Evaluating Photochemical Interactions Between Rose Bengal Photodynamic Antimicrobial Therapy and Topical Antifungals in Fungal Keratitis Isolates.
The purpose of this study was to evaluate the in vitro effects of combining rose bengal (RB) photodynamic antimicrobial therapy (PDAT) with conventional topical antifungals (amphotericin B, natamycin, and voriconazole) on clinical Fusarium and Aspergillus isolates and to determine the antifungal stability under experimental conditions. Clinical isolates of Fusarium spp. (n = 3) and Aspergillus spp. (n = 3) were cultured using standard microbiology techniques. Agar plates were prepared with three concentrations of amphotericin B, natamycin, or voriconazole along with plates containing no antifungals. The following groups were tested in both dark and light conditions: (1) antifungal monotherapy (antifungal only), (2) RB monotherapy (RB only), and (3) antifungal and RB combination therapy. Plates were incubated for 7 days and percent growth inhibition was quantified. UV-visible spectroscopy and mass spectrometry was performed to assess antifungal stability. Overall, Fusarium isolates were more susceptible to RB-PDAT than Aspergillus isolates. RB-PDAT combination therapy with natamycin reduced growth inhibition, whereas combination therapy with voriconazole increased growth inhibition compared to antifungal monotherapy. UV-visible spectroscopy demonstrated 70% degradation of amphotericin B and full degradation of natamycin when combined with RB-PDAT, whereas voriconazole remained stable. Across both Fusarium and Aspergillus isolates, treatment responses to antifungals with or without RB-PDAT demonstrated strain-specific effects. The increased fungal inhibition and maintained stability of voriconazole when combined with RB-PDAT highlight its potential compatibility with RB-PDAT. In contrast, the reduced inhibition and degradation of natamycin and amphotericin B with RB-PDAT suggests that drug stability may be a factor to consider when integrating RB-PDAT into fungal keratitis management.
Disseminated Fungal Infection in Profoundly Immunocompromised Hosts: A Three-Case Series.
Disseminated fungal infection (DFI) is uncommon in the general population but represents a severe and frequently fatal complication in immunocompromised patients, particularly those with hematologic malignancies and prolonged neutropenia. In high-risk settings, invasive fungal infections occur with clinically meaningful frequency; a large systematic review of critically ill patients reported a pooled prevalence of invasive fungal infection of approximately 5%, highlighting that while DFI is uncommon in the general population, serious fungal disease is encountered more frequently in select high-risk cohorts. We report a case series of three immunocompromised patients with DFI to highlight shared clinical patterns, diagnostic limitations, and poor outcomes. Two patients developed disseminated fusariosis with pulmonary nodules and characteristic necrotic cutaneous lesions, while the third developed Scedosporium prolificans fungemia with pulmonary involvement and progressive encephalopathy concerning for early central nervous system (CNS) dissemination. Diagnosis required a multimodal approach, including tissue biopsy, bronchoalveolar lavage (BAL), blood cultures, and advanced molecular testing; however, precise quantification of diagnostic delay was not systematically available across cases. In this series, diagnosis was nonetheless delayed relative to initial clinical presentation, largely due to nonspecific radiographic findings, initially negative or indeterminate cultures, overlap with bacterial or noninfectious etiologies, and the need for invasive sampling in profoundly cytopenic patients. Management was further limited by intrinsic antifungal resistance, drug toxicity, and failure of immune recovery; despite aggressive therapy, all patients experienced rapid clinical deterioration over days to weeks following diagnosis, culminating in death or transition to hospice care. These cases underscore the aggressive nature of DFI in profoundly immunocompromised hosts, the central role of neutropenia in facilitating dissemination, and the need for early recognition, multidisciplinary management, and timely goals-of-care discussions when DFI occurs in the setting of advanced immunosuppression.
Surgical Aspects of Patients With Leukemic Testicular Infiltration.
