Os sintomas podem incluir desconforto ou dor geral no corpo, sangramento nasal e/ou sangue na urina, problemas respiratórios, anemia, dor no peito e insuficiência renal. Acredita-se que a doença anti-GBM resulte de um insulto ambiental (tabagismo, infecções, exposição a certos medicamentos) em uma pessoa com suscetibilidade genética, como um tipo específico de antígeno leucocitário humano (HLA). O diagnóstico é confirmado pela presença de anticorpos anti-GBM no sangue ou nos rins. O tratamento de escolha é a plasmaférese associada à prednisona e ciclofosfamida.

Treatment of dual-positive anti-glomerular basement membrane and ANCA disease using rituximab in addition to plasma exchange, corticosteroids and cyclophosphamide.
We present the case of a transgender man with dual-positive anti-glomerular basement membrane and anti-neutrophil cytoplasm antibody disease who was able to achieve dialysis independence with sustained disease remission and renal recovery. This was achieved using a treatment regimen of plasma exchange, high-dose corticosteroids, cyclophosphamide and rituximab. Despite a number of poor prognostic factors, including requirement for dialysis during the index admission and a high proportion of crescents on diagnostic biopsy, the patient was able to achieve renal recovery. This case report adds to the limited literature supporting the use of rituximab as part of the treatment regimen of dual-positive disease.
Mitochondrial abnormalities in nondiabetic and nonhypertensive glomerular diseases: a comprehensive review.
Glomerulonephritis (GN) is a general term which encompasses various types of glomerular disorders characterized by damage to the capillary endothelium, basement membrane, podocytes, mesangium, or parietal epithelial cells with different combinations leading to proteinuria, hematuria, and azotemia. Although disease process begins in the cells of mentioned above, there is cross-talk with tubular cells leading to tubular atrophy and interstitial fibrosis in the final stages of most GN. Recent developments in genetic, molecular, serologic methods enhances understanding of the pathophysiology and management of GN although more work is needed. The recent ultra-structural studies demonstrated various subcellular disorders present in the context of GN. Mitochondria are one of the most studied subcellular organelles, and various mitochondrial structural and functional alterations have been identified in GNs, including focal segmental glomerulosclerosis, IgA nephropathy, lupus nephritis and anti-glomerular basement membrane disease. However, these studies are still at an early stage and currently the impacts of mitochondrial dysfunction on the development and progression of glomerular disease are not well defined. In the current review article, we examine how mitochondrial dysfunction associates with GN, and discuss the unknowns, conflicting issues and potential treatment options regarding mitochondrial dysfunction and GN.
Relapse of subacute anti-glomerular basement membrane nephritis following self-discontinuation of steroid therapy.
Anti-glomerular basement membrane (GBM) nephritis is an autoimmune disease which typically presents rapid progressive glomerulonephritis. A subacute course and relapse are rare. We report the case of a 23-year-old woman with no prior medical history, who was referred for proteinuria and elevated serum creatinine (Cr) level of 3.3 mg/dL. Two weeks later, upon her visit to our hospital, her creatinine level was 3.2 mg/dL. Five months prior, her Cr was 1.2 mg/dL. Urinalysis showed 2.3 g/gCre of proteinuria and microscopic hematuria. The anti-GBM antibody titer was high at 135 U/mL. Anti-neutrophil cytoplasmic antibody and anti-nuclear antibody were negative. Kidney biopsy revealed crescentic glomerulonephritis with advanced glomerular sclerosis and diffuse linear staining of IgG along the GBM. She received corticosteroids, intravenous cyclophosphamide and 7 sessions of plasma-exchanges. After three months, her Cr improved to 1.9 mg/dL and the antibody became undetectable. However, she self-discontinued prednisolone 2 months later. Three weeks after discontinuation, she presented with nausea, oliguria. Her Cr level had risen to 9.6 mg/dL and the anti-GBM antibody titer was 4.0 U/mL. A second kidney biopsy showed progression of glomerulosclerosis, endothelial injury in the arterioles characterized by fibrin deposition, and extensive tubulointerstitial damage. Despite resuming treatment, her kidney function did not recover, and she required maintenance hemodialysis. This case highlights two atypical features of anti-GBM nephritis: subacute progression over 5 months, and relapse after antibody negativity triggered by steroid discontinuation. This emphasizes the importance of adherence to immunosuppressive therapy, even after serological remission.
Elevated serum soluble α3(IV)NC1 correlates with kidney injury and worse outcome in patients with anti-glomerular basement membrane disease.
Anti-glomerular basement membrane (GBM) disease is the most severe form of autoimmune kidney diseases characterized by pathogenic autoantibodies against GBM component, α3(IV)NC1. However, the status of circulating soluble α3(IV)NC1 (also known as tumstatin) in patients with anti-GBM disease and its clinical relevance remain unclear. The aim of this study was to investigate the serum soluble α3(IV)NC1 level [ss-α3(IV)NC1] in patients with anti-GBM disease, and to analyze its association to clinical characteristics. Seventy patients with anti-GBM disease and 30 healthy individuals were enrolled. Ss-α3(IV)NC1 was measured by enzyme-linked immunosorbent assay. The level was compared and analyzed with clinical-pathological features and kidney outcome, and the statistical significance was determined. Ss-α3(IV)NC1 concentrations were elevated in 41 (58.6%) patients with anti-GBM disease and 0 (0.0%) healthy individuals. They were significantly higher in patients with anti-GBM disease (18.32 ± 9.56 ng/mL) than those in healthy individuals (5.84 ± 2.78 ng/mL) (P < 0.001). Ss-α3(IV)NC1 correlated with age (r = 0.385, P = 0.001), serum creatinine (r = 0.286, P = 0.016), eGFR (r = -0.304, P = 0.010), crescent percentage (r = 0.339, P = 0.030), and normal glomeruli percentage (r = -0.354, P = 0.023). Kaplan-Meier analysis revealed that patients with elevated ss-α3(IV)NC1 had a worse kidney outcome than those with normal level (P = 0.011). The ss-α3(IV)NC1 was abnormally elevated in patients with anti-GBM disease. Elevated ss-α3(IV)NC1 was associated with the disease severity in anti-GBM disease. The role of these elevated levels warrants further investigation.
An Exceptional Case of Crohn-Associated IgA-Mediated Goodpasture Disease.
Anti-glomerular basement membrane (anti-GBM) disease is typically mediated by IgG autoantibodies and presents with pulmonary hemorrhage and rapidly progressive glomerulonephritis, classically referred to as Goodpasture disease. We describe a novel IgA-mediated variant in a 53-year-old man with Crohn colitis, who presented with oligo-anuric acute kidney injury and hemoptysis. Despite having negative serology for anti-GBM antibodies, kidney biopsy revealed necrotizing crescentic glomerulonephritis with linear IgA deposition along the glomerular basement membrane. We suspect that a Crohn colitis flare may have incited a novel IgA-driven anti-GBM process which, remarkably, resolved in the absence of conventional immunosuppressive therapy, sharply deviating from previously reported IgA-mediated disease requiring aggressive intervention. Such a case suggests a possible link between abnormal IgA glycosylation in inflammatory bowel disease and the development of a unique IgA-mediated anti-GBM process. Recognition of this entity may broaden the diagnostic approach to seronegative pulmonary-renal syndromes and also highlight the potential for nonimmunosuppressive management.
