Species-Level Comparative Metagenomic Analysis of the Bacterial Abundance of the Gut Microbiome in Psoriasis, Hidradenitis Suppurativa, and Pemphigus Foliaceous Patients Using Shotgun Next-Generation Sequencing.
Recent studies have revealed a specific relationship between gut bacteria and inflammatory skin profiles. We aimed to perform a species-level comparative metagenomic analysis of the gut microbiome in patients with psoriasis, hidradenitis suppurativa (HS), and pemphigus foliaceus (PF). We included omnivorous nonsmokers and nondrinkers with psoriasis (n = 24), HS (n = 10), and PF (n = 11), as well as healthy controls (n = 10). We collected faecal samples from all patients for classic parasitological analysis. Gut microbiome analysis was conducted using shotgun next-generation sequencing. We used the Deseq2, Limma_voom, LinDA, and MaAMaAsLin 2 bioinformatics tools to evaluate concordance and differential abundance between patients. Thirteen patients (23.64%) were diagnosed with active intestinal parasitosis. The presence of intestinal parasitosis was significantly related to immunosuppression (p = 0.009). The most abundant microorganism species found in the faeces of the patients evaluated was Escherichia coli. Psoriasis patients presented a greater abundance of bacteria from the Veillonellaceae family, whereas PF patients presented a greater abundance of Firmicutes bacteria. Patients with PF showed increased E. coli virulence and antibiotic resistance functional markers. Immunosuppression significantly influenced the presence of intestinal parasitosis as well as increased the virulence of functional markers in patients with PF receiving systemic corticosteroid therapy.
A Single-Center Profile of Pemphigus in China: Significant Diagnostic Delay and Evolving Treatment Patterns.
Pemphigus encompasses a group of rare and potentially life-threatening autoimmune bullous diseases. Epidemiological research on pemphigus in China remains limited. This study aimed to analyze the clinical characteristics, diagnostic delay, and initial treatment patterns of patients with newly diagnosed pemphigus at a tertiary center in China. This retrospective study included patients newly diagnosed with pemphigus between January 2020 and December 2024 at the outpatient department of Peking Union Medical College Hospital. Demographic and clinical data were collected and analyzed. A total of 138 patients were included. Pemphigus vulgaris was the most prevalent subtype (68 cases, 49.3%), followed by pemphigus erythematosus (34 cases, 24.6%), pemphigus foliaceus (13 cases, 9.4%), and pemphigus herpetiformis (11 cases, 8.0%). Notably, the diagnostic delay was substantial, with a median of 5.0 (2.0-12.0) months, and 106 patients (76.8%) had been misdiagnosed before a definitive diagnosis was made. Regarding initial treatment, the most frequently used drugs were corticosteroids (104/132, 78.8%), followed by mycophenolate mofetil (33/132, 25.0%), Tripterygium wilfordii Hook F (29/132, 22.0%), minocycline (28/132, 21.2%), and rituximab (24/132, 18.2%). There was no significant difference among the subtypes in the proportion of patients receiving non-steroidal therapies. These findings highlight a significant diagnostic delay and outline evolving treatment patterns for pemphigus in a contemporary Chinese cohort. This information may inform future research directions and health policy decisions for managing this rare disease.
Diagnostic Accuracy of Anti-Desmoglein 1 and 3 Antibodies by Enzyme-Linked Immunosorbent Assay (ELISA) in Pemphigus Vulgaris.
