Unilateral Multifocal Choroiditis in HTLV-1 Infection.
This case report discusses a unilateral multifocal human T-lymphotropic virus type 1 choroiditis in a male patient presenting with panuveitis of the right eye.
Interpretable Deep Learning for OCT-Based Diagnosis of Vitreoretinal Lymphoma Versus Uveitis.
To develop and validate an interpretable deep learning model that classifies optical coherence tomography (OCT) scans as vitreoretinal lymphoma (VRL) or non-infectious uveitis (NIU)-intermediate, posterior, or panuveitis-and visualizes differentiating pathological OCT features. This cross-sectional study included 45 patients with VRL and 52 with NIU who underwent SPECTRALIS OCT imaging. OCT B-scans were analyzed with a custom-designed deep convolutional neural network classification model combined with gradient-weighted class activation mapping (Grad-CAM). Interpretability was assessed using Grad-CAM heat maps that visualize disease-specific OCT features. We evaluated diagnostic performance at the eye level using the area under the receiver operating characteristic curve (AUROC), area under the precision-recall curve (AUPRC), accuracy, F1 score, precision, sensitivity, specificity, and kappa. The model differentiated VRL from NIU at the eye level, with mean ± SD scores of 76.0 ± 8.0 for AUROC, 79.4 ± 10.4 for AUPRC, 75.4 ± 13.3 for accuracy, 74.9 ± 13.7 for F1 score, 73.9 ± 19.3 for precision, 77.7 ± 13.6 for sensitivity, 73.4 ± 20.7 for specificity, and 51.0 ± 26.6 for kappa. Grad-CAM features of VRL included preretinal deposits and changes in the retinal pigment epithelium. In contrast, NIU features included denser vitreous cells and opacities, epiretinal membrane, intraretinal cells, and choroidal and scleral abnormalities consistent with NIU pathophysiology. A deep learning model with interpretability can reliably differentiate VRL from NIU, highlighting disease-relevant OCT features. The proposed deep learning method may serve as a non-invasive triage tool by identifying patients who need confirmatory pars plana vitrectomy.
Ocular Manifestations of Biopsy-Proven B Virus Infection with Central Nervous System Involvement.
B virus, or herpes B infection, is a rare zoonotic disease acquired through contact with macaques. Ocular involvement of B virus is rarely documented in literature, yet it may progress rapidly and result in blindness if not recognized promptly. To present a unique case of human B virus infection with clinical features including severe central nervous system dysfunction and bilateral panuveitis in a patient with history of monkey bite. This descriptive, qualitative report examined a unique case of B virus infection treated at a tertiary center in Hong Kong with in-depth analysis of clinical presentation and treatment response. The study was conducted in September 2025 and analyses were performed in November 2025. Clinical features of B virus-related uveitis and chorioretinitis were described in detail. Serial fundus photographs were used to monitor disease progression and response to treatment. The diagnosis of B virus meningoencephalitis with ocular involvement was confirmed with polymerase chain reaction analysis of cerebrospinal fluid and vitreous fluid. Ocular features of B virus infection of the reported case include anterior uveitis, vitritis, multifocal chorioretinitis, vasculitis, and optic nerve involvement. Treatment included systemic and intravitreal ganciclovir, and corticosteroids for inflammation control. Timely diagnosis and initiation of antiviral therapy are crucial for controlling B virus infection. Additional studies are required to better determine the clinical features and presentation of B virus infection in different patient demographics to reduce disease-related blindness and improve survival.
[Noninfectious uveitis in childhood : Clinical aspects, diagnostics, treatment and transition].
