Brief Report: Androgen Receptor Expression Is Associated With Male Predominance in Thymic Neuroendocrine Neoplasms.
Neuroendocrine neoplasms represent a rare and poorly understood collection of malignancies. A better understanding of their biology is needed to improve treatment options. Male predominance to the incidence of neuroendocrine neoplasms of the thymus suggests a biological basis for this observation. This single-institution, retrospective cohort study evaluated androgen receptor (AR) expression and other clinicopathologic features in thymic neuroendocrine neoplasms compared with thymic epithelial tumors and neuroendocrine neoplasms of other sites. Expression of neuroendocrine and prostate markers was also assessed. For further molecular characterization, copy number analysis of AR and other relevant genes was assessed by single nucleotide polymorphism array. Last, analysis of single genes and thymic epithelial gene sets was performed using published thymic tumor transcriptomes. A cohort was assembled from 17 patients with neuroendocrine malignancies of primary thymic origin with assessable tissue for immunohistochemical analysis. The cohort was predominantly male (12 males, five females). Immunohistochemical analysis of the AR demonstrated positive staining in nine cases, all of which were male. Compared with thymic neuroendocrine neoplasms, AR expression is considerably less frequent in thymomas, thymic carcinoma, and neuroendocrine neoplasms of other sites of origin. No relevant copy number changes in the AR were identified. Further molecular characterization revealed expression of ASCL1 and limited expression of common prostate lineage and differentiation markers within our cohort, including thymic neuroendocrine epithelial gene set enrichment in published transcriptomes of thymic neuroendocrine neoplasms. The AR is expressed in a considerable fraction of thymic neuroendocrine neoplasms and is associated with male predominance. This observation has implications for investigation of androgen deprivation and receptor blockade.
Dose and normal tissue complication probability analysis of various radiotherapy regimens for thymomas.
This study aimed to compare dosimetric differences and normal tissue complication probability (NTCP) among coplanar intensity-modulated radiation therapy (CO-IMRT), non-coplanar IMRT (NONCO-IMRT), coplanar volumetric-modulated arc therapy (CO-VMAT), and non-coplanar VMAT (NONCO-VMAT) in thymoma radiotherapy and clarify the value of non-coplanar plans. Forty-eight post-thymoma surgery patients were enrolled in this study. Four radiotherapy plans (50 Gy/25 fractions) were made for each patient. The target conformity index (CI), homogeneity index (HI), and organ-at-risk (OAR) (lungs, heart, etc.) dose parameters were then evaluated. NTCP was calculated using the Lyman-Kutcher-Burman (LKB) model. Statistical analysis was performed using the Student-Newman-Keuls (SNK-q) test (p < 0.05 for significance). There were no significant clinical differences in target volume coverage and homogeneity among the four plans. The lung V20, heart V30, spinal cord Dmax, and esophagus Dmax of the VMAT techniques were all lower than those of IMRT (p < 0.001). The breast dose of CO-VMAT was significantly higher, while the breast parameters of NONCO-VMAT were close to those of IMRT. NTCP results were consistent with dosimetric findings: VMAT plans had lower NTCP for the heart, spinal cord, and esophagus; moreover, the breast NTCP of CO-VMAT was significantly increased (1.75%), while that of NONCO-VMAT decreased to 0.28%, which was similar to that of IMRT. All four plans meet the thymoma target dose requirement. However, NONCO-VMAT is preferred for its advantages in multi-organ protection and balanced breast dose. Additionally, CO-VMAT should be used cautiously in female patients due to the higher breast dose.
Impact of the COVID-19 pandemic on thoracic surgery volume in Japan: an analysis of the National Clinical Database.
