Evaluation of Immune Risk Factors for Developing Complications After Bacterial Meningitis in Children.
This study investigates selected components of the immune system and the prevalence of single-nucleotide polymorphisms in genes involved in the immune response in children following bacterial meningitis, with particular focus on their potential role in the development of postinfectious complications. The study included 47 children hospitalized with pneumococcal or meningococcal meningitis between 2010 and 2020. Analyses involved leukocyte profiles, immunoglobulin levels (IgA, IgM, IgG), complement activity (C3, C4, CH50), and 11 single-nucleotide polymorphisms across 7 genes. Data on complications were collected from medical records, follow-ups, and patient surveys. Complications occurred in 59.6% of cases, with cognitive impairment, focal neurologic deficits, subdural effusion, and hearing disorders being most common. CH50 defects showed a statistical trend toward an increased risk of neurologic complications (P = .064), and the TIRAP rs8177374 single-nucleotide polymorphism demonstrated a borderline trend toward significance (P = .054). Bacterial meningitis may lead to long-term complications. Complement system disturbances and genetic predispositions are important prognostic factors influencing outcomes.
Nigeria's post-COVID-19 vaccine manufacturing ambitions: Opportunities and regional implications.
Global health emergencies consistently expose and exacerbate vaccine inequities, with high-income countries prioritising their populations and leaving low- and middle-income countries (LMICs) facing delays and shortages. Diseases primarily affecting LMICs receive limited attention from global vaccine developers because of perceived low market value and limited financial return, further entrenching these disparities. Africa's limited investment in vaccine manufacturing has heightened its vulnerability during outbreaks, including yellow fever, Ebola, meningococcal meningitis, mpox, and most recently, coronavirus disease 2019 (COVID-19). This study examines the barriers to vaccine equity in Africa and critically analyses Nigeria's post-COVID-19 efforts to establish a viable, local vaccine manufacturing ecosystem. The research focuses on Nigeria within the broader African vaccine manufacturing landscape, using it as a case study to explore both national and continental dynamics. The study draws on document analysis of strategic plans, regulatory reports, and partnership announcements, complemented by qualitative insights from key informant interviews with stakeholders involved in vaccine policy, regulation, and production in Nigeria and across West Africa. Post-pandemic momentum has catalysed significant shifts: Nigeria has developed a national vaccine manufacturing strategy and secured international partnerships and financing commitments. The regulatory authority, National Agency for Food and Drug Administration and Control (NAFDAC), achieved the World Health Organization (WHO) Maturity Level 3 status, marking critical progress. Nonetheless, persistent gaps remain in research and development capacity, workforce development, regulatory agility, and infrastructure readiness. While Nigeria has made notable progress since the COVID-19 pandemic, sustainable vaccine manufacturing requires long-term investment in research and development, policy reform, skills development, and regional cooperation. Failure to address these challenges systematically risks undermining current gains. This article provides insights to support ongoing and future investments in Nigeria's vaccine manufacturing sector, guiding government policy, international partnerships, and potential investors.
Epidemiology and characteristics of bacterial meningitis of children with cerebrospinal fluid leakage or cochlear implant.
Cerebrospinal (CSF) fluid leakage is a recognized risk factor for bacterial meningitis. Few data are published concerning bacterial meningitis in children with cerebrospinal fluid leakage, and the impact of 13 valent pneumococcal conjugate vaccination (PCV13) in this population is not well known. The aim of this study was to describe the epidemiology of bacterial meningitis in children with known CSF leakage.Methods: Among all bacterial meningitis in children > 3 months old, we analyzed those with known CSF leakage (meningeal breach and/or cochlear implant resulting in meningitis) in a nationwide prospective cohort between 2001 and 2024 in France, with 227 pediatric wards and 168 microbiology departments. Of 5,879 cases of bacterial meningitis in children over 3 months old, 251 (4.3%) were associated with known CSF leakage. In this population, Streptococcus pneumoniae (78.9%) was the most frequent bacteria involved, followed by Haemophilus influenzae (10%), mainly non typeable strains. Two cases of Neisseria meningitidis were recorded and only in children with cochlear implant. For children with known CSF leakage, in late PCV13 period, PCV13, PCV15, PCV20, PCV21 and PPSV23 serotypes accounted for 14%, 16.3%, 37.2%, 79.1% and 27.9%, respectively. In this population, serotype 23B was the most frequent (18.6%), followed by 11A (9.3%), 15A (9.3%), 19F (7%), 3 (4.7%) and 19A (2.3%). Bacterial meningitis in children with known CSF leakage is largely due to pneumococci and now mainly due to serotypes not included in PCV13. Only two cases of meningococcal meningitis were found and only for children with cochlear implant.
