Secondary trigeminal autonomic cephalgia with cavernous sinus involvement: a diagnostic challenge.
Chronic, side-locked headache refractory to standard treatment warrants investigation for underlying inflammatory, infectious or infiltrative aetiologies. This case highlights a rare presentation of a patient with a chronic history of hemicranial headache who had presented with recent worsening and unresponsiveness to conventional treatment. IgG4-related disease with hypertrophic pachymeningitis (HPM), primarily affects the cavernous sinus and orbital apex, can lead to multiple cranial nerve palsies and mimics secondary trigeminal autonomic cephalalgia. The diagnostic challenge was compounded by concomitant extrapulmonary tuberculosis, a frequent mimic in endemic regions, obscuring the clinical picture. This case underscores the importance of integrating advanced neuroimaging and serological markers for accurate diagnosis. Given the absence of definitive treatment guidelines and high chances of relapses for IgG4-related HPM, it highlights the need for personalised therapeutic strategies and long-term follow-up. This report adds to the limited literature, emphasising the need for heightened clinical suspicion in atypical headache syndromes.
Onset of secondary SUNCT syndrome with concomitant occipital neuralgia after dorsal medullary infarction.
Short-lasting Unilateral Neuralgiform headache attacks with Conjunctival injection and Tearing (SUNCT) syndrome is a rare trigeminal autonomic cephalalgia characterised by brief, unilateral, severe pain episodes and autonomic symptoms. Secondary SUNCT has been associated with brainstem lesions, implicating the trigeminocervical complex in its pathogenesis. We describe a man in his 40s with secondary SUNCT and occipital neuralgia following a right dorsolateral medullary infarction. He presented with right facial numbness and burning pain lasting about a minute, occurring over four times daily, triggered by touch. Examination revealed dacryorrhoea, rhinorrhoea and ipsilateral facial oedema. Brain MRI confirmed subacute medullary infarction, establishing secondary SUNCT. Subsequently, he developed intermittent electrical pain in the right posterior head, neck, shoulder and arm, diagnosed as occipital neuralgia, which responded to gabapentin. This case highlights the importance of high-resolution neuroimaging and follow-up in atypical headache presentations to identify structural lesions and optimise management.
Risk of Attempted and Completed Suicide in Persons Diagnosed With Headache.
Although past research suggests an association between migraine and attempted suicide, there is limited research regarding risk of attempted and completed suicide across headache disorders. To examine the risk of attempted and completed suicide associated with diagnosis of migraine, tension-type headache, posttraumatic headache, and trigeminal autonomic cephalalgia (TAC). This was a population-based cohort study of Danish citizens from 1995 to 2020. The setting was in Denmark, with a population of 5.6 million people. Persons 15 years and older who were diagnosed with headache were matched by sex and birth year to persons without headache diagnosis with a ratio of 5:1. Data analysis was conducted from May 2023 to May 2024. First-time headache diagnoses identified from inpatient hospitalizations, emergency department visits, and outpatient specialty clinic visits using International Statistical Classification of Diseases and Related Health Problems, Tenth Revision (ICD-10) codes. Diagnostic codes from the ICD-10 were used to identify attempted suicide from the Danish National Patient Registry and the Danish Psychiatric Central Research Register and completed suicides from the Danish Register of Causes of Death. Absolute risks (ARs) and risk differences (RDs) for attempted and completed suicide were calculated using the cumulative incidence function. Hazard ratios (HRs) for attempted and completed suicide associated with headache diagnosis were computed adjusting for age, sex, year, education, income, baseline comorbidities, and accounting for competing risk of death. In total, 119 486 persons (83 046 female [69.5%]) diagnosed with headache were identified and matched with 597 430 persons (415 230 female [69.5%]) drawn from the general population. Participants' median (IQR) age was 40.1 (29.1-51.6) years. The 15-year AR of attempted suicide among persons diagnosed with headache was 0.78% (95% CI, 0.72%-0.85%) vs 0.33% (95% CI, 0.31%-0.35%) in the comparison cohort (RD, 0.45%; 95% CI, 0.39%-0.53%). The 15-year AR of completed suicide among persons diagnosed with headache was 0.21% (95% CI, 0.17%-0.24%) vs 0.15% (95% CI, 0.13%-0.16%) in the comparison cohort (RD, 0.06%; 95% CI, 0.02%-0.10%). The hazards of attempted suicide (HR, 2.04; 95% CI, 1.84-2.27) and completed suicide (HR, 1.40; 95% CI, 1.17-1.68) were elevated among persons with headache vs comparison cohort members. Findings were consistent across headache types, with stronger associations for TACs and posttraumatic headache. Results of this cohort study revealing the robust and persistent association of headache diagnoses with attempted and completed suicide suggest that behavioral health evaluation and treatment may be important for these patients.