Intraocular inflammation in childhood represents a major clinical challenge as if untreated the disease can lead to severe complications and permanent vision loss. The onset is often paucisymptomatic, resulting in delayed detection and consequently a high risk of amblyopia. Pediatric uveitis is classified into idiopathic and associated forms. In juvenile idiopathic arthritis, it is considered an extra-articular manifestation. Among the most common systemic associations are sarcoidosis, tubulointerstitial nephritis and uveitis (TINU) syndrome, Behçet's disease and Vogt-Koyanagi-Harada syndrome. Less frequent, although always to be excluded, is an infectious etiology. As in adults, anterior uveitis is the predominant form in children, followed by intermediate, posterior and panuveitis. Depending on the predominant site of inflammation, key clinical findings and ancillary testing, a differential diagnostic work-up is required to initiate an individualized treatment strategy. Die intraokulare Entzündung im Kindesalter stellt eine bedeutende Herausforderung dar, da sie unbehandelt zu schweren Komplikationen mit Sehverlust führen kann. Sie beginnt oft symptomarm, wird verzögert erkannt und birgt damit ein hohes Amblyopierisiko. Die Erkrankung wird in idiopathische und assoziierte Formen differenziert. Bei der juvenilen idiopathischen Arthritis wird sie als extraartikuläre Manifestation betrachtet. Zu den häufigsten systemischen Erkrankungen zählen Sarkoidose, TINU-, Behçet- und das Vogt-Koyanagi-Harada-Syndrom. Seltener – aber stets auszuschließen – ist eine infektiöse Genese. Auch im Kindesalter dominiert die anteriore Entzündungsform gefolgt von intermediärer, posteriorer und Panuveitis. Abhängig vom Schwerpunkt der Entzündung, von Leitbefunden und Zusatzuntersuchungen sollte eine differenzialdiagnostische Einordnung erfolgen, um eine individuell angepasste Therapie einzuleiten.
NON-INFECTIOUS POSTERIOR UVEITIDES - Atypicals, Variants, and Masquerades: the jungle of differential diagnosis.
Non-infectious posterior and panuveitides (NIPUs) comprise a heterogeneous group of inflammatory disorders of the outer retina and choroid, historically referred to as "white dot syndromes." Recent consensus efforts by the Multimodal Imaging in Uveitis (MUV) Task Force have established standardized diagnostic criteria for the major NIPUs, including multiple evanescent white dot syndrome (MEWDS), multifocal choroiditis and panuveitis/punctate inner choroiditis (MFCPU/PIC), acute posterior multifocal placoid pigment epitheliopathy (APMPPE), serpiginous choroiditis, and birdshot chorioretinopathy (BSCR). Nevertheless, a substantial proportion of cases deviate from classical presentations and fall into diagnostic "grey zones", blurring boundaries between diseases entities and complicating both differential diagnosis and management. This review aims to describe the broad spectrum of atypical, variant, and secondary forms of NIPUs as well as masquerade syndromes. Atypical MEWDS includes bilateral presentations or complicated courses, while MFCPU/PIC with outer retinal atrophy emerges as a notable entity with unclear therapeutic implications. Inflammatory reactions resembling both MEWDS and MFCPU/PIC may also occur as secondary phenomena, triggered by other chorioretinal disorders, most notably inherited retinal diseases (IRDs). Placoid chorioretinopathies, including APMPPE, persistent placoid maculopathy, serpiginous choroiditis, and relentless placoid chorioretinitis, are often distinguished only a posteriori based on disease course, but likely represent a continuum of disorders unified by choroidal ischemia. Atypical presentations of BSCR may feature extensive outer retinal damage, mimicking IRDs. Equally important is the consideration of masquerade syndromes in all suspected cases of NIPUs, as they can present with similar features yet require entirely different treatments. Infectious masquerades include tuberculosis-associated serpiginous-like choroiditis, acute syphilitic posterior placoid chorioretinopathy, and West Nile virus chorioretinitis, whereas vitreoretinal lymphoma is the most frequent neoplastic masquerade. In conclusion, integrating clinical context with high-quality multimodal imaging remains essential to navigate the jungle of differential diagnosis in NIPUs, while future studies should aim to link imaging phenotypes with immune and molecular biomarkers to refine classification and guide targeted therapies.
