Emergency incarcerated "little old lady's hernia": a case report and review of the literature.
An obturator hernia (OH) is an uncommon type of pelvic hernia that typically presents as a bowel obstruction. It occurs when a portion of the intestine, most commonly the ileum, herniates through the obturator foramen in the pelvic wall. A 76-year-old woman with a known history of Tako-Tsubo cardiomyopathy and chronic obstructive pulmonary disease presented with a 2-day history of acute abdominal pain, vomiting, and abdominal bloating. On physical examination, she exhibited diffuse abdominal tenderness and a painful right-sided mass. Abdominal computed tomography (CT) revealed bowel distension with an ileal loop incarcerated through the right obturator foramen. After initial resuscitative measures, the patient underwent a midline laparotomy. Surgical exploration confirmed a strangulated ileal segment that remained viable. Both obturator foramina were repaired with sutures and reinforced using adjacent tissue structures. Due to its non-specific clinical presentation, the diagnosis of OH is frequently delayed or missed. A palpable mass is rarely observed, and one of the more distinctive signs, the Howship-Romberg sign, characterized by pain radiating to the inner thigh and knee, may be mistaken for other conditions such as gonarthrosis or lumbar disc disease. Prompt use of abdominal CT imaging has been shown to improve diagnostic accuracy and can significantly reduce both the morbidity and mortality linked to this type of hernia. OH should be considered in at-risk individuals due to its subtle presentation. Prompt identification using CT scans and timely surgical intervention are crucial for enhancing prognosis and minimizing potential complications.
A Case of Takotsubo Cardiomyopathy Triggered by Human Metapneumovirus.
Takotsubo cardiomyopathy (TC) is a transient, left ventricular dysfunction and apical ballooning that is commonly associated with emotional or physical triggers; however, it is not a known complication of infection with human metapneumovirus (HMPV). This case report describes a 59-year-old woman with a history of anxiety, depression, seizure disorder, fibromyalgia, and chronic obstructive pulmonary disease, who initially presented to the emergency department with pneumonia and sepsis. A sputum polymerase chain reaction (PCR) was positive for HMPV. The following day after admission, the patient developed acute chest pain, increased troponin that peaked at 12.29 ng/L, and ST-segment elevation in L1, V2-V6 on ECG. Subsequent cardiac catheterization revealed normal coronary arteries, and an echocardiogram showed left ventricular systolic dysfunction with apical ballooning and an ejection fraction of 15%, consistent with a diagnosis of TC. To the best of our knowledge, this is the first reported case of TC in an adult secondary to infection with HMPV and thus marks an important contribution to the current knowledge of HMPV complications.

Atypical Takotsubo Post-Natural Delivery: A Guideline-Based Approach for Diagnosing Takotsubo Cardiomyopathy.
Since its first description in 1990, Takotsubo cardiomyopathy (TC) has continued to puzzle physicians and scientists alike as the mechanisms behind the characteristic unique left ventricular dysfunction that marks the condition and its relation to the intense emotional and physiological stressors that usually precede it have remained not fully understood. Since then, several different variants of the condition have been described that do not conform to the conventional narrative of a post-menopausal disease that only affects women of a certain age and is triggered by a preceding event. Of these variants, atypical Takotsubo, which does not assume the typical characteristic apical ballooning that marks the condition has been linked with a younger age group and a more neurological trigger. A strong guideline-based clinical suspicion and an astute knowledge of its clinical, laboratory, and different morphological presentations are often needed to differentiate the case from acute coronary syndromes, which it so closely resembles. This case illustrates an atypical presentation of atypical TC in an otherwise healthy woman and how a guideline-based approach can be utilized to identify and manage the condition while discussing the most recent theories addressing TC.
Tako-Tsubo Syndrome Triggered by a Fibroscopy: Case Report.
Tako-Tsubo cardiomyopathy, also called stress cardiopathy, is a rare syndrome characterized by transient regional systolic dysfunction. It can mimic myocardial infarction but the absence of coronary obstruction allows to redress the diagnosis. Its pathogenesis is not well understood. However, the role of physical or emotional stress has often been associated with this pathology. Here we report, a rare case of a 63-year-old female, with no cardiac risk factors, who presented Tako-Tsubo syndrome after a fibroscopy. This case aims to show that Tako-Tsubo syndrome should be suspected in patients, especially women, with no cardiac risk factors, who present acute chest pain in the context of physical or emotional stress, after excluding differential diagnoses.
