Louse-borne relapsing fever in Ethiopia: an urgent call for WHO recognition as a neglected tropical disease.
Louse-borne relapsing fever (LBRF), caused by Borrelia recurrentis and transmitted by the human body louse, remains a persistent health emergency in Ethiopia, resulting in preventable mortality among young adults. Its continued endemicity reflects systemic failures and amplifies the syndromic effects of poverty, displacement and social stigma-locally termed 'Qmalam'-which deters care-seeking. Clinical management is further complicated by the potentially fatal Jarisch-Herxheimer reaction following antibiotic treatment. Despite these severe consequences, LBRF remains off the official WHO list of neglected tropical diseases (NTDs), hindering the mobilisation of essential resources and political will. LBRF unequivocally meets WHO criteria for NTD designation, a critical step necessary to catalyse the research, funding and coordinated action required for its elimination. In this viewpoint, we present an integrated framework for addressing transmission, treatment and prediction and issue an urgent call for formal WHO recognition.
Borrelia Infections Under B Cell-Depleting Therapies: A Systematic Review of Diagnostic Challenges and Outcomes With Special Focus on Neurological Forms.
B-cell-depleting therapies such as rituximab and newer anti-CD20 agents may impair humoral immune responses and reduce the reliability of serological testing. This systematic review aims to summarize reported cases of Lyme borreliosis and relapsing fever (RF) Borrelia infections in patients receiving B-cell-depleting therapies, focusing on clinical manifestations, diagnostic challenges, and treatment outcomes. A systematic literature search was conducted using PubMed and the Web of Science Core Collection employing search terms linking B-cell-depleting therapies to Lyme borreliosis and to RF Borrelia infections. The most reported cases in the literature were neurological infections due to Borrelia; there were 11 cases of Lyme neuroborreliosis and 8 cases of neurological infections due to B. miyamotoi reported. Lyme neuroborreliosis cases all exhibited negative serology. Pleocytosis of cerebrospinal fluid (CSF) was, however, always present, and PCR could confirm the diagnosis in 8 cases. Diagnosis in all B. miyamotoi cases relied exclusively on direct detection methods. All patients responded to standard antibiotic regimens, although persistent symptoms were reported in some cases. The other infections were 7 erythema migrans (EM), 4 disseminated Lyme borreliosis, and 3 cases of relapsing fever. Neurological Borrelia infections may be underrecognized in patients on B cell-depleting therapies due to atypical presentations and negative serologies. Early consideration of direct diagnostic methods such as PCR or indirect methods such as CSF CXCL13 levels is critical. Neurologists should maintain a high index of suspicion for Borrelia infections in immunocompromised patients presenting with CSF pleocytosis and neurological symptoms.
Comparative analysis of Borrelia's Defence mechanisms and their impact on genetic manipulation of low-passage isolates of Borrelia afzelii and Borrelia garinii.
Borrelia, a highly prevalent tick-borne pathogen, has a genome with a linear chromosome and numerous linear and circular plasmids. There are three groups of Borrelia: Lyme borreliosis, relapsing fever, and Echidna-reptile. In Europe, Borrelia afzelii and Borrelia garinii are the main causative agents of Lyme borreliosis. The primary defence mechanism of bacteria against bacteriophages and other invading DNA elements is the restriction-modification system (RMS), which discriminates between native and foreign DNA based on their distinct methylation patterns. This present study compares the RMS of all the Borrelia species available in the REBASE database. Additionally, it investigates the effect of the RMS on the transformation efficiency of low-passage B. afzelii and B. garinii isolates. Upon comparing the RMS of 18 Borrelia species, differences in the number, location and characteristics of genes were observed between groups. Given that Lyme borreliosis species exhibit higher genomic plasticity, we hypothesise that they possess a greater number of RMS genes to ensure functionality of the RMS even if some plasmids are lost. In this study, we demonstrate a large increase in transformation efficiency of low-passage strains by using an in vitro methylated shuttle vector, confirming our hypothesis that the RMS of Borrelia recognises pre-methylated vectors as native DNA. The knowledge gained in this study contributes to the understanding of Borrelia defence mechanisms and provides possible explanations for the relatively low transformation efficiency observed in previous studies. Consequently, in vitro methylation can serve as a valuable tool for facilitating studies involving genetic manipulation of Borrelia.

In this narrative, I illustrate the power of long-term primary care, knowing the patient and the land, and recognizing patterns others might miss. A teenage patient presented recurrent fevers and vague symptoms. What first appeared to be a viral illness ultimately revealed itself as tick-borne relapsing fever-"Cave Fever"-a rare but locally known condition. In a setting where relationships matter as much as laboratory results, diagnosis becomes an act of attentive presence. Sometimes, as Osler reminds us, the patient truly is telling us the diagnosis.
