Molecular mechanisms underlying Nocardia host interactions.
Nocardia species are opportunistic pathogens that cause localized and disseminated infections, particularly in immunocompromised individuals. Despite their clinical importance, the molecular mechanisms underlying Nocardia pathogenicity remain incompletely understood. This review summarizes current advances in Nocardia virulence factors, host immune responses, and intracellular survival strategies. A diverse array of virulence factors enables Nocardia to invade host cells, circumvent immune defenses, and maintain persistence within host tissues, including mammalian cell entry (Mce) proteins, antioxidant enzymes, phospholipase C, hemolysins, and siderophore-associated proteins. Host protection against Nocardia relies primarily on innate immune responses, with neutrophils playing a central role and being coordinated by γδT cells and interleukin-17-mediated signaling pathways. In addition, the clinical epidemiology of nocardiosis and animal models of Nocardia infection are also briefly summarized. However, most mechanistic studies remain restricted to a limited number of type strains. Further investigations into Nocardia-host interactions are essential for the development of improved diagnostic, therapeutic, and preventive strategies for nocardiosis.
Pulmonary infection by Nocardia saintgeorgesii mimicking lung cancer with concurrent pulmonary embolism in an immunocompetent host: a case highlighting the diagnostic role of mNGS.
Pulmonary nocardiosis presents a diagnostic challenge due to its frequent mimicry of lung cancer on imaging and the low sensitivity of conventional cultures. We report a case initially misdiagnosed as malignancy in an immunocompetent host, where metagenomic next-generation sequencing (mNGS) provided a definitive diagnosis and revealed a concurrent pulmonary embolism, suggesting a potential underrecognized association. This report describes a case of PN in an immunocompetent patient who was initially misdiagnosed with lung cancer based on imaging findings but later confirmed as pulmonary nocardiosis via mNGS. Notably, the patient also developed pulmonary embolism (PE). Empirical antibiotic therapy with piperacillin-tazobactam was initiated initially, supplemented with inhaled ipratropium bromide and expectorants to alleviate symptoms. Based on imaging findings suggestive of lung cancer, an invasive procedure was scheduled. mNGS was subsequently performed for further diagnosis. The subsequent results, along with CT scans, indicated no evidence of malignancy, leading to a consideration of Nocardia infection. The treatment regimen was then adjusted to ceftriaxone sodium combined with compound sulfamethoxazole, and the surgical schedule was canceled. The patient's condition showed significant improvement, and he was discharged without fever or dyspnea. Some literature suggests that many PN patients present with concurrent deep vein thrombosis (DVT), suggesting a potential yet underrecognized association between Nocardia infection and thrombotic events. However, this correlation has not been fully reported before.
Epidemiology, clinical treatment and outcomes, susceptibility patterns and genotypic analysis of 214 Nocardia strains from multiple centers in Henan Province.
This study aimed to investigate the epidemiology, clinical treatment and outcomes, antimicrobial resistance profiles and genotypic analysis of 214 Nocardia strains collected from 9 hospitals in Henan Province spanning 9 years. Through retrospective analysis of hospitalized patients with nocardiosis, the epidemiological characteristics of 214 Nocardia strains were elucidated. These isolates were identified and subjected to the broth microdilution method for the antimicrobial susceptibility profiles, and the resistance and virulence genes were determined using whole-genome sequencing (WGS). Of all strains, 74.8% were collected from lower respiratory tract specimens, and N. cyriacigeorgica was the most commonly isolated species (28%), followed by N. farcinica (24.8%), N. abscessus (7.9%), N. amamiensis (7.9%), N. otitidiscaviarum (7.5%). 93.9% were obtained from in-province patients, and Nanyang City (28.0%) was with the highest isolation rate in Henan Province. Simultaneously, all of the strains were susceptible to linezolid (LZD), and 99.1% susceptible to trimethoprim-sulfamethoxazole (TMP-SMX). The antibiotic resistance profiles of other antibiotics varied tremendously among different Nocardia species. Of all the patients, 108 (50.7%) received TMP-SMX monotherapy or multidrug regimen; moreover, 182 (85.5%) patients recovered after treatment. Notably, 12 antibiotic resistance genes and 11 virulence genes were identified, implicating the complexity of resistance and pathogenicity mechanisms. Meanwhile, the MDR rates for Nocardia species ranged from 68.8% in N. otitidiscaviarum to 17.7% in N. amamiensis. No strains exhibited the XDR and PDR phenotypes. This study provides a comprehensive evaluation of the epidemiology, phenotypic and genotypic profiles, and clinical treatment of Nocardia species in Henan, China. TMP-SMX and LZD can be used respectively for the clinical routine and critical treatment of nocardiosis. Particular emphasis is placed on the fact that antibiotic resistance and pathogenicity are species - specific, therefore, the AST of Nocardia isolates should be conducted and standardized, and attempts should be made to monitor its resistance molecular mechanisms.
Pulmonary nocardiosis masquerading as refractory haemoptysis in a bronchiectasis patient: the critical role of multidisciplinary management.
An immunocompetent male in his 30s with a history of bronchiectasis presented with recurrent haemoptysis (5-10 mL every 2 months) over 4 years. High-resolution CT revealed cystic bronchiectasis and ground-glass opacities. Bronchoscopy identified active bleeding from a ruptured bronchial artery, which was successfully managed with emergent embolisation. Bronchoalveolar lavage demonstrated weakly acid-fast branching filaments, later confirmed as Nocardia asteroides by matrix-assisted laser desorption-ionisation time of flight mass spectrometry. The patient responded well to trimethoprim-sulfamethoxazole and remained stable at follow-up.This case highlights three key points: (1) pulmonary nocardiosis should be considered in chronic haemoptysis, especially in patients with structural lung disease, even if immunocompetent; (2) rapid microbiological diagnosis is critical, particularly when acid-fast staining yields atypical results; and (3) bronchial artery embolisation can be lifesaving in cases of nocardial vasculitis with active bleeding. The report underscores the importance of multidisciplinary management in such complex presentations.
Disseminated nocardiosis involving lung, liver, and spleen in a patient with intestinal Behçet's disease under combined immunosuppressive therapy.
We report a rare case of disseminated nocardiosis in a woman in her 50 s with a 20-year history of intestinal Behçet's disease who had been receiving infliximab, azathioprine, and oral betamethasone. She developed fever and sore throat 6 weeks after infliximab administration. Laboratory findings revealed leukocytosis, elevated liver enzymes, and markedly increased C-reactive protein and procalcitonin. Contrast-enhanced computed tomography revealed multiple nodular lesions in both lungs and multiple abscesses in the liver and spleen. Blood cultures identified Nocardia species, and a diagnosis of disseminated nocardiosis was established. Initial treatment with trimethoprim-sulfamethoxazole was discontinued because of drug-induced pancreatitis. The regimen was switched to imipenem/cilastatin, followed by minocycline, resulting in complete abscess resolution. A literature review identified three similar cases with lung, liver, and splenic abscesses, all with significant T-cell-related immunosuppression. This case highlights the risk of disseminated Nocardia infection in patients receiving combined immunosuppressive therapy, including biologics, and highlights the importance of early diagnosis and adaptable antimicrobial management in immunocompromised hosts.
