Preclinical efficacy and safety evaluation of human embryonic stem cell-derived functional hepatocytes for a clinical trial in acute liver failure.
Acute liver failure (ALF) is a life-threatening syndrome characterized by rapid deterioration of liver function, resulting in high mortality and posing a substantial global health burden. Human embryonic stem cells (hESCs) possess unlimited self-renewal capacity and pluripotent differentiation potential. Transplantation of hESC-derived hepatocyte-like cells (HPLCs) represents a promising therapeutic strategy for ALF. A good manufacturing practice (GMP)-compliant differentiation process was developed to generate HPLCs from hESCs, and their biological characteristics and functional properties were systematically evaluated. A comprehensive series of preclinical safety and efficacy studies was performed, including dose-escalation experiments, biodistribution analysis, comparative evaluation of administration routes, and carcinogenicity testing. The therapeutic efficacy and safety of HPLCs were assessed in a fatal rat model of D-galactosamine (D-gal) and lipopolysaccharide (LPS)-induced ALF. In addition, the HPLCs underwent quality evaluation by the National Institutes for Food and Drug Control (NIFDC), and an independent safety assessment was conducted. A high-efficiency system was established for the generation of qualified, clinical-grade HPLCs from hESCs under GMP-compliant conditions. The HPLCs exhibited multiple mature hepatocyte functions, including carbohydrate and lipid metabolism, hepatic synthetic and storage functions, inducible cytochrome P450 activity, albumin secretion, and urea production. The HPLCs met the certification standards of the NIFDC of China. Transplantation of HPLCs significantly improved survival in ALF rats, with survival rates of 72.4% following tail vein injection and 66.7% following intraperitoneal injection, compared with 6.67% in the control group. HPLC transplantation also promoted recovery of liver function, as reflected by improvements in biochemical and coagulation parameters. Preclinical safety evaluations confirmed the biosafety of HPLCs, with no evidence of acute toxicity or tumorigenicity. A Phase I clinical trial (ChiCTR2100052988) for the treatment of ALF and acute-on-chronic liver failure (ACLF) has been approved by the National Health Commission of the People's Republic of China (Filing Number: MR-43-21-014643) and has been initiated. A novel multistage, GMP-compliant process was developed for the effective and reproducible differentiation of hESCs into hepatocytes. The resulting HPLCs demonstrated robust hepatocyte functions, therapeutic efficacy in an ALF animal model, and favorable biosafety profiles. These findings support the clinical translation of HPLCs, with an ongoing Phase I clinical trial designed to evaluate their safety and feasibility in patients with ALF and ACLF.
Comparative pathophysiological modeling: the advantage of "two-hit" over "one-hit" in acute liver failure studies.
Acute liver failure (ALF), a life-threatening condition marked by rapid hepatocyte death and systemic inflammation, poses significant clinical challenges due to its high mortality. The crosstalk between necrotic hepatocytes and infiltrating immune cells is hypothesized to drive disease progression. To investigate this interplay, we developed a sequential "two-hit" murine model using concanavalin A (Con A) challenges and compared its pathophysiological outcomes with the conventional single-dose "one-hit" approach. The results demonstrated that the "two-hit" model induced more severe hepatic coagulation dysfunction, extensive hepatocellular necrosis, destruction of liver lobular architecture, and inflammatory responses. Furthermore, serum levels of alanine transaminase (ALT) and aspartate transaminase (AST) were markedly elevated in the "two-hit" group. Inflammatory cytokines including interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were also significantly elevated. Moreover, substantial recruitment of macrophages was observed in the "two-hit" model, indicating that these cells are key determinants in the interaction with dying hepatocytes for the progression of ALF. Complementary ex vivo experiments revealed that Raw264.7 cells subjected to a "two-hit" stimulation with Con A and hepatocyte debris produced a robust inflammatory response through the classical NF-κB signaling pathway.
Identification and validation of key PANoptosis-related genes via integrative machine learning and single-cell sequencing in AILI.
