Pearls & Oy-sters: Reversible Leukoencephalopathy and Parkinsonism Due to CNS Involvement in Cryoglobulinemia.

We present a case of reversible leukoencephalopathy and parkinsonism associated with type III mixed cryoglobulinemic vasculitis in a patient who initially presented with systemic vasculitic symptoms such as rash and arthralgias. Neither hepatitis C nor monoclonal gammopathy-typical triggers of cryoglobulinemia-was identified. The neurologic syndrome evolved with tremor, rigidity, significant cognitive decline, and extensive white matter abnormalities on MRI, mimicking primary neurodegenerative conditions such as multiple system atrophy. Diagnostic workup ruled out infectious, metabolic, autoimmune, and drug-related etiologies. After immunosuppressive therapy with methotrexate and steroids, the patient experienced marked clinical and radiologic improvement, paralleled by a decline in serum neurofilament levels. This case highlights the importance of recognizing inflammatory CNS involvement as a potentially reversible cause of rapidly progressive parkinsonism and cognitive impairment. Prompt identification and timely immunosuppressive treatment may reverse symptoms and prevent permanent neurologic damage.

Resolution of Hepatitis Delta Virus-Related Cryoglobulinemic Vasculitis With the Use of Bulevirtide.

The global prevalence of hepatitis B virus (HBV) infection is around 257 million people, and about 5% are coinfected with hepatitis delta virus (HDV). HDV infection can progress from acute fulminant hepatitis to chronic hepatitis, cirrhosis, or hepatocellular carcinoma. About 20% of HBV patients develop extrahepatic manifestations; among them, cryoglobulinemic vasculitis (CV) occurs in less than 5% of cases and can lead to heterogeneous clinical manifestations. We present the case of a woman with HBV-HDV coinfection who was referred to our clinic with suspected virus-related CV. One-year treatment with bulevirtide led to complete and durable resolution of CV.

Short-Term Clinical and Immunologic Outcomes Conditional on SVR12 in Hepatitis C Virus-Associated Cryoglobulinemic Vasculitis Treated With Direct-Acting Antivirals: A Systematic Review and Meta-Analysis.

In contemporary clinical practice, direct-acting antiviral therapy (DAA) produces high rates of sustained virologic response (SVR) among patients with hepatitis C virus infection; however, short-term clinical and immunologic outcomes following viral eradication in HCV-associated cryoglobulinemic vasculitis remain incompletely characterized, particularly among patients who have already achieved SVR12. A systematic review and meta-analysis was performed to evaluate short-term outcomes conditional on achieving SVR12 among patients with symptomatic HCV-associated cryoglobulinemic vasculitis treated with interferon-free DAA therapy. EMBASE and PubMed were searched from inception through January 2026, and two reviewers independently screened studies and extracted data. Pooled proportions of complete clinical response, partial clinical response, and complete immunologic response at the time of SVR12 were calculated using random-effects models. Eighteen observational studies were included. Among patients who achieved SVR12, the pooled proportion of complete clinical response was 64.8% (95% CI: 49.6%-79.3%), while partial clinical response occurred in 23.8% (95% CI: 18.3%-30.4%). Complete immunologic response was observed in 43.9% of patients (95% CI: 36.6%-51.5%). Substantial heterogeneity was observed for clinical outcomes reflecting differences across observational cohorts, whereas heterogeneity was lower for immunologic response. In conclusion, among patients with HCV-associated cryoglobulinemic vasculitis who achieved SVR12 with DAA therapy, short-term clinical improvement was common, although complete immunologic normalization was less frequent. These findings provide clinically relevant prognostic information regarding early expectations following viral eradication, while highlighting that longer-term durability of response requires ongoing evaluation.

Lingual Raynaud's Phenomenon: A Case Series and Literature Review.

Background: Raynaud's phenomenon of the tongue is a rare manifestation that may be associated with systemic diseases. The clinical manifestations, etiologies and management of this condition are poorly described. Methods: We report 10 cases of lingual Raynaud's phenomenon (LRP) and 26 cases from a structured literature review. Results: In 38.8% of cases, the LRP occurred in the context of a previously diagnosed systemic sclerosis; 16.6% followed radiotherapy for head and neck cancer; and 27.8% of patients presented with an idiopathic-like form. The manifestations classically included a syncopal phase (91.7%) associated with hypoesthesia (88.9%) and possible dysarthria (52.8%). Atypical presentations with a primary cyanotic phase were also observed, particularly in the context of vasculitis, notably cryoglobulinemic vasculitis (four patients). Active smoking was a significant triggering factor in idiopathic forms (60%). Across all patients-both primary and secondary forms-the most common triggering factor was cold exposure (75%). Vasodilator use showed good efficacy and should be considered for all highly symptomatic patients. Conclusions: In summary, LRP is more frequently associated with systemic sclerosis, manifesting as blanching of the tongue associated with hypoesthesia and dysarthria in more than half of cases. Vasodilators may reduce symptoms. Larger studies are needed to confirm these findings.

Cryoglobulinemia, monoclonal and mixed cryoglobulinemia syndromes, cryoglobulinemic vasculitis: a proposal for comprehensive nomenclature and definition.

Cryoglobulinemia indicates the reversible, cold-dependent precipitation of immunoglobulins (Ig), which may be monoclonal (MoC) or mixed IgG-IgM (MC). Although this in vitro phenomenon is relatively frequent, only a minority of cases develop clinically relevant manifestations, often falling within lymphoproliferative, autoimmune, or infectious disease domains. This complexity highlights the need for clearer nomenclature and definition systems. The term "cryoglobulinemia" is often used interchangeably to describe either simple laboratory finding, asymptomatic cryo-Ig precipitation (MoC or MC) or corresponding clinical syndromes (MoCs or MCs). This overlap creates ambiguity, hinders expert communication, and complicates the comparison of clinical studies. A standardized nomenclature is therefore essential to define disease subsets, establish boundaries with related conditions, and enable reliable data collection and multicenter analyses. When cryoglobulins are detected, the cryoprecipitate must be isolated and defined as MoC or MC, followed by a systematic clinical and laboratory assessment to identify potential underlying disorders. Immunoserological, microbiological, and pathological investigations are required to classify both MoC and MC as asymptomatic or symptomatic, and as essential or secondary. Asymptomatic individuals must be monitored over time for potential progression to overt clinical syndromes; similarly, those with essential forms require surveillance for the emergence of systemic diseases. Possible overlap with autoimmune conditions, particularly Sjögren's disease, should always be considered. Despite some shared features, MoCs and MCs represent distinct clinical entities. Accurate diagnosis relies primarily on cryoprecipitate analysis. MoCs are usually associated with thrombotic, non-inflammatory microangiopathy and lymphoproliferative disorders, whereas MCs are characterized by immune-complex-mediated leukocytoclastic vasculitis, complement consumption, and frequent association with viral infections, especially HCV and HBV. The introduction of new-generation antivirals in the last decade has markedly reduced the prevalence of virus-related MCs in more developed countries. A harmonized nomenclature and standardized definitions for various cryoglobulinemia subsets, supported by careful clinical-immunological and pathological characterization, are essential to improve diagnostic accuracy, ensure interobserver consistency, and enable the development of evidence-based, mechanism-driven therapies for major cryoglobulinemic syndromes.
