Preoperative sonography facilitates minimally invasive costal graft harvesting for reconstruction of advanced capitellar osteochondritis dissecans.
Costal osteochondral autograft transplantation is a promising surgical option for lateral capitellar osteochondral dissecans. However, this approach is limited by the difficulty in identifying the costal osteochondral junction (COCJ) in patients with a well-developed pectoralis major or excessive subcutaneous fat. Moreover, the cross-section of the costal cartilage usually remains unknown until the graft is harvested, which often leads surgeons to make long skin incisions. This study investigated the accuracy of sonography in identifying the COCJ and predicting the cross-sectional size of the costal cartilage. Two orthopedic surgeons performed sonographic assessment to identify 15 COCJs in Thiel-embalmed cadavers. They measured the long and short axes of the cross-section of 20 costal cartilages 10 mm away from the COCJ. After sonographic assessment, costal grafts were harvested to measure the distance between the sonographically marked and actual COCJ. The long and short axes of the costal cartilage 10 mm from the COCJ were manually measured. The mean distance between the sonographically marked and actual COCJ was 0.1 ± 0.4 and 0.7 ± 0.9 mm in Testers A and B, which were significantly smaller in Tester A with more experience in musculoskeletal sonography (p < 0.05). Pearson correlation coefficients showed significant, strong, and positive correlations between sonographic and manual measurements of costal graft size in both testers (for long axis, 0.79 in Tester A and 0.88 in Tester B, for short axis, 0.75 in Tester A and 0.73 in Tester B, p < 0.001 for all correlations). Our findings indicate the accuracy of sonography in assessing the COCJ and cross-sectional size of the costal cartilage. Preoperative sonography could provide help surgeons determine the number of costal grafts to be harvested in advance and minimize skin incisions during graft harvesting.
Juvenile Osteochondritis Dissecans of the Knee in the General Adolescent Population: Prevalence and Longitudinal Trends.
We established the prevalence of juvenile osteochondritis dissecans (JOCD) of the knee in a general adolescent population undergoing magnetic resonance imaging (MRI), irrespective of symptoms. This study was part of an open population-based cohort study, the BLINDED study. Between 2017 and 2020, adolescents aged 13 years old underwent MRI of both knees. These MRIs were assessed for signs of JOCD. The participant characteristics and presence of symptoms of those with and without JOCD were compared. If an MRI at 13 years old showed a JOCD lesion, a follow-up MRI at 17 years old was assessed as well. 1910 participants (13.5 years, interquartile range 13.4-13.7, 52% girls) were included in this study. 13 JOCD lesions were found in 11 participants. This corresponds to a prevalence of JOCD of 1 in 174 participants. All 11 participants with JOCD were boys, as opposed to 48.0% boys in the entire group (p=0.01). One participant with JOCD experienced pain in the knee (9.1%), compared to 8.0% of the total number of participants (p=0.89). Out of 7 follow-up MRIs at 17 years old, none of the participants had a JOCD lesion anymore. In this general adolescent population, we found a prevalence of 1 in 174 adolescents being affected by JOCD of the knee. This suggests that JOCD is not as rare as commonly perceived. As participants with JOCD did not have more pain than those who did not, this may indicate that JOCD occurs asymptomatically more often than previously perceived. Level III.
Restoration versus stimulation: Meta-analysis of osteochondral autograft transplantation and microfracture for osteochondritis dissecans of the capitellum.