Understanding the Diagnosis of Primary Headaches in Outpatients Referred to Japanese Headache Specialists.
Background The timely and accurate diagnosis of primary headaches is crucial for enhancing patients' quality of life. However, primary headaches have been reported to be misdiagnosed and underdiagnosed worldwide, especially by nonspecialists. In Japan, despite the high prevalence of primary headaches, research on the extent of headache misdiagnosis and underdiagnosis remains limited. Therefore, we aimed to assess the rates of concordant primary headache diagnoses between previous physicians and headache specialists among outpatients who were referred to the headache center in Japan. Methodology In this retrospective, observational study, we collected data at the renowned Headache Center of Tominaga Hospital in Japan from August 2024 through April 2025. The target population was headache patients aged six years or older who were referred from or previously visited another medical institution. We classified headache diagnoses into the following five categories: migraine; tension-type headache; trigeminal autonomic cephalalgia; other primary headaches; and other headaches, including secondary headaches and painful cranial neuropathies. We calculated the accuracy rate of previous physicians' diagnoses by comparing theirs with those made by board-certified headache specialists at Tominaga Hospital. Results We analyzed 147 patients (mean age = 41.5 years; SD = 18.0 years) who were referred to Tominaga Hospital for headaches. Of these patients, 47 (32.0%) were male, and 100 (68.0%) were female. Overall, 69 (47.3%) patients were undiagnosed by previous physicians. The overall accuracy rate of previous physicians' diagnoses was 43.8% (95% confidence interval (CI) = 36.0-51.9%). The sensitivity and specificity of migraine were 52.6% (95% CI = 43.5-61.6%) and 78.1% (95% CI = 61.2-89.0%), respectively. Conclusions Approximately half of the patients had not received any specific diagnosis, and fewer than half had diagnoses consistent between headache specialists and previous physicians. These findings highlight the challenges of headache diagnosis and underscore the need for further investigation to enhance diagnostic accuracy.
A case report: Infrazygomatic approach neurocryoablation of the sphenopalatine ganglion under cone beam computer tomography guidance in treating refractory headaches.
The sphenopalatine ganglion (SPG) drives the cranial autonomic symptoms associated with trigeminal autonomic cephalalgias, migraine, and trigeminal neuralgia. Infrazygomatic approach SPG block serves as the gold standard for both prognostic and therapeutic intervention for management of refractory headache with autonomic symptoms. However, this intervention often provides only short-term relief. Identification of novel interventions that provide more durable relief is imperative. An 84-year-old female with chronic headache featuring autonomic cranial symptoms that failed both medication and conservative intervention management. Patient responded well to infrazygomatic approach SPG block using combined cone beam CT and fluoroscopy guidance, however, experienced fading therapeutic response over the years. Subsequently, patient underwent neurocryoablation of the SPG, from which, satisfactory and sustained improvement of her headache pain was achieved at six month follow up. This is the first case demonstrating the clinical feasibility and therapeutic outcomes of SPG neurocryoablation in treating refractory headaches.
