O envenenamento por escorpião é uma intoxicação rara causada por uma picada de escorpião que normalmente se manifesta com dor localizada, edema, eritema e parestesias no local da picada e, quando grave, progride para produzir sintomas sistêmicos de gravidade variável que incluem dificuldades respiratórias, pressão arterial sistêmica anormal, arritmia cardíaca e uma combinação de sintomas parassimpáticos (ou seja, salivação e lacrimejamento excessivos, diaforese, miose, micção frequente, diarréia, vómitos, priapismo) e manifestações simpáticas (por exemplo, hipertermia, hiperglicemia, midríase). Manifestações neurológicas também podem estar associadas, como movimentos oculares anormais, visão turva, agitação e inquietação, além de fasciculações e espasmos musculares. Os sinais e sintomas são altamente variáveis ​​e, na maioria dos casos graves, podem causar choque cardiogênico e edema pulmonar.

Immunogenicity of two representative American consensus scorpion neurotoxins from the genera Tityus and Centruroides.

Two consensus scorpion toxins derived from venoms of the genera Centruroides (NATx, North America), and Tityus (SATx, South America) were rationally designed and recombinantly expressed in Escherichia coli Origami. Both recombinant proteins were produced as inclusion bodies and subsequently purified and refolded in vitro to obtain biologically active isoforms. The expression yields were approximately 1 mg/L for rNATx and 0.5 mg/L for rSATx. Structural characterization by circular dichroism spectroscopy revealed that rNATx and rSATx exhibited folding patterns typical of scorpion β-toxins. The purified recombinant toxins were used as immunogens to raise polyclonal antibodies in New Zealand rabbits. The resulting antisera were evaluated for their capacity to neutralize isolated toxins and whole scorpion venoms. rNATx elicited a stronger immune response and showed superior immunogenicity compared to rSATx. Notably, 1-1.5 mg of anti-rNATx antibodies were sufficient to neutralize up to 3LD50 of venoms from the most medically relevant Centruroides species in México. On the other hand, approximately 20 mg of anti-rSATx antibodies were required to neutralize only 2LD50 of Tityus venoms. Additionally, T-cell subsets quantified by flow cytometry showed that rNATx is more immunogenic and probably confers improved antivenom efficacy, highlighting its potential application in the development of broad-spectrum antivenoms against scorpion envenomation.

Scorpionism at the human-environment interface: an eco-epidemiological synthesis and conceptual framework.

Scorpion envenomation is a major but persistently under-recognized public health problem in tropical and subtropical regions. Its occurrence arises from complex interactions among climatic, ecological, socioeconomic, and health-system drivers. Despite sustained high incidence and expanding risk associated with climate and land-use change, these relationships remain insufficiently synthesized. Addressing this gap requires an integrative framework capable of capturing the multi-level determinants shaping scorpionism risk and outcomes. Drawing on Human Ecology Theory and socio-ecological systems thinking, we developed an integrated conceptual model to organize determinants of scorpion envenomation. The framework synthesizes evidence from environmental science, climate impacts, scorpion biology, venom characteristics, epidemiology, and clinical and public health interventions. More than thirty determinants were categorized into six components: (1) physical-climatic variables, (2) scorpion behavior and ecology, (3) venom composition and toxicity, (4) human exposure and envenomation risk, (5) health impacts and public health response, and (6) socioeconomic factors. The model is structured in two stages: a risk-and-vulnerability stage and a scorpionism-occurrence stage. The resulting conceptual model provides a unified systems-based framework that integrates environmental, ecological, behavioral, socioeconomic, and health-system determinants of scorpion envenomation. It clarifies pathways linking upstream drivers to exposure, clinical outcomes, and public health responses, enabling interpretation of regional heterogeneity in incidence. The framework also identifies priority leverage points for intervention and offers a structured basis for empirical research, risk mapping, and early-warning development. This work highlights the importance of adopting a socio-ecological systems perspective to better understand and address scorpionism. By integrating diverse determinants into a coherent analytical framework, the model serves as a hypothesis-generating and interpretive tool rather than a predictive model. The approach supports the development of targeted prevention and control strategies and reinforces the need to recognize scorpionism within the broader agenda of neglected tropical health conditions, particularly given its substantial burden and strong socio-ecological determinants.

Predictive value of admission blood glucose for early severity stratification in pediatric scorpion envenomation.