Estimating the impact of decreasing vaccination response times for outbreaks of vaccine-preventable diseases in low- and middle-income countries.
The 7-1-7 targets are gaining traction as measurable targets for assessing a country's readiness to detect and respond to outbreaks of infectious diseases. The targets are outbreak detection within 7 days of emergence, notification to health authorities within 1 day, and key early response actions commenced within another 7 days. For outbreaks of measles, cholera, yellow fever, and meningococcal meningitis, we estimated the impact of initiating outbreak response immunisation (ORI) within 15 days of outbreak emergence, relative to the mean ORI response time for each disease in low- and middle-income countries (LMICs) since 2000. Initiating ORI within 15 days of outbreak emergence aligns with the 7-1-7 targets and supports outbreak containment. Using agent-based models for four diseases, a status quo and series of 'Faster response' scenarios were compared for simulated outbreaks of each disease, with a 15-day ORI response time as the minimum. The Starsim modelling framework was used to build the models to provide a single common software and analysis architecture for all models while having the flexibility to account for very different modes of transmission across the diseases being modelled. The models were calibrated to epidemiological and programmatic response data for 51 measles, 40 cholera, 24 meningococcal meningitis, and 88 yellow fever outbreaks in LMICs. In a synthetic model population, a 15-day ORI response could avert: 80% of cases from cholera outbreaks relative to a historical response time of 105 days, 35% of cases from meningococcal meningitis outbreaks relative to a historical response time of 75 days, 0-35% of cases from yellow fever outbreaks relative to a historical response time of 105 days (depending on routine vaccine coverage and environmental suitability), and 0-55% of cases from measles outbreaks relative to a historical response time of 120 days (depending on routine vaccine coverage). Improvements made to ORI response time could reduce disease burden and decrease the risk of large outbreaks of vaccine-preventable diseases in LMICs. Efforts to improve ORI timeliness should be prioritised to higher risk settings, and it was clear that even a slow vaccination response was beneficial relative to no response at all.
High risk and low incidence diseases: Meningococcal disease.
Meningococcal disease is a serious condition that carries with it a high rate of morbidity and mortality. This review highlights the pearls and pitfalls of meningococcal disease, including presentation, diagnosis, and management in the emergency department (ED) based on current evidence. Invasive meningococcal disease is a severe, rapidly progressive infection caused by Neisseria meningitidis associated with high morbidity and mortality. Emergency physicians must maintain a high index of suspicion, as early symptoms may mimic a benign viral illness, potentially delaying treatment with serious consequences. This early phase is typically followed by the emergence of more specific features of meningococcal infection, such as meningococcal meningitis, meningococcemia, or a combination of both. Suspected cases warrant immediate implementation of droplet precautions and strict isolation to reduce the risk of healthcare worker exposure and secondary transmission. Prompt treatment, ideally within one hour of clinical suspicion, is essential. Management includes empiric parenteral antibiotics, and in cases of suspected meningitis, adjunctive corticosteroids should be administered prior to or concurrently with the first antibiotic dose. Patients with meningococcemia and shock require aggressive fluid resuscitation, vasopressor support as indicated, and prompt admission to a critical care setting. All close contacts of confirmed cases should receive chemoprophylaxis, regardless of their meningococcal immunization status. An understanding of meningococcal disease can assist emergency clinicians in diagnosing and managing this potentially deadly disease.
