Historical evolution of microvascular decompression after Jannetta's establishment: Anatomical maps and physiological compasses-a narrative review.
Building on the pioneering observations of Dandy and Gardner and on Jannetta's establishment of microvascular decompression (MVD) through microsurgical demonstration of vascular compression, MVD has continuously evolved into a safe and durable treatment for neurovascular compression syndromes. Despite advances in radiosurgery and pharmacotherapy, MVD remains widely used as a first-line surgical option for appropriately selected patients with trigeminal neuralgia, hemifacial spasm, and glossopharyngeal neuralgia. This selective narrative historical review traces MVD's modern evolution after its establishment by Jannetta, organized into four pillars: surgical anatomy, optimization of approaches, decompression strategies, and operative support. Detailed microsurgical anatomy, including the "Rule of Three" framework for the cerebellopontine angle, has helped guide tailored, minimally invasive approaches (e.g., the lateral supracerebellar-infratentorial route for trigeminal neuralgia and infrafloccular exposure for hemifacial spasm). Decompression strategies have extended from prosthetic interposition toward noncompressive transposition and biologically harmonious fixation. Advances in visualization (endoscopic and exoscopic systems), simulation, and neuronavigation have extended the original visualization ethos and enhanced surgical education. Intraoperative monitoring, including brainstem auditory evoked potentials and lateral spread responses, has improved both complication avoidance and intraoperative confidence. Together, these anatomical "maps" and physiological "compasses" help define contemporary MVD practice aimed at safe and durable outcomes across diverse patient anatomies.
Treatment of Glossopharyngeal Neuralgia: A Systematic Review.
Glossopharyngeal neuralgia (GPN) is a rare neuropathy characterized by paroxysmal, unilateral, and severe pain episodes affecting the throat, tonsils, base of the tongue, and pharynx. Its management remains challenging due to the absence of standardized treatment protocols and the limited availability of high-quality comparative studies. To review and compare the effectiveness and safety of available treatments for GPN. A systematic review was conducted, analyzing studies published in the last decade that evaluated GPN treatments. Searches were performed in PubMed, SCIELO, and the Regional Portal of BVS. Studies were assessed for quality, risk of bias, and key outcomes, including pain relief and adverse effects. Twenty-six studies, comprising 973 patients, were included. Seven treatment modalities were analyzed. Microvascular decompression (MVD) demonstrated the highest efficacy and durability, with an immediate pain relief rate exceeding 86.7% and exhibiting the lowest recurrence rates. Alternative treatments, including gamma knife radiosurgery, radiofrequency thermocoagulation, and pulsed radiofrequency, showed initial pain relief but had higher recurrence rates over time. The posterior inferior cerebellar artery was the most frequently involved vessel in cases with vascular compression. MVD is the preferred first-line surgical treatment when pharmacological therapy fails, vascular compression is present, and the patient is a suitable candidate for surgery. In cases where MVD is contraindicated or vascular compression is absent, alternative treatments should be considered based on individual patient factors, balancing efficacy and long-term outcomes. Further high-quality studies are needed to refine treatment strategies for GPN.
How I do it: microvascular decompression for vago-glossopharyngeal neuralgia.
When a neurovascular conflict is demonstrated on high-resolution MRI in patients with drug-refractory Vago-Glossopharyngeal neuralgia microvascular decompression is regarded as the most effective therapeutic option. The aim of surgery is to relieve the conflict, most often produced by the posterior inferior cerebellar artery, not infrequently in association with a compressive megadolicho-vertebrobasilar artery. This can be safely achieved by exposure of the Vago-Glossopharyngeal nerve complex through a retromastoid-retrosigmoid infrafloccular approach through the cisterna magna, allowing exposure the lateral medulla and root entry zone of the lower cranial nerves. This report provides our step-by-step recommended technique for optimal efficacy and safety.
Evaluation of ultrasound-guided glossopharyngeal nerve block technique: A prospective observational study.
Glossopharyngeal nerve block provides long-term pain relief in glossopharyngeal neuralgia patients; the nerve block can be performed using landmarks or ultrasound guidance. The present study has evaluated the efficacy of ultrasound-guided glossopharyngeal nerve block utilizing the small-sized hockey stick probe. The present study was a prospective, observational study; twenty-five adult patients diagnosed with primary glossopharyngeal neuralgia not responding to medical management were included in this clinical trial. Glossopharyngeal nerve block was done under ultrasound guidance using hockey stick probe; patients having more than 50% reduction in numeric rating scale (NRS) score, for at least 2 h following nerve block, were enrolled in the study and followed for 6 months. The primary outcome measure was the severity of pain, measured by NRS score. Secondary outcome measures were percentage pain relief, reduction of analgesic usage, and PHQ-9 score for psychological assessment. All these assessments were done prior to the procedure and at 2 weeks, 1, 3 and 6 months after the procedure. We observed significant reduction in the NRS scores at 2 weeks (1.7 ± 1.6), 1 (1.9 ± 1.3), 3 (1.8 ± 1.3) and 6 months (2.1 ± 1.5) after Glossopharyngeal nerve block as compared to the baseline (6.1 ± 1.3; P value < 0.05); we also observed a significant pain relief (76%) and significantly reduced analgesic consumption (68%) and PHQ-9 scores (2.3 ± 1.7) compared to the baseline values (P value < 0.05). Ultrasound-guided glossopharyngeal nerve block with a linear array hockey stick probe provided significant pain relief in 75% of study participants with glossopharyngeal neuralgia over a six-month follow-up period.
