Systematic evaluation of genetic polymorphisms in carcinogen metabolizing enzymes associated with the risk of oral potentially malignant disorders occurrence.
Accumulated studies investigate the association of single-nucleotide polymorphisms (SNPs) in carcinogen metabolizing enzymes with oral potentially malignant disorders (OPMD) risk. However, these results were inconsistent and conflicting. The purpose of this pooled analysis was to systematically evaluate the associations of SNPs in 6 enzymes (CYP1A1, CYP2E1, GSTM1, GSTM3, GSTT1 and GSTP1) with risk of OPMD occurrence. A systematic evaluation was performed to identify all eligible case ndash;control studies on the association between SNPs in 6 enzymes and OPMD onset. Odds ratios (ORs) and 95 % confidence intervals (CIs) were pooled to estimate association strength. A significant association of CYP2E1 PstI polymorphism with OPMD was found (OR, 1.46; 95%CI, 1.07-2.00) in 430 cases and 818 health controls. A significant association of GSTM1 null genotype with OPMD, especially oral leukoplakia, was found (OR, 1.72; 95%CI, 1.24-2.37) in 2228 cases and 4425 controls. A significant association of GSTT1 null genotype with OPMD, particularly oral submucous fibrosis, was found (OR, 1.50; 95%CI, 1.01-2.22) in 1798 cases and 3934 controls. A marginally significant association of GSTM3 polymorphism with OPMD was found (OR, 1.41; 95%CI, 1.00-1.98) in 321 cases and 622 controls. There was no significant association of polymorphisms in CYP1A1, CYP2E1 RsaI variant, and GSTP1 with OPMD. This analysis for the first time investigated SNPs in carcinogen metabolizing enzymes with OPMD, suggesting that polymorphisms in CYP2E1 PstI, GSTM1, GSTM3 and GSTT1 play roles in OPMD occurrence and highlighting their potential in risk stratification and early detection strategies.
Metformin alleviates the progression of oral submucous fibrosis through downregulation of metastasis associated lung adenocarcinoma transcript 1 (MALAT1).
/purpose: Emerging evidence demonstrates the anti-fibrotic properties of metformin, a first-line anti-diabetic drug; however, its inhibitory effects on oral fibrogenesis warrant further evaluation. The cytotoxic effects of metformin on normal and fibrotic buccal mucosal fibroblasts (fBMFs) derived from OSF tissues were evaluated using the MTT assay. Collagen gel contraction, wound healing, and transwell migration assays were carried out to assess myofibroblast features. In addition, the expression levels of alpha-1 type I collagen (COL1A1), alpha-smooth muscle actin (α-SMA) and Smad2 were measured. Moreover, RNA sequencing was performed to explore potential targets participating in the anti-fibrotic effects of metformin. A lower dose of metformin was sufficient to inhibit the proliferation of fBMFs without affecting normal BMFs, and to attenuate various myofibroblast characteristics, including collagen gel contraction, wound healing, transwell migration capacities, and the expression of fibrosis markers. Additionally, we showed that administration of metformin prevented the arecoline-induced myofibroblast activation. Most importantly, our results suggest that metformin may exert suppressive effects on myofibroblast activities by inhibiting ROS accumulation through the downregulation of MALAT1. These findings indicate that metformin may serve as a preventive agent against OSF progression.
Uncovering the mechanism of arecoline's effect on oral submucous fibrosis: integrating transcriptomics and in vitro and in vivo experiments.
This study aimed to elucidate the potential targets and molecular mechanisms underlying arecoline-induced oral submucous fibrosis through integrated transcriptomic profiling and experimental validation. Transcriptomic sequencing was first employed to identify key pathways and targets influenced by arecoline in rat oral mucosa and whole blood. Subsequently, in vitro experiments using human primary oral mucosal fibroblasts (hOMFs) were conducted to validate the molecular mechanisms. In vivo experiments demonstrated that chronic topical application of arecoline significantly reduced oral opening distance and induced histopathological features of oral submucous fibrosis (OSF), including epithelial atrophy, collagen deposition, and elevated TGF-β expression. Transcriptomic analysis revealed significant enrichment of pathways associated with fibrosis, including PPAR signaling, AMPK signaling, p53 signaling, and Hippo signaling pathways. In vitro validation further confirmed that arecoline dose-dependently upregulated α-SMA and Col1a1 expression, enhanced fibroblast proliferation, and activated Hippo pathway effectors (YAP/TAZ). These findings highlight the Hippo signaling pathway as a critical mediator of arecoline-induced OSF, providing novel insights for therapeutic targeting and mechanistic exploration in OSF management.