Scorpion stings are an increasing public health concern in Brazil, where children are at a heightened risk of severe outcomes. Simple biomarkers, such as admission blood glucose may improve early risk stratification. To assess the association between admission blood glucose and clinical severity in children envenomated by Tityus serrulatus. This prospective observational cohort study was conducted at João XXIII Hospital, Minas Gerais State Hospital Foundation (FHEMIG), Belo Horizonte, Brazil, from September 2023 to March 2024. We included patients aged 0 to 17 years with confirmed scorpion envenomation. Clinical severity was classified as mild (no antivenom) or moderate/severe (antivenom administered). Blood glucose was measured at admission and two hours thereafter. Predictive performance was assessed using receiver operating characteristic (ROC) analysis. Sixty-seven children were included; 13.4% progressed to moderate/severe envenomation. Admission glucose ≥ 142 mg/dL showed 100% specificity and a 100% positive predictive value for severe cases (ROC AUC 0.979; 95% CI, 0.937-1.000; p < 0.001). A 105 mg/dL threshold maximized sensitivity (100%) at the expense of lower specificity (79.3%). To our knowledge, this is the first study to systematically evaluate admission glucose in pediatric T. serrulatus envenomation and to propose cutoffs for risk stratification. Admission blood glucose is an accessible and accurate biomarker for early triage of severity in pediatric scorpion envenomation. A 105 mg/dL threshold favors maximal sensitivity for screening, whereas 142 mg/dL ensures high specificity for severe cases. Although promising, these cutoffs should be treated as clinical hypotheses and require multicenter external validation before widespread adoption.

The sting that clots: The Factor VII and Factor X activating procoagulant effects of Androctonus scorpion venoms are potentiated by Factor Va as a cofactor.

While scorpion venoms are well-characterised as being potently neurotoxic, their effects upon blood coagulation are understudied. Here, we report novel procoagulant toxicological functions for Androctonus amoreuxi, A. australis, A. bicolor and A. crassicauda venoms. Factor activation tests with A. amoreuxi venom revealed cofactor-dependent activation of Factor VII (FVII) and Factor X (FX) to be the primary zymogen targets, with FX the more potently activated. Activation of both factors was demonstrated to be dependent upon the proteinaceous cofactor Factor Va (FVa) and the biochemical cofactors calcium and phospholipid. It was also shown that venom was able to convert Factor V (FV) into a form of FVa that was equipotent to endogenous FVa, suggestive of the venom cleaving FV at the same activation site as thrombin. Intriguingly, low level FXII activation only proceeded with the venom-activated form of FVa and was not reliant upon calcium or phospholipid. Antivenom produced with Androctonus species included in the immunising mixture failed to neutralise procoagulation. However, neutralisation of procoagulant activities was achieved by the metalloprotease inhibiting drugs marimastat and prinomastat, thereby not only revealing the enzyme type responsible for the effects upon blood coagulation, but also suggesting therapeutic options. These results indicate that venom-induced coagulopathy resulting from scorpion envenomation may require greater consideration in pathophysiological profiling of envenomed patients. The implications extend beyond the field of toxinology, building a foundation for evolutionary studies into the selection pressures that have resulted in some species having potent effects upon blood biochemistry, whether as a weapon for predation or defence.

Scorpion Stings and Antivenom Coverage in the Brazilian Amazon.

IntroductionScorpion envenomation in the Amazon causes variable clinical manifestations. Data on these incidents, as well as on the demand, distribution, and use of antivenoms for treatment, are still limited in the literature. This study describes the epidemiology of scorpion envenomation and antivenom coverage in the Legal Amazon area from 2010 to 2020.MethodsData on scorpion sting victims, such as sex, age, circumstances of the incident (including location and time), and severity of the case, were obtained from an open health data system called the Notifiable Diseases Information System. The data on antivenom use to treat scorpion stings in each Amazonian state was estimated from consolidated data from the Strategic Inputs Information System.ResultsFrom 2010 to 2020, 62,388 people were envenomed by scorpions in the Legal Amazon area, resulting in 118 deaths (a fatality rate of 0.19%). The regular distribution of antivenom by the Brazilian Ministry of Health resulted in the allocation of 109,276 vials to the region. However, only 62.2% (of the regional antivenom stockpile 67,994 vials) was used. The state of Pará recorded the highest number of deaths from scorpion stings (33% of total deaths) and the highest use of antivenom per year (∼2705 vials over the 10-y period). Tocantins presented the highest incidence rate, with 119 cases per 100,000 inhabitants.ConclusionAlthough the availability of antivenom in the Legal Amazon between 2010 and 2020 exceeded actual demand, this was not enough to prevent the deaths of 118 victims of scorpion stings. These fatalities primarily affected demographic groups without referral units in their regions and point to critical failures in the spatial distribution of emergency medical services. Rural communities at risk of animal envenomation in the Amazon need timely treatment, a crucial condition for saving lives.
