Tsutsugamushi Hepatopathy: A Scoping Review.
Scrub typhus, caused by Orientia tsutsugamushi, is a significant public health concern, particularly in endemic regions of Asia. This mite-borne disease, transmitted by chiggers, presents with varied clinical manifestations, ranging from mild symptoms to severe complications, including multi-organ failure. Hepatic dysfunction is a common and often underappreciated clinical feature in scrub typhus patients, with recent studies highlighting its frequent occurrence. This dysfunction is characterized by elevated liver enzymes, jaundice, and, in severe cases, fulminant hepatic failure, often mimicking acute viral hepatitis. The pathophysiology of hepatic dysfunction is multifactorial, involving direct bacterial invasion, immune-mediated damage, and systemic inflammatory responses. Key mechanisms include focal or disseminated vasculitis and perivasculitis, endothelial dysfunction, and potential direct cytopathic effects on hepatocytes. Dysregulated cellular immunity, marked by a strong Th1 response, also contributes to tissue damage. Additionally, sepsis can lead to ischemic hepatitis and further hepatic dysfunction. A scoping review of 63 studies, predominantly from Asia, revealed varying incidences of hepatomegaly (2.5%-98.5%), jaundice (1.4%-36.5%), and abnormal transaminase levels (27.5%-100%). Hepatic dysfunction is typically hepatocellular, with AST often more elevated than ALT. While acute liver failure is rare, it is reported mostly in pediatric patients. Hepatic dysfunction correlates with overall disease severity and is associated with increased risks of delayed defervescence, prolonged hospitalization, other organ failures, and mortality in both adult and pediatric populations. Given its impact on patient outcomes, early recognition of hepatic involvement is crucial for effective management.
Differentiating scrub typhus meningitis from brucellar meningitis in children: A single-center retrospective study.
To investigate the clinical distinctions between scrub typhus meningitis and brucellar meningitis in children, and to identify potential biomarkers with early differential diagnostic value to support clinical decision-making. A retrospective analysis was conducted on 13 pediatric patients diagnosed with brucellar meningitis admitted to Kunming Children's Hospital over the past decade. Thirteen cases of scrub typhus meningitis were selected as controls using an age- and sex-matching strategy. The clinical manifestations, laboratory findings, and cerebrospinal fluid (CSF) characteristics were compared between the two groups. Receiver operating characteristic (ROC) curve was employed to assess the diagnostic performance and clinical utility of key biomarkers. The pre-admission fever and duration of fever were significantly shorter in the scrub typhus meningitis group. Laboratory evaluation revealed that serum ferritin, procalcitonin (PCT), C-reactive protein (CRP), and creatinine (Cr) levels were markedly higher in the scrub typhus group compared with the brucellosis group. No statistically significant differences were observed in CSF biochemical parameters. ROC analysis demonstrated that ferritin (AUC = 0.870) and PCT (AUC = 0.846) exhibited the greatest diagnostic accuracy, followed by CRP (AUC = 0.814) and Cr (AUC = 0.799). All patients achieved complete clinical recovery following standardized treatment, with no recurrences or fatalities. Although scrub typhus meningitis and brucellar meningitis share considerable clinical overlap in children, serum ferritin and PCT levels may represent potential diagnostic signals for early differential diagnosis, warranting validation in larger prospective cohorts. High Ferritin levels or PCT levels may provide preliminary clues toward scrub typhus meningitis. Early recognition and targeted antimicrobial therapy are associated with favorable prognostic outcomes.
Scrub and murine typhus seroprevalence among blood donors in Laos.
Scrub typhus and murine typhus, caused by Orientia tsutsugamushi and Rickettsia typhi, respectively, are important causes of febrile illness in Laos. Although several studies have assessed rickettsial infection in selected provinces, the nationwide distribution remains unclear. This study aimed to estimate exposure to scrub typhus group (STG) and typhus group (TG) across twelve provinces of Laos and identify potential hotspots. We screened 1,200 serum samples from blood donors (100 per province) for STG and TG IgG antibodies using enzyme-linked immunosorbent assays (ELISA). Samples with optical density ≥ 0.5 were confirmed by immunofluorescence assays (IFA). Overall seroprevalence was 7.26% (95%CI:5.93-8.87) for STG and 4.09% (95% CI:3.11-5.37) for TG. STG seroprevalence was highest in Huaphan (27%), Oudomxay (19%) and Xiangkhuang (17%), all in northern Laos. TG seroprevalence was 10% in both Oudomxay (north) and Attapue (south). Compared with Vientiane Capital, these provinces had significantly higher seropositivity. No significant association was observed with age group. STG seropositivity was higher in males, whereas TG seropositivity was higher in females. Rickettsial infections are widespread in Laos, with STG seroprevalence concentrated in the north and TG seroprevalence present in both the north and south, guiding future research priorities and informing targeted public health interventions.
