Skin-Limited Neonatal Lupus in an Infant Conceived by In Vitro Fertilization and Surrogate Gestation.
Neonatal lupus erythematosus (NLE) is a rare autoimmune condition caused by the transplacental passage of maternal anti-Ro/SSA and anti-La/SSB antibodies, typically presenting with annular erythema or congenital heart block. We report a 3-month-old infant conceived by in vitro fertilization and carried by a surrogate mother who developed annular erythematous plaques in typical and atypical locations at 1 month of age. Skin biopsy and serologic testing confirmed NLE despite the absence of a known maternal autoimmune history; maternal anti-Ro/SSA and anti-La/SSB positivity was identified retrospectively through the surrogate's obstetrician. The cutaneous findings improved significantly with photoprotection and resolved without systemic involvement.
Systemic Manifestations of Neonatal Lupus in an Infant Born to an Asymptomatic Mother: A Case Report of Anemia and Transaminitis.
BACKGROUND Neonatal lupus erythematosus (NLE) is a rare autoimmune disorder caused by the transplacental transfer of maternal autoantibodies, primarily anti-Ro/SSA and anti-La/SSB. Common clinical manifestations include cutaneous lesions and congenital heart block; however, systemic involvement such as anemia and transaminitis remains infrequently reported, particularly in Southeast Asian populations. Diagnosis is often missed, especially in infants born to asymptomatic mothers. CASE REPORT A 1-month-old full-term female infant born via cesarean section presented with annular erythematous skin lesions since birth, distributed across the face, chest, and extremities. Laboratory tests revealed normocytic normochromic anemia, thrombocytopenia, and elevated aspartate aminotransferase/alanine aminotransferase levels. Although the electrocardiogram was normal, echocardiography showed a secundum-type atrial septal defect and mild tricuspid regurgitation. Skin biopsy revealed histopathological features consistent with cutaneous lupus. Both maternal and infant serology were positive for anti-Ro and anti-La antibodies. A diagnosis of cutaneous NLE with systemic manifestations was established. Prednisone therapy at a dose of 0.5 mg/kg/day was initiated, along with education on how to avoid sun exposure. After 2 months of therapy, the skin lesions had resolved, and laboratory parameters had improved. CONCLUSIONS This case highlights the importance of considering NLE as a differential diagnosis in infants presenting with cutaneous lesions and systemic abnormalities, even when the mother is asymptomatic. Early diagnosis and appropriate treatment can prevent complications and support optimal clinical recovery.
Seizure phenotypes and short-term prognosis in neonatal lupus: a multicenter retrospective cohort in China.
Neonatal lupus erythematosus (NLE) is an antibody-mediated autoimmune disorder that can affect neurologic outcomes. Seizures are an uncommon but clinically important phenotype. This study assessed clinical features, neuroimaging findings, and short-term outcomes in infants with NLE and seizures. We conducted a multicenter retrospective cohort study of infants with NLE admitted to seven tertiary centers in China. Infants were categorized as epileptic seizures (ES), non-epileptic CNS involvement (NE), or no CNS involvement (nCNS). Maternal characteristics and medications during pregnancy, infant clinical manifestations, laboratory indices, autoantibodies, EEG findings, and neuroimaging were extracted from medical records. Group comparisons and multivariable logistic regression were performed to identify factors associated with ES. Follow-up was conducted to 6 months of age. Among 246 infants with NLE, 17 (6.91%) had seizures, 39 (15.85%) had non-seizure CNS involvement, and 190 (77.24%) had no CNS involvement. All ES infants presented with acute symptomatic seizures, predominantly focal. Neuroimaging abnormalities were common, most frequently intracranial hemorrhage. Anti-SSA/Ro positivity was universal in ES cases and significantly higher than in nCNS (P < 0.05). Maternal hydroxychloroquine (HCQ) use was less frequent in ES than nCNS (P < 0.05). Regarding organ involvement, the ES group showed higher rates of thrombocytopenia, coagulation abnormalities, hypocomplementemia, and pancytopenia than the nCNS group, as well as higher frequencies of hypocomplementemia and pancytopenia compared with the NE group (all P < 0.05). Multivariate regression analysis revealed that maternal HCQ use during pregnancy was independently associated with a lower odds of ES (OR: 0.066, 95% CI: 0.015-0.288, P = 0.001). No recurrent seizures were observed in neonates in the ES group after hospital discharge. However, nine infants exhibited varying degrees of developmental delay. NLE infants with seizures often exhibit focal seizures and structural brain injury, associated with anti-SSA/Ro positivity, limited maternal HCQ exposure, and hematologic abnormalities. Despite good seizure control, developmental delay was frequent, indicating risk for adverse neurodevelopment. Lupus