Percutaneous retrieval of the lattice-tip catheter entrapped in post-surgical atrioventricular valve apparatus: two case reports.
The novel lattice-tip catheter (Sphere-9, Medtronic) enables high-resolution mapping and the ability to switch between radiofrequency and pulsed electric field. Initial experience has shown promising results for treating complex ventricular tachycardias (VTs). However, safety data remain limited, particularly regarding catheter-valve interactions. We report two cases of mechanical entrapment of the lattice-tip catheter in the atrioventricular valves during VT ablation performed via a retrograde transaortic approach. In both cases, the catheter became entangled within the subvalvular apparatus-specifically, in the tricuspid valve of a patient with congenitally corrected transposition of the great arteries and in the mitral valve of a patient who had previously undergone mitral repair. Standard retrieval manoeuvres were unsuccessful, and resolution required percutaneous bailout strategies, specifically transseptal access with bioptomes or forceps. Both procedures were completed without long-term sequelae. These are the first reported cases of large-footprint catheter entrapment in the atrioventricular valve apparatus, highlighting potential mechanical risks associated with its employment in the ventricular setting. Our experience emphasizes the importance of preferring transseptal access over retrograde access where feasible, due to its more favourable trajectory and the ability to support safer rescue techniques. Real-time imaging, particularly intracardiac echocardiography, proved essential both for navigation and complication management. While transaortic access remains a viable option, it should be approached with caution, necessitating a thorough understanding of device behaviour and a readiness to manage complications. These observations add important safety considerations for the expanded use of novel ablation platforms in patients with complex ventricular anatomy.
Percutaneous treatment with the 'valve-in-valve' technique in congenitally corrected transposition of the great arteries and systemic atrioventricular valve with severe double lesion: case report.
Congenitally corrected transposition of the great arteries (CCTGA) is a rare congenital heart disease characterized by atrioventricular (AV) and ventriculoarterial discordance, allowing near-normal circulation without early surgical correction. However, long-term complications such as valvular dysfunction, systemic right ventricular (RV) failure, and pulmonary hypertension are common. Management becomes particularly challenging in patients with prior interventions, where anatomical complexity and elevated surgical risk limit therapeutic options. We present the case of a 47-year-old Chinese female diagnosed with CCTGA who had previously undergone surgical implantation of a bioprosthetic valve in the systemic AV position in her home country. She presented with significant deterioration in functional capacity, and imaging revealed severe dysfunction of the surgical prosthesis, characterized by severe mixed valvular disease, aneurysmal dilation of the left atrium, and severe systemic RV dysfunction. A multidisciplinary team assessment determined that the patient was at high surgical risk, leading to the decision to perform a percutaneous valve-in-valve implantation. An Edwards SAPIEN® 26 mm bioprosthetic valve was successfully implanted, yielding excellent haemodynamic results without complications. The patient was discharged 24 h post-procedure with significant clinical improvement. Although percutaneous interventions for systemic tricuspid valve disease have been described, they typically involve native valves and remain anecdotal. To date, no published reports have documented valve-in-valve implantation in a systemic AV prosthesis in a patient with CCTGA. This case illustrates the feasibility and safety of such an approach in a high-risk patient, offering a valuable alternative to surgical replacement and potentially delaying the need for heart transplantation.
Anaesthetic management of a paediatric patient with situs inversus totalis and congenitally corrected transposition of the great arteries undergoing adenotonsillectomy.
Situs inversus totalis (SIT) with congenitally corrected transposition of the great arteries (ccTGA) represents a rare and complex congenital combination that significantly complicates anaesthetic management. We report the anaesthetic management of an early childhood (2-5 years), 13 kg child diagnosed with SIT and ccTGA undergoing elective adenotonsillectomy. Individualised perioperative strategies were implemented, including modified ECG monitoring, arrhythmia preparedness and explicit mg/kg drug dosing. Intraoperative management was uneventful, and the patient had a stable recovery. This case highlights anaesthetic considerations and monitoring adaptations in non-cardiac paediatric surgery involving mirror-image congenital cardiac anatomy.
Arrhythmias in congenitally corrected transposition of the great arteries: an international study.
There is a paucity of data on arrhythmia burden in patients with congenitally corrected transposition of the great arteries (ccTGAs). The present study sought to quantify the incidence of atrial arrhythmia (AA), ventricular arrhythmia (VA), and complete atrioventricular block (CAVB) in patients with ccTGA and identify associated factors. An international, multi-centre, retrospective cohort study was conducted in 29 tertiary hospitals (six countries) between 1990 and 2018. Primary analyses consisted of determining the incidence of a combined outcome consisting of AA, VA, or CAVB, along with its individual components. Factors associated with the different types of arrhythmias were assessed by uni-variable and multi-variable Cox regression analyses. A total of 1131 patients with ccTGA were followed for 9.0 (interquartile range 4.0-17.2) years. Cumulative rates of the primary endpoint at 10, 15, and 20 years were 44.5%, 51.0%, and 58.8%, respectively. AA, VA, and CAVB occurred in 2.1, 1.4, and 2.0 cases per 100 person-years, respectively. In multi-variable analyses, surgery/intervention conferred a greater than three-fold higher risk of AA [hazard ratio (HR) 3.01, 95% confidence interval (CI) 1.90-4.78, P < .001]. Furthermore, surgery/intervention was significantly associated with a greater risk of VA (HR 1.73, 95% CI 0.98-3.05, P = .003) and CAVB (HR 3.65, 95% CI 2.20-6.05, P < .001). The left bundle branch block was associated with a higher risk of VA (HR 4.03, 95% CI 1.59-10.23, P = .003) and the right bundle branch block with a higher risk of CAVB (HR 3.71, 95% CI 1.81-7.63, P < .001). The arrhythmia burden in patients with ccTGA is substantial, with a high incidence of AA, VA, and CAVB.
