A hepatite B é uma doença infecciosa que afeta o fígado causada pelo vírus da hepatite B (VHB). O vírus pode causar infecções tanto agudas como crônicas. Durante a infeção inicial aguda muitas pessoas não manifestam sintomas. Em algumas pessoas os sintomas manifestam-se de forma súbita e incluem vômitos, pele amarela, fadiga, urina de cor escura e dor abdominal. Os sintomas podem demorar entre 30 a 180 dias até se manifestarem e geralmente manifestam-se ao longo de várias semanas. Só muito raramente é que a infecção inicial causa morte.

Role of Hydroxychloroquine in Prevention of Congenital Heart Block in Pregnant Women With Positive Anti-Ro/SSA and/or Anti-La/SSB.
Neonatal lupus, a condition primarily affecting infants born to mothers with anti-Ro/SSA and/or anti-La/SSB antibodies, is linked to congenital heart block (CHB), a potentially life-threatening complication. Hydroxychloroquine (HCQ), an antimalarial with immunomodulatory properties, has been shown to reduce CHB recurrence. This review examines current evidence on HCQ's role in preventing CHB in pregnancies affected by those autoantibodies, focusing on overall, primary and secondary prevention. Evidence supports HCQ in overall and secondary prevention, but data on its effectiveness for primary prevention is lacking. Clinical guidelines, antibody titre thresholds, foetal heart rate monitoring and the need for randomised controlled trials are discussed.
A region-wide hub and spoke approach to standardize and decentralize type 1 diabetes management.
Type 1 diabetes (T1D) requires specialized, multidisciplinary, and technology-driven care, which can be difficult to guarantee across regions with fragmented services. In July 2023, the Liguria region (Italy) reorganized pediatric diabetes care into a Hub and Spoke model, coordinated by the Gaslini Institute (hub) and three peripheral pediatric departments (spokes). This study aimed to evaluate the first year of application of this model in terms of clinical, technological, and environmental outcomes. This retrospective observational study included T1D patients aged less than 35 years in follow-up at the new regional Hub and Spoke system. Patients were categorized as transferred (previously followed at the hub), acquired (previously followed at a local center), and new onset (followed from the beginning at the Hub and Spoke system). The primary outcome was the change in time in range 70-180 mg/dL after 12 months. Secondary outcomes were additional indicators of glycemic control (CGM metrics), adoption of technologies (rtCGM and AID), and economic and environmental impact (travel distance, costs, and CO₂ emissions). 129 patients were referred to the new Hub and Spoke system during the first 2 years of its implementation. Among them, 88 (50 transferred, 38 acquired) had available data at baseline and completed the 1-year follow up period and were included in the study. In this cohort, TIR increased from 57.2 to 65.1% and TITR from 35.1 to 41.7% (p < 0.001), with improvement in other CGM metrics. Improvements were more pronounced in acquired patients, whose TIR rose from 53.9 to 65.7% (+11.2%, p < 0.001). AID use increased significantly with a larger rise among acquired (from 26.3 to 60.5%, +34.2%) compared with transferred patients (+26%). The program generated environmental and economic benefits, with estimated 70,192 km of travel, 11,932 kg CO₂, and € 25,760 saved yearly. The Hub and Spoke model proved feasible and effective, leading to more standardized care delivery, broader adoption of diabetes technologies, improved glycemic control, and significant reductions in travel-related costs and environmental impact. This approach may offer a scalable and sustainable solution for managing T1D, particularly in regions with complex geography and variable healthcare infrastructures.
Hydroxychloroquine therapy for women with both recurrent pregnancy loss and autoimmune disease: association with pregnancy outcomes and maternal-fetal complications.