The COVID-19 pandemic has disrupted elective surgeries worldwide. This study evaluated the impact of the pandemic on thoracic surgery volumes in Japan and investigated real-world triage practices by thoracic surgeons. Using Japan's National Clinical Database, we analyzed both annual and quarterly trends in surgical procedures and patient characteristics of lung and thymic tumors from 2019 to 2023. Compared to 2019, surgeries for primary lung cancer, thymic tumors, and spontaneous pneumothorax declined in 2020, with the most pronounced decreases occurring in Q2 during Japan's first state of emergency. Specifically, the numbers declined by 8.7%, 9.7%, and 27.1%, respectively. The greatest decline in pathological (p-) stage 0-IA1 lung cancer cases occurred in Q2 2020 (- 22% vs. Q2 2019), while reductions in more advanced stages during Q2 remained below 10%, with peak decreases observed in Q3. By 2023, the number of p-Stage II-IV cases had decreased, while the number of IA1-IB cases had increased. Thymoma and thymic cysts declined in Q2 2020 (- 4.4%, - 35.1%, respectively), but thymic cancer remained stable. Notably, thymic tumor size showed no temporal change throughout the study period. Despite strategic triage by thoracic surgeons during the initial phase of the pandemic, the overall disease profile of thoracic malignancies remained largely unaffected by treatment delays through 2023 in Japan. Catheter-directed thrombectomy combined with angioplasty and stent implantation for superior vena cava syndrome: a case report.
Superior vena cava (SVC) syndrome (SVCS) caused by thrombosis in artificial vascular grafts is characterized by high morbidity and mortality, with endovascular treatment being the primary therapeutic approach. In this case report, we present a safe multimodal endovascular treatment protocol that can rapidly alleviate acute SVCS symptoms, combining targeted thrombectomy via a guiding catheter with balloon dilation and Bard venous stent placement. Such combined endovascular therapies remain relatively uncommon in clinical reports. A 61-year-old female with a 10-year history of mediastinal nodules was admitted to hospital for recurrent dizziness. Via computed tomography (CT), she was diagnosed with a malignant thymic tumor invading the SVC and subsequently underwent sternotomy via a video-assisted thoracoscopic approach for tumor resection and SVC reconstruction with an 8-mm expanded polytetrafluoroethylene (e-PTFE) graft. Postoperatively, she developed SVCS (upper extremity/chest wall swelling, cyanosis) due to graft occlusion. Emergency treatment included 8F guiding catheter thrombectomy, sequential balloon dilatation, and Bard venous stenting, which resolved the stenosis. She received anticoagulants (low-molecular-weight heparin and subsequent rivaroxaban) and had no thrombosis at follow-up in August 2024. The combination of guiding catheter thrombectomy, balloon dilatation, and Bard stent implantation is feasible, safe, and effective for e-PTFE graft-associated SVCS, relieving symptoms quickly and avoiding reoperation. This approach can benefit patients unsuited to surgery, but they should be monitored closely for adverse hemodynamic events after stenting. Further studies are needed to validate the efficacy of this method.
Comparative analysis of the tumor immune microenvironment between thymoma and thymic carcinoma.
Thymic epithelial tumors (TETs) often linked with autoimmune syndromes such as myasthenia gravis (MG), have an underexplored tumor immune microenvironment (TIME), limiting the effective use of Immune Checkpoint Inhibitors. This study aims to address the immunology of TETs, focusing on the prognostic significance of TIME and its association with MG. This study investigates the TIME in TET subtypes, thymic carcinoma (TC) and thymoma, assesses their prognoses, and examines the relationship between thymoma's immune microenvironment and MG. This study included 157 patients with TET who underwent surgical resection at Juntendo Hospital between January 2012 and September 2020. Immune cell densities of tissue specimens were analyzed using multiplex fluorescence immunohistochemical staining. Thymoma and TC were diagnosed in 145 and 12 patients, respectively. Thymoma types B1 and B2 showed elevated CD4+ and CD8+ T cells, while TC had decreased T cells and increased Foxp3+ T cells. Thymoma with MG had higher CD20+CD79a- cells counts in the stromal area compared to those without MG (p = 0.0165). Elevated CD4+ T cells (p = 0.0310), CD8+ T cells (p = 0.0215), and CD68+ cells (p = 0.0432) in the tumor area were linked to good prognosis, while higher CD20+CD79a- cells in the stromal area indicated poor prognosis (p = 0.0088). Thymomas had a higher prevalence of T cells compared to TC. CD4+ T cells, CD8+ T cells, and CD68+ cells in the tumor area were favorable prognostic markers, while B cells in the stromal area indicated a poor prognosis for thymoma.