Acetaminophen (APAP) overdose is a leading cause of drug-induced liver injury and acute liver failure. PANoptosis, a recently defined form of programmed cell death, is closely linked to immune regulation; however, its role in APAP-induced liver injury (AILI) remains unclear. Here, we aimed to identify PANoptosis-related biomarkers and elucidate their functions in AILI. By integrating bulk RNA-seq, weighted gene co-expression network analysis, machine learning, and single-cell RNA-seq, we identified Cdkn1a and Pdk1 as key PANoptosis-related genes with high diagnostic potential. Immune infiltration analyses revealed significant associations between these genes and multiple immune cell populations. Single-cell analysis demonstrated cell-type-specific expression patterns and enhanced signaling between hepatocytes and macrophages, as well as between T cells and neutrophils. Experimental validation confirmed that Cdkn1a and Pdk1 correlated with liver injury severity, and in vivo knockdown of Pdk1 exacerbated AILI by promoting PANoptosis. Collectively, our findings identify Cdkn1a and Pdk1 as promising biomarkers in AILI.
Clinical and Epidemiological Profile of Liver Dysfunction in Dengue Fever - A Cross-Sectional Study.
Dengue fever is a major public health concern in tropical and subtropical regions, including Kerala, India. Hepatic involvement is a common but variable feature of the disease, ranging from asymptomatic transaminitis to acute liver failure. We conducted a single-center, observational, cross-sectional study over a 6-month period. Adult patients (≥18 years) with laboratory-confirmed dengue fever (NS1 antigen or IgM ELISA positive) were enrolled consecutively. Detailed clinical and demographic data were collected. Liver function tests, including aspartate aminotransferase (AST) and alanine aminotransferase (ALT), were monitored. Hepatic dysfunction was defined as an elevation of AST or ALT >2.5 times the upper limit of normal (>100 IU/L). A total of 216 patients were included (mean age 42.5 ± 16.1 years; 56.5% male). The overall incidence of hepatic dysfunction was 64.4% (139/216). A characteristic pattern of AST elevation greater than ALT was observed, with peak AST levels noted on day 4 of admission. The most common symptoms were fever (100%), myalgia (76.4%), and fatigue (69.9%). Patients with hepatic dysfunction had a significantly longer duration of hospital stay compared to those without (median 5 days vs. 3 days; P = 0.005). Furthermore, the need for platelet transfusion was significantly higher in the group with hepatic involvement (12.2% vs. 1.3%; P = 0.005). No mortalities were recorded in the cohort. Hepatic dysfunction is a frequent complication among hospitalized dengue patients in South Kerala and is associated with a more protracted clinical course, including longer hospital stays and a greater need for platelet transfusions.
Tsutsugamushi Hepatopathy: A Scoping Review.
Scrub typhus, caused by Orientia tsutsugamushi, is a significant public health concern, particularly in endemic regions of Asia. This mite-borne disease, transmitted by chiggers, presents with varied clinical manifestations, ranging from mild symptoms to severe complications, including multi-organ failure. Hepatic dysfunction is a common and often underappreciated clinical feature in scrub typhus patients, with recent studies highlighting its frequent occurrence. This dysfunction is characterized by elevated liver enzymes, jaundice, and, in severe cases, fulminant hepatic failure, often mimicking acute viral hepatitis. The pathophysiology of hepatic dysfunction is multifactorial, involving direct bacterial invasion, immune-mediated damage, and systemic inflammatory responses. Key mechanisms include focal or disseminated vasculitis and perivasculitis, endothelial dysfunction, and potential direct cytopathic effects on hepatocytes. Dysregulated cellular immunity, marked by a strong Th1 response, also contributes to tissue damage. Additionally, sepsis can lead to ischemic hepatitis and further hepatic dysfunction. A scoping review of 63 studies, predominantly from Asia, revealed varying incidences of hepatomegaly (2.5%-98.5%), jaundice (1.4%-36.5%), and abnormal transaminase levels (27.5%-100%). Hepatic dysfunction is typically hepatocellular, with AST often more elevated than ALT. While acute liver failure is rare, it is reported mostly in pediatric patients. Hepatic dysfunction correlates with overall disease severity and is associated with increased risks of delayed defervescence, prolonged hospitalization, other organ failures, and mortality in both adult and pediatric populations. Given its impact on patient outcomes, early recognition of hepatic involvement is crucial for effective management.
