669.8 ng/mL (normal reference range in pregnancy: 10-209 ng/mL). She was treated with steroids, and cabergoline was restarted. However, the tumor continued to grow, causing symptoms necessitating repeat urgent pituitary resection in the second trimester of pregnancy. She remained on cabergoline through the rest of her pregnancy and delivered a healthy male neonate via cesarean section, done for breech presentation at 39 weeks. The postpartum, fetal, and neonatal stages were all uneventful. This case highlights the complexities of managing dopamine agonist-resistant macroprolactinomas during pregnancy and underscores the importance of individualized, multidisciplinary decision-making to optimize maternal and fetal outcomes.
Pituitary Apoplexy Following Gonadotropin-Releasing Hormone Agonist Administration for Prostate Cancer.
Pituitary apoplexy represents an uncommon endocrine emergency with potentially life-threatening consequences. Gonadotropin-releasing hormone agonist used for prostate cancer has the potential to induce pituitary apoplexy, particularly in the setting of a preexisting pituitary adenoma. A 76-year-old male with prostate cancer initially chose active surveillance; however, prostate-specific antigen (PSA) elevation required hormonal therapy 5 years later. A pancreatic islet tumor had been previously identified; however, its details were unavailable. He was presented to the emergency department 24 h after receiving the first gonadotropin-releasing hormone agonist injection. He showed severe headache, general fatigue, diplopia, and ptosis of the right eye. Brain MRI revealed a right deviated suprasellar pituitary adenoma with hemorrhagic infarction. He was conservatively treated with high-dose steroids; symptoms improved within 1 week. Clinicians should be aware of the association of pituitary apoplexy with the use of gonadotropin-releasing hormone agonist and should ask about the patient's past history of multiple endocrine neoplasia (MEN).
Differences in clinical course between pituitary apoplexy and non-apoplexy patients with nonfunctioning pituitary neuroendocrine tumors.
Although the imaging diagnosis of pituitary apoplexy has been well reported, research on the clinical symptoms of this condition remains scarce. This study aimed to provide an accurate clinical evidence of patients with pituitary apoplexy. This retrospective study used data from 311 patients with nonfunctioning pituitary neuroendocrine tumors treated with initial transsphenoidal surgery. Pituitary apoplexy was defined as the presence of acute headache or visual impairment along with a tumor with pathological findings of necrotic changes and/or diffuse hemorrhage. To alleviate the effect of tumor volume and patient sex, propensity score matching was performed between the pituitary apoplexy (APO) and non-apoplexy (non-APO) groups. A total of 20 patients were matched to each of the two groups, and their data were statistically compared. Compared with the non-APO group, the APO group had higher preoperative body temperature (degrees Celsius) and lower levels of preoperative serum sodium (mEq/L) and prolactin (ng/mL). Receiver operating characteristic curve analysis revealed that the optimal prolactin level that supports the diagnosis of pituitary apoplexy was 7.5 ng/mL (AUC = 0.75250, 50% sensitivity, 100% specificity). The patients in the APO group were further divided into two subgroups, those who were evaluated within 3 days of onset and those who were evaluated after 3 days of onset, and the former subgroup was found to have lower serum PRL and sodium level as well as higher body temperature. Hyperthermia, hyponatremia, and hypoprolactinemia indicate the possibility of pituitary apoplexy.Trial number: E-2020-2022, June 1, 2022, retrospectively registered.