Surgical management of osteochondritis dissecans of the capitellum (OCDC) can range from simple arthroscopic debridement to microfracture to osteochondral autograft transplantation (OAT). The current review aimed to pool the data from the published literature and analyse the outcomes of microfracture and OAT in OCDC. Electronic databases of PubMed and Embase were searched from inception to March 31, 2024 following PRISMA 2020 guidelines. The population of interest was patients with OCDC managed surgically by either microfracture or OAT. Studies were included if they had ≥24 months of follow-up. A total of 21 eligible studies (14 OAT, 7 microfracture) involving 452 patients were analyzed. The search yielded 14 studies that reported on the outcome of OAT and 7 studies on microfracture. 452 patients were included in the review (OAT = 370, Microfracture = 182). There was no statistical difference in the duration of return to sports between the two groups. A statistically significant difference was noted in the number of patients returning to sports in both groups (OAT - 98%, 95% CI 97-100, Microfracture - 78%, 95% CI 61-96). Similarly, a significant difference was noted when comparing the number of patients returning to their original sports in the OAT group (92%, 95% CI 87-96) as compared to the microfracture group (66%, 95% CI 48-84). Early diagnosis and intervention in OCDC have the potential to lead to optimal functional outcomes in the elbow. The indication for a particular surgery depends on the patient's age, skeletal maturity, lesion stability, symptoms as well as vocation. Patients undergoing OAT have a higher statistical chance of returning to their original sports compared to those undergoing microfracture. Long-term studies will help us further understand the natural course of the procedure. III.
Increased contact pressure at the radiocapitellar joint with anterior and posterior osteochondral defects in the sagittal section of the humeral capitellum: a cadaveric study.
In osteochondritis dissecans (OCD) of the humeral capitellum, defect location and size were important in surgical planning. Recently, it was reported that posterior or large osteochondral defects of the humeral capitellum on preoperative sagittal computed tomography affected outcomes of arthroscopic debridement for OCD in adolescent baseball players. Here, we investigated radiocapitellar joint contact pressure for anterior and posterior osteochondral defect positions in the sagittal section of the humeral capitellum. An anterior or posterior osteochondral defect of the humeral capitellum (10 mm in diameter) was created on each side in 7 matched pairs of fresh-frozen cadavers. Radiocapitellar joint contact pressure was measured with and without valgus stress at 0°, 30°, 60°, and 90° of elbow flexion and with and without varus stress at 0° of elbow extension. In the posterior defect group, comparison of radiocapitellar joint contact pressure was significantly greater when the elbow joint was at 0° extension and 90° flexion than at other angles with valgus and varus stress (P < 0.05). Lower locations in posterior defects in the sagittal section showed significantly greater radiocapitellar joint contact pressure at 0° extension and 90° flexion of elbow joint with valgus stress. (both P < 0.05). Radiocapitellar joint contact pressure with valgus and varus stress was significantly greater with a posterior osteochondral defect than with an anterior defect when the elbow joint was at 0° extension and 90° flexion. For posterior defects, the contact pressure increased at lower locations in defects in the sagittal section.
Capitellar Osteochondritis Dissecans Lesion Treated With Bioabsorbable Pin, Extracellular Matrix Cartilage Allograft, and Intraosseous Bioplasty: A Case Report.
Capitellar osteochondritis dissecans (OCD) is a painful and debilitating condition, most often seen in adolescent overhead athletes and gymnasts. Treatment options for unstable capitellar OCD lesions include fragment fixation, autologous chondrocyte implantation, osteochondral autograft, fragment debridement, and drilling of the lesion. While many current surgical techniques yield suboptimal outcomes, this case demonstrates a novel technique for treating an unstable capitellar OCD lesion with associated subchondral cyst in a 14-year-old female patient. This technique combines bioabsorbable pin fixation and allograft cartilage extracellular matrix with intraosseous bioplasty to restore the articular surface and subchondral support. A lateral approach to the elbow was used to create a reamed window in the capitellum, allowing access to the cystic defect. The osteochondral fragment was fixed with a poly-L-lactide acid pin, and the defect was backfilled using demineralized bone matrix mixed with bone marrow aspirate. The residual cartilage lesion was filled with extracellular matrix cartilage allograft. The graft was contained in the lateral window with fibrin glue. At 4-month follow-up, the patient displayed full elbow range of motion with no pain. At 6-year follow-up, magnetic resonance imaging showed a well-healed capitellum with normal contour and signal of the repaired cartilage. This case demonstrates a novel technique for treating OCD lesions of the capitellum with durable clinical and radiographic success.
