RAD51 -Related Fanconi Anemia: Expanding the Phenotypic Spectrum and Strong Association With VACTERL.
Fanconi anemia (FA) is a multiorgan disease caused by pathogenic variants in genes involved in the FA/BRCA DNA repair pathway. We report a 10-year-old female who presented with multiple congenital anomalies consistent with VACTERL (Vertebral anomalies, Anal atresia, Cardiac anomalies, Tracheoesophageal fistula, Esophageal/duodenal atresia, and Renal and Limb anomalies) and PHENOS (abnormal Pigmentation, small Head, small Eyes, central Nervous system anomalies, Otological anomalies, short Stature), and later exhibited global developmental delay. She tested positive for a de novo likely pathogenic variant in RAD51 and had inconclusive chromosomal breakage studies. This case provides evidence for a common association between RAD51-related FA and VACTERL and expands its genotypic and phenotypic spectra.
Partial Duodenal Obstructions: A National Surgical Quality Improvement Program Pediatric Analysis of Clinical Factors.
Duodenal atresia is one of the most common causes of intestinal obstruction, typically presenting in the first few days of life. Partial duodenal obstructions (PDO) can allow for the passage of liquids and may not be diagnosed in early infancy until feeding intolerance becomes more apparent. We aimed to identify characteristics of patients who may have PDO and describe their outcomes. Utilizing the National Surgical Quality Improvement Program-Pediatric database, patient characteristics and surgical outcomes were compared between patients who underwent operative repair for duodenal obstructions before 180 d (full duodenal obstruction) and after 180 d (PDO). Of 2213 patients who underwent procedures for duodenal obstructions between 2012 and 2020, 138 were performed after 180 d (PDO). These patients were more likely to be male (66.7 versus 49.7%, P = 0.0001), have a structural pulmonary/airway abnormality (10.9 versus 2%, P < 0.0001), developmental delay (33.3 versus 12.2%, P < 0.0001), and undergo elective surgery (70.3 versus 43.7%, P < 0.0001). Patients who underwent surgery before 180 d (full duodenal obstruction) were more likely to have major cardiac risk factors (32.2 versus 12.3%, P < 0.0001) and birthweight <1500g (7.3 versus 0%, P = 0.0010). Readmission (11.7 versus 3.1%), reoperation (13.3 versus 4.1%), organ space infection (3.9 versus 0.3%), and sepsis (3.9 versus 1.6%) were more likely in patients undergoing procedures after 180 d (PDO) (P < 0.05). PDO appears more likely in patients without the characteristic cardiac risk factor comorbidities. Recognizing these factors will be important for timely surgical intervention to minimize postoperative complications.
Handing off hope: transition of care in pediatric surgery.
The importance of transitioning care from pediatric to adult practitioners is an often overlooked aspect of chronic disease. The benefits of a planned transition in health care are that patients will learn valuable life-long lessons on healthcare maintenance, when and how to seek medical attention for new issues that may arise, and improvements on their overall well-being. The purpose of this SAGES White Paper was to summarize the available knowledge for several pediatric surgical conditions to aid in transition of care for this patient population. The members of the SAGES Pediatric Surgery Committee elected to produce this White Paper. The group chose to focus on several important gastrointestinal diseases that may require lifelong care: tracheoesophageal fistula, duodenal atresia, anorectal malformations, childhood obesity, and gastrointestinal malignancies. For each disease process a summary of long term issues facing these patients, stakeholders involved, and follow up recommendations if required were identified. Each disease process has its own unique set of long-term issues as well as multidisciplinary stakeholders and need for follow-up. However, individualized care is needed based on each patient's unique needs. To facilitate consistent transfer of care standardization is needed for surgical diseases. Key aspects of standardization include identifying a multidisciplinary team, working towards consistent quality improvement, and implementation of policy guided processes with individual treatment plans. Continued work in standardizing transition of care is required for optimal treatment of this complex patient population.
A Rare Dual Atresia Case Report Introducing Gastric Distension-Assisted Esophageal Approximation (GDEA): A Hypothetical Novel Technique for Long-Gap Esophageal Atresia.
The simultaneous occurrence of pure esophageal atresia (EA) and duodenal atresia (DA) is an exceptionally rare congenital anomaly, posing significant diagnostic and surgical challenges. While each condition alone is well-documented, their coexistence is reported in only a small fraction of EA cases, and often necessitates complex, staged interventions. We present a unique case of a neonate with Down Syndrome diagnosed with both pure EA and DA. Intraoperatively, the esophageal segments were found in unexpectedly close proximity, allowing for a tension-free primary anastomosis-an outcome less likely achievable in pure EA which typically has a long-gap. Postoperative imaging confirmed DA, which was subsequently repaired. Despite initial surgical success, the patient experienced a complicated postoperative course and ultimately succumbed to multi-organ failure. This case highlights the diagnostic complexity and high morbidity associated with dual gastrointestinal atresia. Notably, the gastric distension caused by DA appeared to facilitate esophageal approximation, prompting the hypothesis of a novel technique: Gastric Distension-Assisted Esophageal Approximation (GDEA). GDEA proposes pyloric occlusion with monitored physiologic or controlled gastric loading-as a means to approximate esophageal segments in long-gap EA. This approach may offer a new adjunct to existing surgical strategies. The hypothesized GDEA technique, inspired by this case, introduces a potentially transformative concept in the treatment of long-gap EA. We emphasize that GDEA is a speculative hypothesis derived from a single clinical observation and is not supported by experimental or clinical evidence.
Laparoscopic management of duodenal atresia: a decade of single surgeon's perspective.
This study aimed to evaluate the outcomes and learning curve of laparoscopic duodenal atresia repair performed by a single pediatric surgeon over 10 years and compare it with traditional open repair. This retrospective study included 28 neonates who underwent surgical repair for congenital duodenal atresia between 2010 and 2024 at a tertiary center. In total, 20 patients underwent laparoscopic repair by a single surgeon between 2013 and 2024, whereas 8 underwent open repair between 2010 and 2014. Clinical characteristics, operative times, feeding milestones, and complications were compared. To assess the effect of surgical experience, the laparoscopic cohort was divided into early and late phases. The median operative time was longer in the laparoscopic group than in the open group (124 minutes vs. 80 minutes, P < 0.05). However, laparoscopic patients achieved earlier oral intake (5 days vs. 9 days, P = 0.021) and full enteral feeding (8 days vs. 13 days, P = 0.028). No significant differences were observed in complication rates. The operative time improved significantly throughout the laparoscopic series, decreasing from 155 minutes in early cases to 110 minutes in later cases (P = 0.039). No conversions or anastomotic leaks were observed in the late laparoscopic group. Laparoscopic duodenal atresia repair performed by a single surgeon is safe and effective, allowing for faster postoperative recovery than open repair. With accumulated experience, operative efficiency improves significantly, supporting its broader adoption in neonatal surgery.
