Gastric bronchogenic cyst pooled case analysis: a case report and systematic review.
Gastric bronchogenic cyst constitutes a rare form of ectopic bronchogenic cyst, with an estimated incidence of less than 1 in 68,000 to 1 in 42,000. This study reported a case who was preoperatively misdiagnosed as gastrointestinal stromal tumor and reviewed the literature on gastric bronchogenic cyst to summarize its clinical features, diagnosis, treatment, pathological manifestations, and prognosis. A systematic search of the literature in the PubMed, Embase, Cochrane Library, Web of Science and Scopus databases was conducted by two independent reviewers focusing on cases of gastric bronchogenic cyst. Firstly, we report a rare case of bronchogenic cyst in a middle-aged female, with the lesion located near the gastric fundus. After literature review, 48 cases of gastric bronchogenic cyst (male-to-female ratio of 1:1.67, mean age was 47.54 ± 15.83 years) derived from 46 studies were enrolled. Abdominal pain (45.8%) was the most prevalent symptom, whereas 15 patients (31.3%) remained asymptomatic. Gastric bronchogenic cysts predominantly affected the gastric fundus (33.3%) and cardia (27.1%). Gastrointestinal stromal tumor (37.5%) was the most frequent misdiagnosis, while 9 patients (18.8%) were misdiagnosed as cysts originating from other abdominal organs. Laparoscopic surgery was the primary treatment (50.0%), followed by laparotomy (35.4%). Median follow-up was 10 months, no postoperative recurrence or mortality was reported in the pooled cases. Gastric bronchogenic cyst is a rare and diagnostically challenging congenital abnormality with limited evidence to guide preoperative diagnosis. It should be incorporated into the differential diagnosis of gastrointestinal neoplasms, surgical resection is regarded as the effective approach for definitive diagnosis treatment.
Beyond rhythm disorder: a rare case report of a young patient with mediastinal bronchogenic cyst.
Mediastinal bronchogenic cysts are sporadic congenital tumors, accounting for approximately 10%-15% of all mediastinal masses. In most cases, they remain asymptomatic. We present a case of a young man with new-onset atrial fibrillation caused by extra cardiac compression of the left atrium by mediastinal bronchogenic cyst. We report the first documented case in our country of a patient with a mediastinal bronchogenic cyst presenting with cardiac symptoms. A 39-year-old man was admitted to the emergency department with palpitations, dyspnea, and tachycardia that developed after physical activity. Electrocardiography demonstrated the first episode of atrial fibrillation. Echocardiography revealed an oval echolucent mass compressing the left atrium. Cardiac magnetic resonance imaging showed a massive mediastinal bronchogenic cyst causing compression of the left atrium, right inferior pulmonary vein, and esophagus, which was displaced to the left side. Robot-assisted thoracoscopic surgery was planned and performed successfully. Although typically asymptomatic, bronchogenic cysts can acutely present with life-threatening complications, particularly when they compress important cardiovascular structures. This case highlights the important role of comprehensive evaluation in young patients. It is crucial to consider extra cardiac causes in new-onset atrial fibrillation, especially in young adults. Fast and accurate diagnosis is essential to prevent further complications, with management extending beyond restoring sinus rhythm.
Bimanual Use of Articulating Laparoscopic Instruments (ArtiSential) in Pediatric Surgery: A Report of Three Cases.
Articulating laparoscopic instruments such as ArtiSential have been developed to overcome the limited range of motion associated with conventional straight instruments, particularly in narrow operative fields. While robot-assisted surgery provides highly precise manipulation, its introduction in pediatric surgery remains limited due to high cost and restricted insurance coverage. We introduced bimanual use of ArtiSential in pediatric minimally invasive surgery as a potential alternative to robotic assistance and evaluated its feasibility and technical advantages. We performed laparoscopic procedures using bimanual use of ArtiSential instruments in 3 pediatric patients: 2 single-incision laparoscopic appendectomies for chronic appendicitis (10- and 15-year-old patients) and 1 laparoscopic resection of an esophageal bronchogenic cyst with Dor fundoplication (11-year-old patient). In all cases, the procedures were safely completed without intraoperative complications, and blood loss was minimal. The articulating function allowed stable triangulation and fine manipulation even in restricted spaces such as the umbilical single-port approach and the esophagogastric junction. All patients were discharged between postoperative days 3 and 4 with uneventful recovery. Bimanual use of ArtiSential instruments enabled precise dissection and suturing in narrow pediatric operative fields, suggesting that this approach may serve as a practical, lower-cost alternative to robotic surgery where robotic systems are unavailable or not indicated. Further accumulation of cases is required to assess the learning curve, indications, and long-term outcomes.
Bronchogenic cyst of pancreas: a rare case report.
Bronchogenic cyst is a rare congenital developmental abnormality of the anterior bowel. Peri-pancreatic bronchogenic cyst (PBC) is extremely rare, when only a few cases reported worldwide. Diagnosis of PBC is difficult due to the lack of specific clinical features and symptoms, laboratory and imaging findings. Currently, the diagnosis of PBC mainly relies on the comprehensive judgment of multidisciplinary collaboration, combined with clinical manifestations, imaging features and pathological verification. Most bronchogenic cysts are benign and the long-term survival rate after complete resection is close to 100%. A 21-year-old male with a pancreatic mass was admitted to our hospital. CT scan showed a low-density cystic shadow about 3.6 × 3.5 cm2 on the pancreatic body, with clear boundaries, no enhancement, and no dilation of the main pancreatic duct. The patient underwent open distal pancreatectomy. Postoperative histopathological findings allowed for a definitive diagnosis of PBC. There was no recurrence for two years postoperatively. In this study, we reported a rare case of PBC. At present, radical surgical resection remains the most effective treatment for bronchogenic cyst with mass effects, infection, bleeding, rupture, or larger than 3 cm in diameter. Further research on the diagnosis and treatment strategies of PBC is required.
A Rare Case of an Abdominal Pedunculated Bronchogenic Cyst.
Bronchogenic cysts (BC) are rare congenital malformations that result from abnormal budding of the primitive foregut. Most of these cysts are located in the mediastinum or the lungs. Rarely, abdominal BCs result from embryonic budding migrating below the diaphragm prior to the fusion of the pleuroperitoneal membranes. We present a case of a 34-year-old male patient undergoing evaluation for bronchopneumopathy. Preoperative imaging revealed a well-circumscribed cystic lesion adjacent to the colon, which failed to provide a definitive diagnosis due to its atypical location and nonspecific radiological features. The patient underwent laparoscopic exploration. During the intervention, we discovered a BC connected to the oesophagus. Histopathological analysis confirmed the diagnosis of a BC, demonstrating ciliated pseudostratified epithelium and bronchial wall elements. Follow-up examinations confirmed an unremarkable post-operative course and total recovery. BC are most commonly identified during early childhood and are typically located in the mediastinum or lungs. They should also be considered in the differential diagnosis of incidentally discovered abdominal cystic lesions. There is no consensus on the standard of care; however, surgery provides favorable clinical results and a low risk of morbidity. Our case illustrates that laparoscopic removal is a safe and feasible option when malignancy has been carefully excluded.