erythematosus is a multisystem disorder that predominantly affects the skin. There are many types of lupus, including systemic lupus erythematosus (SLE), drug-induced lupus erythematosus, neonatal lupus erythematosus, and cutaneous lupus erythematosus. The most common types of cutaneous lupus erythematosus include acute cutaneous lupus erythematosus, subacute cutaneous lupus erythematosus, and chronic cutaneous lupus erythematosus; discoid lupus erythematosus is a subtype of chronic cutaneous lupus erythematosus. Dr James Gilliam described the most commonly used classification of cutaneous lesions in lupus erythematosus. He distinguished lesions as either specific or nonspecific, based on the presence of interface dermatitis on histopathologic examination. Within the category of specific cutaneous lesions, he subdivided these into acute cutaneous lupus erythematosus, subacute cutaneous lupus erythematosus, and chronic cutaneous erythematosus. Discoid lupus erythematosus is the most common subset of chronic cutaneous lupus erythematosus. Patients may or may not report photosensitivity, but lesions are frequently photodistributed and often exhibit secondary atrophy or scarring. Most patients with discoid lupus erythematosus do not have significant systemic disease. Discoid lupus erythematosus can also occur as a manifestation of SLE in approximately 20% of patients. Other less common variants of chronic cutaneous lupus erythematosus include hypertrophic lupus erythematosus, tumid lupus erythematosus, lupus erythematosus panniculitis (also known as lupus profundus), chilblain lupus erythematosus, oral discoid lupus erythematosus, and discoid lupus erythematosus lesions on the palms and soles.
Twin Presentation of Cutaneous Neonatal Lupus Associated With Maternal Anti-U1RNP Antibodies: A Case Report.
Neonatal lupus erythematosus (NLE) is a rare autoimmune condition caused by the transplacental transfer of maternal IgG autoantibodies, most commonly anti-Sjögren's-syndrome-related antigen A (anti-Ro/SSA) and anti-Sjögren's-syndrome-related antigen B (anti-La/SSB). NLE associated solely with anti-U1RNP antibodies is uncommon and typically limited to cutaneous manifestations, with extremely few cases reported in twins. We describe bi-chorionic bi-amniotic preterm twin girls who developed atypical erythematous lesions along the trajectory of epicutaneo-cava catheters during the second week of life. Autoimmune screening revealed strongly positive anti-U1RNP antibodies in both infants; cardiac evaluation was normal. Autoantibodies became negative by eight months. Both twins showed normal development and no further manifestations at 15 months. Maternal evaluation uncovered previously unrecognized antinuclear antibody (ANA) and anti-ribonucleoprotein antibody(anti-RNP) positivity. This appears to be the first reported case of anti-U1RNP NLE presenting with catheter-aligned inflammatory lesions in twins. The case highlights the role of trauma-induced Koebnerization as a potential trigger and underscores the importance of considering autoimmune etiologies in synchronous neonatal skin lesions, as well as the value of neonatal findings in identifying occult maternal autoimmunity.
[Congenital periorbital skin changes in an infant-how to interpret raccoon eyes?].
A 4-month-old female infant presented with congenital periorbital skin changes. The patient showed bilateral periorbital erythema, telangiectasia and eczematous skin lesions resembling "raccoon eyes" which resulted in a clinical diagnosis of neonatal lupus erythematosus. The diagnosis was confirmed by the detection of elevated anti-Ro antibodies in both the infant and her mother. No cardiological or other abnormalities were found. Neonatal lupus erythematosus (NLE) is an acquired autoimmune disorder caused by transplacental passage of maternal immunoglobulin G (IgG) autoantibodies, predominantly Sjögren syndrome antigens A (Ro/SSA) and B (La/SSB), leading to clinical manifestations in the neonate. This pathogenesis parallels other neonatal autoimmune conditions, including antiphospholipid antibody syndrome, Graves disease, immune thrombocytopenic purpura, myasthenia gravis, and autoimmune blistering diseases. Bridge and Foley first described NLE in 1954, after observing transplacental transmission of the maternal lupus erythematosus factor to newborns. That same year, a lupus rash was reported in a 6-week-old infant whose mother was later diagnosed with systemic lupus erythematosus (SLE). In 1957, congenital heart block was documented in an infant born to a mother with SLE. Recognition of NLE as a multisystem disorder has broadened its clinical spectrum to include hepatic enzyme elevations and transient cytopenias. The clinical spectrum of NLE ranges from transient hematological and dermatological abnormalities to severe, irreversible cardiac manifestations, especially congenital heart block. Notably, a substantial proportion of affected neonates are born to mothers without clinically apparent autoimmune disease, highlighting the importance of maternal antibody screening during pregnancy.
