Impact of a Structured Brain Care Bundle on Severe Germinal Matrix Hemorrhage-Intraventricular Hemorrhage in Very Preterm Neonates: A Retrospective Cohort Study.
Germinal matrix hemorrhage-intraventricular hemorrhage (GMH-IVH) remains a leading cause of neurologic morbidity and mortality in very preterm infants. Evidence supports the assertion that standardized neuroprotective "Brain Care Bundles" (BCBs) can reduce GMH-IVH risk, but data from Middle Eastern Level III neonatal intensive care units are limited. We performed a retrospective cohort study comparing pre-BCBs (January-December 2021; n = 80) versus post-BCB (January-December 2022; n = 77) cohorts of inborn infants ≤30 weeks' gestation and or birth weight ≤1500 g. The ten-component BCBs included antenatal steroids, magnesium sulfate, delayed cord clamping, thermoregulation, midline head positioning, minimal handling, permissive hypercapnia, targeted oxygen saturation, hemodynamic thresholds, and neurodevelopmental support. The primary outcome was rate of severe GMH-IVH (Papile grade III-periventricular hemorrhagic infarction) on cranial ultrasound after 72 hours. Secondary outcomes included any-grade GMH-IVH, post-hemorrhagic hydrocephalus, necrotizing enterocolitis (≥stage 2), late-onset sepsis, bronchopulmonary dysplasia, retinopathy of prematurity (≥stage 3), and mortality. Baseline demographics were comparable except for a higher male proportion in the pre-BCBs cohort (63.7% vs 44.0%; P = 0.017). Delayed cord clamping and completion of antenatal steroids improved significantly post-BCBs (P < 0.05). Severe GMH-IVH incidence declined from 15% to 5% (AR = 10.2%; P = 0.045); BCBs implementation remained independently protective in multivariable analysis (adjusted odds ratio = 0.52; 95% confidence interval: 0.27-0.99; P = 0.048). Late-onset sepsis decreased markedly (P = 0.002). No significant differences were found in rates of necrotizing enterocolitis, bronchopulmonary dysplasia, retinopathy of prematurity, hydrocephalus, or mortality. Implementation of the BCBs in a high-risk Middle Eastern neonatal intensive care unit was associated with significantly reduced severe GMH-IVH and late-onset sepsis, along with better antenatal steroid adherence. These results support adopting neuroprotective BCBs as a quality improvement strategy in vulnerable preterm populations.
Urinary biomarkers and serum VEGF in preterm infants with retinopathy of prematurity treated with ranibizumab.
Intravitreal injection of anti-vascular endothelial growth factor (VEGF) antibodies is widely used to treat retinopathy of prematurity (ROP), yet, its systemic effects remain unclear. In adults, intravitreal anti-VEGF therapy has been associated with proteinuria and acute kidney injury. This study aimed to evaluate the renal effects of anti-VEGF antibody (Ranibizumab) by longitudinally measuring urinary biomarkers and serum VEGF levels in preterm infants with ROP. Preterm infants were categorized into two groups based on ROP treatment status: those receiving intravitreal ranibizumab (IVR) (treated group) and age-matched controls without treatment (nontreated group). Urinary biomarkers, including angiotensinogen, liver-type fatty acid-binding protein (L-FABP), albumin, and podocalyxin, were assessed at three time points: before treatment, and at one and 2 months post-IVR. Serum VEGF levels were measured 1 month post-IVR. The study included 30 infants in the treated group and 12 in the nontreated group. The mean gestational age and birth weight were 24.9 weeks and 664 g in the treated group, and 26.8 weeks and 845.6 g in the nontreated group. The mean postnatal age at treatment was 68.8 days, with a postmenstrual age of 34.8 weeks. No significant differences were observed in urinary biomarker levels between the groups at one and 2 months posttreatment. Serum VEGF levels were higher in the treated group than in the nontreated group after IVR. IVR treatment did not show the sustained renal injury or suppress serum VEGF levels in preterm infants with ROP.
Risk factor ranking of retinopathy of prematurity using classification and regression tree analysis.
