Immunotherapies on Guillain-Barré syndrome and other acute autoimmune neuropathies.
Guillain-Barré syndrome (GBS) and other acute autoimmune neuropathies are rare but severe immune-mediated diseases of the peripheral nervous system. Early diagnosis is important for adequate monitoring and treatment, but it can be complicated by the rarity, heterogeneity in clinical presentation, and extensive differential diagnosis. The current proven effective treatments for GBS are intravenous immunoglobulins administered within 2 weeks and plasma exchange within 4 weeks from the onset of weakness. These therapies are immunomodulatory and aim to prevent further damage of peripheral nerves during the acute stage of the disease. Here we provide an overview of the early presentation of GBS and the required diagnostic workup to discriminate GBS from other causes, including other acute forms of autoimmune polyneuropathies. In addition, the current evidence-based immunotherapies for GBS are discussed along with the presumed pathophysiologic mechanisms of action and practical recommendations for the treatment in clinical practice. Despite current treatment, a substantial proportion of patients with GBS have a poor clinical course and outcome. We will present the latest developments regarding potential new treatments that are currently being evaluated and discuss various opportunities to optimize the design of future treatment trials toward a more effective and personalized treatment of GBS.
Overview of Immunotherapy Administration for Miller Fisher Syndrome in Japan: A Nationwide Retrospective Cohort Study.
Miller Fisher syndrome (MFS) is typically associated with a benign course and spontaneous recovery. However, immunotherapies such as intravenous immunoglobulin (IVIG) and plasma exchange (PE) are often administered for this condition because of the potential for clinical deterioration. We aimed to evaluate the real-world use of immunotherapies in MFS management using a nationwide Japanese database. We conducted a retrospective cohort study between April 2014 and March 2020 using the Japanese Diagnosis Procedure Combination database. Patient demographics and treatment modalities were described. Outcomes included length of hospital stay, in-hospital mortality, hospitalization cost, and activities of daily living defined by the Barthel index score. We identified 1,595 patients with MFS. Of these, 999 (62.6%) received immunotherapy (including IVIG for 908 [56.9%] patients, intravenous methylprednisolone for 219 [13.7%] patients, and PE for 18 [1.8%] patients). In patients with and without immunotherapy, the median length of hospital stay was 18 and 12 days, in-hospital mortality was 0.7% and 0.2%, median total hospitalization costs were ¥1,660,200 and ¥549,375, and the proportions of Barthel index score of 95-100 at discharge were 75.3% and 75.2%, respectively. Our findings highlight the widespread use of immunotherapies for MFS in Japan despite its generally favorable natural course.
Human Herpesvirus 6-Associated Miller-Fisher Syndrome in a 5-Year-Old Child: A Case-Based Narrative Review of Pediatric Cases with Infectious Triggers.
Miller-Fisher syndrome (MFS) is a rare Guillain-Barré variant defined by ophthalmoplegia, ataxia, and areflexia. Pediatric MFS is uncommon, and infectious triggers remain underrecognized. Human herpesvirus 6 (HHV-6) is neurotropic but rarely linked to immune-mediated neuropathies. In this paper, we describe a child with MFS associated with HHV-6 detected in cerebrospinal fluid (CSF) and review reported pediatric infections related to MFS. A 5-year-old girl presented with acute ophthalmoplegia, ataxia, and diminished reflexes. Neuroimaging, ophthalmologic tests, CSF analyses, and serologic andpolymerase chain reaction (PCR) assays were performed, including multiplex reverse transcription-PCR of cerebrospinal fluid using the BioFire® Meningitis/Encephalitis panel. A literature search was performed on Pubmed to identify pediatric (0-18 years) MFS cases with infectious triggers. Two reviewers independently screened and summarized the literature, and a PRISMA-style flow diagram was used to transparently report the study selection process. HHV-6 DNA was detected via CSF PCR twice, while tests for other pathogens were negative. Anti-GQ1b and related antibodies were negative or borderline. The patient received intravenous immunoglobulin and corticosteroids, with full recovery after one month. Among 20 published pediatric cases (1997-2021), Campylobacter jejuni was most frequent, followed by Mycoplasma pneumoniae and influenza viruses. Anti-GQ1b IgM positivity and favorable outcomes were commonly reported, including cases managed conservatively. This case raises the hypothesis that HHV-6 may represent a potential post-infectious association in pediatric MFS. The review findings indicate that pediatric MFS generally follows infection, responds well to immunotherapy, and has an excellent prognosis. Viral testing may be considered in selected, hypothesis-generating contexts in atypical or seronegative pediatric MFS presentations.
The eye as a diagnostic key: the ophthalmologist's role in an atypical case of Miller Fisher Syndrome.
Miller Fisher syndrome (MFS) is an uncommon variant of Guillain-Barré syndrome characterized by the triad of ophthalmoplegia, ataxia, and areflexia. Diagnosis can be particularly challenging when the clinical presentation is incomplete or atypical, especially in early stages. We report the case of a 54-year-old woman who presented to the emergency department with blurred vision and nonspecific dizziness, without diplopia or other relevant neurological findings. Hospital discharge was initially considered. However, ophthalmologic examination revealed severe bilateral ophthalmoplegia with supranuclear involvement and convergence impairment, in the absence of pupillary abnormalities. These findings prompted hospital admission and neurological evaluation, which confirmed the diagnosis of MFS. Treatment with intravenous immunoglobulins led to complete recovery. This case represents an atypical form of MFS, lacking all three classical components at onset and showing no positive findings on serologic or neuroimaging studies. The initial clinical suspicion raised by the ophthalmology team was essential to prevent premature discharge and allowed early diagnosis and treatment. This report underscores the importance of thorough ocular examination in patients with nonspecific visual symptoms and highlights the key role of ophthalmologists in the early recognition of rare neurological disorders with ocular manifestations.
Timeliness and Diagnostic Yield of Suspected Botulism Notifications in a Tertiary Medical Center in Taiwan, 2014-2024.
Botulism is a rare but high-consequence foodborne intoxication in which early recognition and prompt reporting are essential for a timely public health response. We conducted a retrospective review of suspected botulism notifications reported through the hospital notifiable-disease system at National Taiwan University Hospital from 2014 to 2024. Demographics, presenting features, time from hospital presentation to notification, specimen types, and reference laboratory results were summarized, and the institutional response workflow was described as the operational context. Between 2014 and 2024, six suspected Clostridium botulinum cases were reported to the Taiwan Centers for Disease Control (Taiwan CDC). The mean age was 49.8 ± 30.9 years, and most patients were female (5/6). Presentations were heterogeneous, with three cases featuring primarily ocular symptoms and three featuring limb weakness. The median interval from hospital presentation to notification was 6.0 days (IQR, 2.25-10.5 days). Notifications originated from general wards (5/6) and the intensive care unit (1/6). Specimens submitted for testing included stool, vomitus, and/or serum; all samples tested negative for botulinum toxin. Final diagnoses included ophthalmoplegia, Guillain-Barré syndrome, Miller Fisher syndrome, diabetic polyneuropathy, encephalitis, and systemic toxic/metabolic disorder. Although confirmed botulism was not identified, suspected notifications activated the institutional response pathway and highlighted the importance of clinician recognition and timely reporting. Routine assessment of TOCC (travel, occupation, contact, and cluster) history together with dietary exposure and compatible neurologic features may improve early suspicion and enhance the public health value of hospital-based surveillance for botulism.