Establishing an external quality assurance scheme for the detection of Orientia tsutsugamushi IgM in clinical samples: Strengthening quality control of scrub typhus diagnosis in Indian laboratories.
Scrub typhus, caused by Orientia tsutsugamushi, is a re-emerging zoonotic disease that is associated with significant morbidity and mortality in many parts of India. Early and accurate diagnosis, primarily through the detection of IgM antibodies, plays a pivotal role in patient management and disease surveillance. However, there is a paucity of diagnostic methods in our country, and only in recent years have more diagnostic assays been rolled out. To address the heterogeneity in the laboratory diagnosis of scrub typhus, an external quality assurance scheme (EQA Scheme) for the detection of O tsutsugamushi IgM (OTM) was conceptualized and implemented across a network of laboratories in India. This study provides detailed insight into the design, implementation, and outcomes of this EQA scheme initiative. Initially, a pilot phase was conducted among the 10 participant laboratories, and a total of 51 Virus Research and Diagnostic Laboratories (VRDLs) under DHR-ICMR undertaking OTM serology subsequently participated in the EQA Scheme. Proficiency testing (PT) panels consisting of 5 well-characterized and validated pooled serum samples were distributed to the participants for testing. Participating laboratories have employed various IgM detection methods, including ELISA and rapid immunochromatographic tests (ICTs). Performance was evaluated qualitatively based on the testing results and adherence to reporting timelines. Most laboratories demonstrated concordant results (n = 47) in OTM testing, while 4% of laboratories (n = 2) reported discordant results. The study underscores the vital role of an EQA scheme in ensuring quality-assured diagnostic testing for infectious diseases in the country and builds confidence in Indian laboratory results. Regular participation in such schemes, coupled with targeted capacity-building initiatives, can substantially improve laboratory diagnostics, facilitate timely clinical interventions, and contribute to a more robust national surveillance system. This model may serve as a foundation for strengthening quality assurance frameworks for other emerging infectious diseases in India and similar low- and middle-income settings.
Outcomes of noninvasive versus invasive respiratory support in pediatric scrub typhus-associated acute respiratory failure: a retrospective cohort study from South India.
Scrub typhus is an important cause of acute febrile illness in children that may progress to acute respiratory failure (ARF). However, evidence for noninvasive respiratory support (NIRS) in this setting is limited. The main objective of this study is to compare outcomes of NIRS versus invasive mechanical ventilation (IMV) in pediatric scrub typhus-related ARF and identify predictors of NIV failure. This study included children aged 28 days-15 years with confirmed scrub typhus and ARF who were admitted to the pediatric critical care unit of a tertiary center in South India. Patients were classified according to the type of respiratory support in the first 4 h (NIRS: heated humidified high-flow nasal cannula [HHHFNC] and/or noninvasive ventilation [NIV]; or IMV). Clinical and laboratory data and outcomes were analyzed. The predictors of NIV failure were evaluated in a subgroup of NIV recipients. Of the 160 children, 46 (28.8%) received HHHFNC, 52 (32.5%) NIV, and 62 (38.7%) IMV as initial support. Eighteen NIV patients (34.6%) required intubation and were analysed with the IMV group (IMV n=80, NIRS n=80). The overall survival was 67.5%, higher with NIRS than IMV (96.3% vs 38.7%, P<.001). NIV failure occurred in 22.5% of patients and was associated with a mortality rate of 66.7%. Younger age, male sex, facial puffiness, hepatomegaly, thrombocytopenia, elevated lactate levels, and higher SOFA, PIM2, and VIS scores predicted NIV failure (P<.05). The duration of respiratory support, ICU stay, and inotrope use were greater with IMV. NIRS is effective and is associated with better survival in children with scrub typhus ARDS. Vigilant monitoring and early escalation in high-risk patients may improve outcomes.