To determine the diagnostic accuracy of anti-Desmoglein 1 (Dsg1) and anti-Desmoglein 3 (Dsg3) antibodies detected by the enzyme-linked immunosorbent assay (ELISA) in clinically suspected cases of pemphigus vulgaris (PV). A cross-sectional study. Place and Duration of the Study: Department of Immunology, the Armed Forces Institute of Pathology, Rawalpindi, Pakistan, from July to December 2024. A total of 105 clinically suspected PV patients were enrolled using a non-probability consecutive sampling technique. Serum was analysed for the detection of Dsg1 and Dsg3 antibodies by the ELISA, with histopathology and direct immunofluorescence used as the reference methods for skin biopsy examination. The diagnostic accuracy of the ELISA was assessed through sensitivity, specificity, positive and negative predictive values, and Cohen's kappa statistics. A total of 105 clinically suspected cases, comprising 50 (47.6%) males and 55 (52.4%) females (male-to-female ratio 10:11), were included. PV was detected in 82 (78%) cases by both Immunofluorescence and histopathology, while 23 (22%) were diagnosed as other (non-PV). Among the PV, 70 (85.4%) were positive for both Dsg1 and Dsg3 antibodies, while 12 (14.6%) were negative. Anti-Dsg1 and 3 positivity was observed in 10 (9.5%) and 5 (4.7%) non-PV cases, respectively, diagnosed as pemphigus foliaceus and bullous pemphigoid. Both anti-Dsg1 and anti-Dsg3 yielded equal sensitivity of 85.4%; however, the specificity of anti-Dsg1 was lower (56.5%) than that of anti-Dsg3 (78.3%) antibodies. Both anti-Dsg1 and anti-Dsg3 antibodies detected by the ELISA demonstrated good sensitivity; the comparatively lower specificity of anti-Dsg1 antibodies, however, restricted its applicability in PV in resource-limited settings. Case Report: Pemphigus following COVID-19 mRNA vaccination: two cases achieving long-term remission without relapse.
Autoimmune blistering diseases (AIBDs) are mediated by autoantibodies targeting structural proteins of the epidermis or dermo-epidermal junction. Since the introduction of COVID-19 mRNA vaccines, isolated cases of pemphigus temporally associated with vaccination have been described, although causal relationships remain uncertain. We report two patients who developed pemphigus following COVID-19 mRNA vaccination and showed an exceptionally favorable clinical course. A 66-year-old man developed pemphigus foliaceus one week after his second vaccine dose, and a 45-year-old woman developed pemphigus vulgaris two months after her second dose. Both patients responded promptly to corticosteroid therapy, with normalization of autoantibody titers and achievement of long-term complete remission lasting over two years without relapse or maintenance therapy. These outcomes stand in contrast to sporadic pemphigus, which usually follows a chronic relapsing course, and suggest that vaccine-related pemphigus may, in rare instances, represent a transient and self-limited immune dysregulation. Such cases are exceptional and should not discourage COVID-19 vaccination.
Correlation of platelet-related parameters and autoantibodies in patients with autoimmune bullous diseases.
Autoimmune bullous diseases (AIBDs) are rare, tissue-specific autoimmune diseases of the skin, and corresponding autoantibodies have been proved to be pathogenic. Recently, we reported that variations in platelet-related parameters may reflect the fluctuations of circulating AIBD-correlated antibody titers during the disease progression of 1 patient with AIBD. The purpose of this article is to further investigate the possible correlation between autoantibody titers and platelet-related parameters in patients with AIBD. This study collected data on autoantibody titers and platelet-related parameters from 136 patients with bullous pemphigoid positive for anti-BP180 antibodies, 54 patients with pemphigus foliaceus positive for anti-desmoglein (Dsg) 1 antibody, 55 patients with pemphigus vulgaris positive for both anti-Dsg1 and Dsg3 antibodies, and 16 patients with pemphigus vulgaris positive for anti-Dsg3 antibody alone. Two groups of healthy individuals served as controls. Comparative analyses revealed clinically significantly elevated platelet-related parameters, such as platelet count and thrombocytocrit, in the autoantibody-positive patient groups relative to control individuals. Correlation analyses demonstrated statistically significant positive associations between autoantibody titers and specific platelet-related parameters. These findings represent the first documented evidence of a positive correlation between autoantibody titers and platelet-related parameters in patients with AIBDs. The data implied that platelets may contribute to the disease pathophysiology and progression of AIBDs.