Numerous risk factors for ROP have been identified, with gestational age and birth weight being the most established. Ongoing improvements in neonatal care may change the critical thresholds for these factors associated with ROP development. We applied a classification and regression tree (CART) analysis to predict the likelihood of developing ROP using a contemporary database. This cross-sectional study included 1296 premature infants who completed ROP screening by pediatric ophthalmologists from a children's hospital. Relevant infant and maternal parameters were obtained from medical records. CART analysis with the recursive partitioning package was used to predict ROP incidence, treatment need, and plus disease. The dataset was randomized, with 80% for training and 20% reserved for testing the model's accuracy. A 10-fold cross-validation was conducted to assess the model's generalizability. The mean gestational age was 27.7 ± 2.5 weeks, and the birth weight was 1017 ± 346.9 g; 538 infants developed ROP. The CART analysis produced a predictive model for ROP incidence with a mean absolute error of 0.25. The critical thresholds identified for ROP development were gestational age <27.2 weeks and birth weight <775 g. Two models for ROP progression showed that age <25 weeks and weight <569 g were linked to the need for ROP treatment, and age <24 weeks and non-Black maternal race were associated with plus disease. Our CART analysis provides updated clinical thresholds for ROP development and progression in a modern U.S. preterm population. Prospective validation across diverse populations will further enhance the model's utility and generalizability.
Quantitative Assessment of the Extent of Retinal Vascularization in Retinopathy of Prematurity.
To evaluate pixel-based measurements of the minimum nasal and temporal extent of retinal vascularization (NERV and TERV) from RetCam images in retinopathy of prematurity (ROP). Retrospective, multicenter diagnostic accuracy study. Infants screened for ROP at 8 high-volume centers in Imaging and Informatics in ROP (i-ROP) consortium between July 2011 and December 2016. Widefield RetCam3 temporal and nasal fundus images showing the optic disc and vascular-avascular junction were selected. A reference standard diagnosis for zone, stage, and plus disease was assigned based on indirect ophthalmoscopy and expert panel review. Each eye was also assigned an artificial intelligence-derived vascular severity score (VSS) (scale 1-9). Manual stage segmentation was performed for each image, combined with automated optic disc localization, and the Euclidean distance from the disc center to the stage was calculated in pixels to derive the NERV and TERV. Intergrader reproducibility was assessed using intraclass correlation coefficient. Minimum NERV and TERV (pixels), comparison of NERV and TERV between zone I and zone II, nasal-temporal asymmetry, area under the receiver operating characteristic curve for zone I diagnosis, and relationship between VSS, stage, and NERV/TERV. A total of 1832 images from 605 eyes of 350 infants met the inclusion criteria. Minimum NERV was shorter than minimum TERV in 98.3% of eyes, being a mean of 20.9% ± 7.9% lower. Both minimum NERV and TERV were significantly lower in zone I (276.5 ± 43.2; 345.0 ± 53.4 pixels) than zone II (360.8 ± 44.5; 452.6 ± 52.5 pixels; P < 0.0001). Area under the receiver operating characteristic curve for zone I was 0.91/0.90 for minimum NERV/TERV, respectively. The VSS was higher in eyes with a higher stage for a given extent of retinal vascularization (ERV) and higher within a given stage for eyes with a lower ERV. The intergrader reproducibility of ERV was excellent (intraclass correlation coefficient = 0.985, 95% confidence interval: 0.977 to 0.992). Objective measurement of the ERV in eyes with ROP demonstrates that the NERV is nearly always less than the TERV. These findings have implications for ROP diagnosis and classification. Understanding the Molecular Basis of Pathological Retinal Angiogenesis: Roles of Lipids, Non-Coding RNAs, and Cellular Crosstalk.
Pathological retinal neovascularization is a prevalent cause of blindness, impacting millions of individuals across all age groups, from infants to older adults. It is a key clinical feature of several retinal diseases, such as diabetic retinopathy (DR), retinopathy of prematurity (ROP), age-related macular degeneration (AMD), and Coats disease. Researchers have extensively investigated the involvement of vascular endothelial growth factor (VEGF) signaling, hypoxia inducible factor (HIF) signaling, and inflammation in the regulation of pathological retinal neovascularization. Recent breakthroughs have demonstrated the influence of several substances and cells in the retina on the regulation of pathological retinal neovascularization. This review emphasizes the rising significance of substances such as lipid mediators, non-coding ribonucleic acids, as well as the involvement of several retinal cell types, including endothelial cells (ECs), pericytes, glial cells, and the retinal pigment epithelium (RPE) in the regulation of pathological neovascularization. Here, we emphasize the interactions among these cell types in maintaining retinal homeostasis and their role in abnormal neovascularization in diseases like DR, AMD, ROP, etc. This review aims to highlight the importance of molecules and cells that go beyond the VEGF-centric therapeutic focus to treat pathological neovascularization.