To evaluate the impact of hydroxychloroquine (HCQ) on pregnancy outcomes and maternal and fetal complications in women with a history of recurrent pregnancy loss (RPL) and autoimmune disease. The medical records of pregnant women with a history of RPL and autoimmune disease attending the Shanghai First Maternity and Infant Hospital between January 2017 and December 2019 were retrospectively reviewed. Primary outcomes were gestational week at delivery (term: ≥ 37 weeks; preterm: < 37 weeks). Secondary outcomes were mid-to-late pregnancy loss and maternal and fetal complications, defined in accordance with national and international guidelines. 103 (20.9%) patients were exposed to HCQ (0.1g twice daily), 389 (79.1%) were not. There were 436 full-term births, 48 preterm births, 5 late-term miscarriages, 2 stillbirths, and 1 fetal malformation-induced abortion. Univariable/multivariable analyses showed a potential protective trend of HCQ against preterm birth (unadjusted OR 0.86 [95% CI 0.40-1.84], p=0.70; Model 1 [adjusted for age, body mass index (BMI), recurrent abortion, antiphospholipid antibodies, undifferentiated connective tissue disease (UCTD)/Sjogren's syndrome (SS), serological markers]: OR 0.59 [95% CI 0.19-1.84], p=0.37; Model 2 [additional adjustment for low molecular weight heparin (LMWH), intravenous immunoglobulin (IVIG), prednisone, aspirin]: OR 0.41 [95% CI 0.11-1.51], p=0.18). Fetal weight (HCQ: 3215.46 ± 459.21g vs. non-HCQ: 3226.96 ± 500.01g, p=0.84) and maternal-fetal complications showed no differences. Subgroup analysis stratified by autoimmune diagnosis (antiphospholipid syndrome [APS], UCTD, SS) revealed no heterogeneity (APS: OR 0.78 [95% CI 0.28, 2.22], p = 0.64; UCTD: OR 0.52 [95% CI 0.15, 1.81], p = 0.30; SS: OR 1.03 [95% CI 0.12, 8.87], p = 0.98). This study showed that in women with a history of RPL and autoimmune disease, HCQ use was not statistically significantly associated with adverse pregnancy outcomes or maternal-fetal complications. A potential protective trend against preterm birth was observed, but this association did not reach statistical significance.
Ambient temperature and preterm birth: Interaction by maternal medical factors and medications.
Associations between high temperatures and preterm birth (PTB) were reported. However, characteristics of vulnerable pregnancies are unknown. We aimed to examine pre-pregnancy comorbidities, PTB history, and prescribed medications during pregnancy as potential modifiers. We conducted a secondary analysis of a historical cohort of 131 599 births in Southern Israel in 2005-2019. Maternal address and a spatiotemporal model were used to assess weekly average temperatures. We employed a Cox model with time-dependent covariates using a distributed lag non-linear model, adjusted for ethnicity, socioeconomic status, and time-varying seasonality. Associations were stratified by PTB history and restricted by comorbidities and medications. P-values for interaction were from likelihood ratio tests of models with multiplicative interaction term. Corrections for multiple comparisons were done using the Holm-Bonferroni method. Associations differed by PTB history (p for interaction <0.001). In women with previous PTB, high temperatures were associated with PTB from week 9, peaking at the last follow-up week (HR = 1.09, 95% CI: 1.01-1.17, 31 °C vs 16 °C); in contrast to the null associations in primiparous women and multiparous women without PTB history. We also found stronger associations for heat in women with autoimmune diseases and for cold in those with an antihistamine prescription, but interactions were not significant. Other comorbidities and medications we examined yielded inconsistent associations with high uncertainty. Our study highlights pregnancies with a PTB history as a high-risk group for focused interventions. Further studies with larger population sizes are needed to examine interactions by comorbidities and medications.
Immune thrombocytopenia in patients with connective tissue disease.
Immune thrombocytopenia (ITP) is a secondary autoimmune complication that can develop in patients with connective tissue diseases (CTDs). Its pathogenesis involves autoantibody-mediated platelet destruction and immune complex deposition, constituting a distinct clinical manifestation of CTD-related autoimmune abnormalities in the hematologic system. Among the subtypes of CTD-related immune thrombocytopenia (CTD-ITP), systemic lupus erythematosus (SLE) and primary Sjögren's syndrome (pSS) are the most prevalent. CTD-ITP occurs within the unique clinical context of being a secondary condition to systemic diseases such as SLE or primary pSS, resulting in more complex clinical management and an increased risk of adverse outcomes. At present, no disease-specific treatment guidelines exist for CTD-ITP, and clinical management is largely based on therapeutic strategies established for primary ITP. However, conventional therapies, including glucocorticoids, often fail to achieve durable clinical remission, and prolonged use is associated with serious adverse effects. In recent years, the introduction of targeted agents such as thrombopoietin receptor agonists (TPO-RAs) and rituximab has markedly improved treatment response rates in CTD-ITP. Meanwhile, emerging targeted therapies, such as spleen tyrosine kinase (Syk) and Bruton's tyrosine kinase (BTK) inhibitors, as well as neonatal Fc receptor (FcRn) inhibitors, have shown considerable clinical promise. This narrative review, based on recent literature, focuses on SLE-ITP and pSS-ITP, summarizing their clinical heterogeneity, disease-specific pathophysiological mechanisms, and recent therapeutic advances. The objective is to provide evidence-based guidance for the personalized management of CTD-ITP.
