Primary low-grade breast angiosarcoma initially misdiagnosed as a benign vascular lesion.

Primary breast angiosarcoma is an exceptionally rare malignancy, particularly in elderly patients, and may closely resemble benign vascular lesions on imaging and core needle biopsy. We report a case of low-grade primary angiosarcoma in a woman in her late 70s, initially misdiagnosed as a haemangioma despite interval tumour growth. A definitive diagnosis was achieved only after wide local excision, which demonstrated well to intermediately differentiated angiosarcoma with clear margins. Given the limited evidence supporting adjuvant therapy in low-grade disease, surgery alone was appropriate, and the patient remains recurrence-free at 2.5 years. This case highlights the diagnostic challenges and importance of maintaining suspicion in older adults.

Primary cerebellar angiosarcoma: A rare case of posterior fossa vascular tumor with hemorrhagic presentation and favorable surgical outcome.

Primary cerebellar angiosarcoma is an exceptionally rare intracranial malignancy, with only 22 cases reported in the literature. We report the case of a 67-year-old male who presented with a severe headache, unsteady gait, and giddiness. Computed tomography of the brain demonstrated a hemorrhagic lesion in the right cerebellar hemisphere with associated vasogenic edema and early hydrocephalus. Magnetic resonance imaging revealed a right cerebellar intra-axial lesion with mixed signal intensities and a characteristic peripheral "bull's eye" enhancement pattern. Histopathological evaluation showed a vasoformative neoplasm with atypical endothelial cells, high mitotic activity, and immunopositivity for CD31 and erythroblast transformation specific (ETS)-related gene (ERG), consistent with low-grade angiosarcoma. The patient underwent gross total resection and remained recurrence-free 16 months postoperatively. This case underscores the importance of multi-modality imaging in the early recognition of hemorrhagic cerebellar tumors and reinforces the role of radiological-pathological correlation in diagnosis and treatment planning. Given its rarity, diagnosis of cerebellar angiosarcoma requires a high index of suspicion, supported by advanced imaging and immunohistochemical profiling. Multidisciplinary management is essential, and further case documentation is necessary to guide therapeutic strategies and prognostication.

Hepatic Angiosarcoma Associated with Chronic Schistosomiasis in a Patient with Decompensated Cirrhosis: A Rare Case Report.

Hepatic angiosarcoma (HA) is a rare endothelial malignancy with poor prognosis. Chronic Schistosoma infection causes periportal fibrosis and portal hypertension, but has not been established as a risk factor for hepatic vascular tumors. We report a case of high-grade HA coexisting with chronic schistosomiasis in a patient presenting with acute decompensated cirrhosis. A 64-year-old Filipino woman with no known chemical exposures presented with progressive abdominal pain, jaundice, ascites, and hepatic encephalopathy. Doppler ultrasound initially suggested Budd-Chiari syndrome, later excluded on advanced imaging. MRI showed multifocal hepatic lesions, and CT demonstrated multiple intra-abdominal hematomas without active bleeding. Liver biopsy confirmed poorly differentiated HA (CD31+/CD34+) and revealed calcified Schistosoma eggs. Her course was complicated by hemorrhagic shock after biopsy and paracentesis, spontaneous bacterial peritonitis, acute kidney injury, and worsening hepatic encephalopathy. She died despite aggressive supportive care. This case illustrates the diagnostic challenges and rapid progression of HA, especially in coagulopathic patients where biopsy carries significant risk. The coexistence of chronic schistosomiasis raises the possibility of parasitic inflammation contributing to hepatic vascular tumorigenesis. HA should be considered among unexplained hepatic masses with atypical imaging and clinical features.

Primary splenic angiosarcoma with diffuse intrasinusoidal hepatic metastasis mimicking lymphoma: a diagnostic challenge.

Primary splenic angiosarcoma is a rare aggressive vascular neoplasm. Although hepatic metastases are common, diffuse intrasinusoidal hepatic involvement is extremely rare and poses a significant diagnostic challenge. A 60-year-old woman presented with bilateral lower extremity edema, hypoalbuminemia, and thrombocytopenia. Contrast-enhanced computed tomography revealed marked splenomegaly, but no distinct hepatic lesions. Gadoxetic acid-enhanced magnetic resonance imaging revealed reduced hepatic uptake without focal abnormalities. 1⁸F-fluorodeoxyglucose positron emission tomography-computed tomography revealed diffuse radiotracer uptake in both the liver and spleen, raising the suspicion of an infiltrative malignant process, such as lymphoma. Liver biopsy revealed sinusoidal infiltration by atypical cells positive for cluster of differentiation 31 and ETS-related gene, but lacked architectural features diagnostic of angiosarcoma. Due to diagnostic uncertainty and concerns regarding splenic rupture, splenectomy was performed. Histopathological examination confirmed a primary splenic angiosarcoma with diffuse intrasinusoidal hepatic metastasis. Primary splenic angiosarcoma rarely presents with non-nodular intrasinusoidal hepatic metastasis, mimicking infiltrative liver diseases. Liver biopsy may fail to yield a definitive diagnosis. Therefore, a splenectomy may be required to establish a definitive diagnosis and guiding appropriate management when liver biopsy is inconclusive. Recognizing this uncommon metastatic pattern is crucial to avoid misdiagnosis and ensure timely treatment.

Spinal epithelioid angiosarcoma: a diagnostically and therapeutically challenging entity treated with chemoradiotherapy.

Primary spinal angiosarcoma is extremely rare with few reported cases globally. While en bloc resection is the only definitive and curative treatment modality, this case highlights the potential for curative-intent, non-surgical management of spinal epithelioid angiosarcoma using taxane-based chemoradiotherapy, especially in anatomically challenging and surgically unresectable locations. It also emphasises the role of multimodal imaging and histopathological correlation in the diagnosis and monitoring of rare spinal tumours.