Leukemia is the most common childhood cancer, accounting for one-third of malignancies in this age group. Testicular infiltration in pediatric leukemia patients represents a manifestation of leukemic dissemination beyond the bone marrow and can occur at diagnosis or be a sign of relapse. Testicular infiltration may occur particularly in specific leukemia subtypes, such as acute lymphoblastic leukemia (ALL) and lymphoblastic lymphoma (LBL) and is an important prognostic factor, as it may indicate an increased risk of leukemia relapse. Therefore, rigorous follow-up is essential to detect testicular infiltration early and initiate appropriate treatment. This study aims to evaluate cases of leukemia with testicular infiltration in two pediatric oncology referral centers in two different countries. We conducted a retrospective study of patients presenting with secondary testicular infiltration due to leukemia over a 14-year period (January 2010 to December 2023). During the study period, we identified 24 patients who developed testicular infiltration secondary to leukemia. All cases of testicular infiltration were diagnosed at the time of disease relapse. Fourteen patients presented with isolated testicular relapse, while the remaining patients had additional relapse sites in combination with testicular recurrence: bone marrow (10 cases) and central nervous system (2 cases). The mean age at diagnosis was 7.91 years. The average leukocyte count at diagnosis was 56 211/mm3, with only five patients presenting with leukocyte counts above 100 000/mm3. Four children (16.6%) were diagnosed with T-cell ALL, 20 (83.4%) with B-cell ALL. Fifteen patients (62.5%) underwent hematopoietic stem cell transplantation, including two haploidentical transplants. Twelve patients (50%) died-three due to disease progression and one from disseminated fusariosis. Data regarding the cause of death were unavailable for the remaining patients. The average follow-up duration was 5.87 years, with an event-free survival of 2.085 years. Although rare, testicular infiltration in leukemia patients is a clinically significant phenomenon that may affect both prognosis and quality of life. Early diagnosis and appropriate treatment of the underlying leukemia are crucial to improve outcomes. Monitoring of testicular function and fertility-related concerns must also be addressed comprehensively in patient management. Future studies with larger cohorts and standardized management protocols are essential to establish clear guidelines for the diagnosis, treatment, and follow-up of patients with testicular relapse secondary to ALL. Given the low number of cases and the diversity of local resources worldwide (especially regarding radiotherapy and endocrine follow up), the best approach for each patient should be determined in multidisciplinary team discussions.
Novel antifungal compounds against Fusarium spp: A systematic review.
Fusarium spp. has increased in relevance due to its resistance, high mortality, increasing incidence, and low preventability. The scarce treatment options compelled the development of antifungals with novel mechanisms of action. We conducted a systematic review to report on novel compounds and their activity on Fusarium spp. We searched for terms related to novel antifungals against Fusarium across multiple databases, up to February 2024. From 622 articles, only 15 articles met the eligibility criteria. The antifungals reported were manogepix (10), olorofim (3), ibrexafungerp (1), and MGCD290 (1), evaluated against 480 Fusarium isolates. Manogepix MIC/MECs had broad ranges for most Fusarium species, with notable exceptions like F. solani (MEC: ≤0.12μg/mL) and F. fujikuroi (MEC: ≤0.25μg/mL). Olorofim could be a good candidate for fusariosis treatment based on in vitro activity. Ibrexafungerp and MGCD290 did not show good activity as monotherapy. Further work on applications and development of new treatments against the resistant Fusarium species is essential.
Fusariosis in Patients With Hematologic Malignancies in the Era of Antifungal Prophylaxis.
Invasive fusariosis (IF) is an uncommon but frequently fatal infection among patients with hematologic malignancies (HM). Despite widespread mould-active prophylaxis, breakthrough infections and treatment failures may occur. We aim to describe the clinical epidemiology, diagnostic approaches, antifungal management strategies, and outcomes of IF in the era of routine antifungal prophylaxis. We conducted a retrospective study of all cancer patients diagnosed with IF at a tertiary cancer center that met probable/proven criteria for IF from 2015 to 2024. We collected data on demographics, oncologic history, diagnostics, therapy, and outcomes. Twenty-two patients met criteria for proven IF. Most had acute myeloid leukemia (73%), and many underwent prior hematopoietic cell transplant (36%); nearly all were neutropenic (96%, median:48 days). Skin was the most common clinical site of infection (68%), followed by sino-pulmonary involvement (41%); fungemia occurred in 32%. Fever (77%) and skin nodules (64%) were frequent, while serum galactomannan was uniformly negative. Most common species complex was Fusarium fujikuroi. In our cohort, 82% occurred during antifungal prophylaxis, most often posaconazole. Minimum inhibitory concentrations were high for azoles and lower for amphotericin B and terbinafine. Antifungal regimens ranged from monotherapy to triple therapy. Overall mortality was 68%, of which 45% of deaths were attributed to IF. Mortality did not differ by antifungal regimen, species complexes, or presence of fungemia. IF remains a life-threatening infection in patients with HM, often presenting as disseminated disease, despite prophylaxis. High azole MICs and the observed diversity of antifungal regimens highlight the ongoing uncertainty regarding optimal treatment and the need for prospective studies to define the role of combination therapy.