VDR polymorphisms and their influence on oral squamous cell carcinoma susceptibility: a systematic review.
Oral Squamous Cell Carcinoma (OSCC) arises through an intricate interplay of underlying genetic traits and surrounding environmental conditions. Variations in the vitamin D receptor (VDR) gene, particularly single-nucleotide polymorphisms (SNPs), may contribute to individual susceptibility and disease progression. To conduct a systematic review evaluating the role of VDR gene polymorphisms in influencing susceptibility to OSCC. A systematic literature search was performed in PubMed, Scopus, and Google Scholar to identify human case-control studies evaluating VDR SNPs in relation to OSCC. The methodological quality and potential bias of the selected studies were assessed using the Newcastle-Ottawa Scale (NOS). Due to substantial heterogeneity among the studies in terms of population characteristics, genotyping methods, and outcome measures, a meta-analysis was not performed, and the findings were summarized descriptively. From an initial pool of 90 records, five case-control studies met the inclusion criteria. Qualitative synthesis revealed consistent associations between polymorphisms in the VDR gene, particularly Fok1 (rs2228570), TaqI (rs731236), and ApaI (rs7975232), and in the CYP24A1 (rs2296241) gene, which encodes a VDR-regulated enzyme involved in the catabolism of active vitamin D metabolites. These variants have also been explored for their potential association with oral potentially malignant disorders (OPMDs), including leukoplakia, oral submucous fibrosis, and oral erythroplakia, which may undergo malignant transformation to OSCC. Most of the included studies demonstrated a low risk of bias and supported the role of these SNPs in OSCC susceptibility, as well as their potential prognostic relevance. VDR polymorphisms may contribute to OSCC risk by affecting immune and cellular pathways. Multimodal Serum Profiling for Early Detection and Risk Assessment of Oral Potentially Malignant Disorders and Oral Cancer.
Oral squamous cell carcinoma (OSCC) has high incidence rates in India, with poor survival rates due to late diagnosis. Oral potentially malignant disorders (OPMDs) such as leukoplakia (L) and oral submucous fibrosis (OSMF) present a critical window for intervention. Minimally invasive approaches capturing early biochemical alterations were investigated to improve early detection and stratification. Blood serum samples from 169 subjects, including healthy controls (C), tobacco habitués (TC), L, OSMF, and OSCC, were analyzed using Raman spectroscopy (RS) and liquid chromatography-tandem mass spectrometry (LC-MS/MS). Principal component-based quadratic discriminant analysis (PC-QDA) models were built with RS data and validated on independent cohorts. Multivariate curve resolution-alternating least-squares (MCR-ALS) analysis identified altered spectral features. Global LC-MS/MS metabolite profiles were assessed by multivariate statistics and pathway enrichment. RS-based PC-QDA models achieved ∼95% accuracy in training and testing when C subjects were compared against TC, L, OSCC, and OSMF in a two-group model. A three-group model with C, TC, and oral diseases accurately classified >80% C and 86% oral disease subjects. The model stratifying L, OSMF, and OSCC identified 100% OSCC and 73-84% of L and OSMF in independent test sets. MCR-ALS revealed spectral features of albumin, immunoglobulins, carotenoids, and lipids, corresponding to LC-MS/MS findings of altered albumin-bound metabolites, bile acids, lipid metabolism, and oxidative stress. Serum RS demonstrated efficacy as a rapid, minimally invasive detection tool for oral disease stratification. LC-MS/MS identified metabolites and pathways aligning with RS spectral signatures. This multimodal approach shows promise for early detection and risk assessment of oral cancer.
