A Bloody Cough and Clouded Lungs: Uncovering a Systemic Culprit in an Adolescent.
Hemoptysis secondary to diffuse alveolar hemorrhage (DAH) is uncommon in children. When it does occur, extensive investigations are often required to determine the underlying cause. We present a unique case of a 17-year-old previously healthy male adolescent with a history of allergic rhinitis, who initially presented with pneumonia in the setting of influenza A. He subsequently developed DAH, prompting further diagnostic evaluations that ultimately revealed a rare condition with long-term morbidity.
Incidence and Impact of Endothelial Dysfunction Events and Veno-Occlusive Disease in Haploidentical Stem Cell Transplantation with Post-Transplant Cyclophosphamide: A Retrospective Analysis From the Argentinian Group of Marrow Transplant and Cellular Therapy (GATMO-TC).
Endothelial dysfunction events (EDE) and veno-occlusive disease (VOD) are clinically relevant early complications after hematopoietic stem cell transplantation (HSCT). Data in haploidentical HSCT (HaploSCT) using post-transplant cyclophosphamide (PTCy) are limited in Latin American settings. We conducted a retrospective multicenter study including 310 consecutive adult patients (≥18 years) undergoing unmanipulated HaploSCT with PTCy between 2015 and 2022 across 8 centers in Argentina. EDE was defined as a composite endpoint including VOD, transplant-associated microangiopathy (TMA), renal dysfunction, diffuse alveolar hemorrhage (DAH), and posterior reversible encephalopathy syndrome (PRES); acute GvHD (aGvHD) was analyzed separately (MAGIC criteria). VOD cases were re-adjudicated using EBMT criteria as the standard. Disease risk was categorized using the disease risk index (DRI). All patients received ursodeoxycholic acid prophylaxis and PTCy-based GvHD prophylaxis. Cumulative incidence functions were estimated with competing risks and compared with Gray's test; Fine-Gray models were used for multivariable analyses. Overall survival (OS) was analyzed by Kaplan-Meier and log-rank testing (2-sided P < .05). Median age was 42 years (range 18 to 77) and median follow-up was 37.7 months. Most patients received myeloablative conditioning (75.2%); conditioning backbone was busulfan-based in 54.0%, TBI-based in 34.6%, and melphalan/other in 11.4%. At 1 year, cumulative incidence of EDE (composite including VOD) was 5.5% (95% CI 3.0 to 8.0), with median time to first EDE 30.5 days (IQR 16.3 to 36.8). Among 18 EDE cases, phenotypes were VOD (n = 9; 50.0%), transplant-associated microangiopathy (TMA) (n = 5; 27.8%), renal dysfunction (n = 2; 11.1%), diffuse alveolar hemorrhage (DAH) (n = 1; 5.6%), and posterior reversible encephalopathy syndrome (PRES) (n = 1; 5.6%). Patients aged ≤25 years had higher EDE incidence than >25 years (11.5% [95% CI 3.9 to 19.0] versus 3.8% [95% CI 1.3 to 6.2]; Gray P = .021). In multivariable Fine-Gray regression, age ≤25 years was the only independent predictor of EDE (SHR 4.26; 95% CI 1.42 to 12.74; P = .01). By day 100, cumulative incidence of grade II to IV and grade III to IV aGvHD was 29.8% (95% CI 24.2 to 35.4) and 9.0% (95% CI 5.3 to 12.8), respectively; EDE was associated with higher aGvHD incidence (SHR 3.14; 95% CI 1.47 to 6.68; P = .003). Median time to VOD diagnosis was 25 days (range 8 to 57); 44.4% were classical (≤21 days) and 55.6% late-onset, with severe VOD comprising 55.6%. Three-month VOD incidence was higher in patients ≤25 years (8.6% [95% CI 2.0 to 15.3]) versus >25 years (1.2% [95% CI 0.0 to 2.7]; Gray P = .001). OS was worse in patients with EDE (log-rank P = .014): median OS 7.9 months (95% CI 2.3 to 15.5) versus 23.4 months (95% CI 16.3 to 29.6), and 2-year OS 25.0% (95% CI 6.7 to 49.1) versus 49.6% (95% CI 43.2 to 55.7). In this multicenter adult HaploSCT-PTCy cohort, EDE and VOD were infrequent but associated with substantially inferior OS. Age ≤25 years consistently identified a higher-risk subgroup for EDE and VOD, supporting intensified surveillance and early recognition strategies in this platform.
Pulmonary hemorrhage as a first manifestation of Sjögren's disease in a young male adult.
Sjögren's syndrome (SS) is a chronic autoimmune disorder primarily affecting exocrine glands, characterized by dry mouth (xerostomia) and dry eyes (keratoconjunctivitis sicca). However, systemic manifestations, including pulmonary complications, are increasingly recognized. Diffuse alveolar hemorrhage (DAH) is a rare but life-threatening presentation of SS, typically associated with vasculitis or coagulation abnormalities. We report the case of a 19-year-old male presenting with dyspnea and hemoptysis, later diagnosed with primary Sjögren's syndrome (pSS). Initial clinical assessment revealed respiratory failure with low blood oxygen levels (hypoxemia), bilateral ground-glass opacities on chest computed tomography (CT), and iron-storing macrophages (hemosiderin-laden) in bronchoalveolar lavage fluid, consistent with DAH. Subsequent autoimmune serology confirmed positivity for anti-Ro/Sjögren's-syndrome-related antigen A autoantibodies (SSA) and anti-La/Sjögren's-syndrome-related antigen B autoantibodies (SSB), while other autoimmune markers, including anti-neutrophil cytoplasmic antibodies (ANCA), were negative. Labial salivary gland biopsy demonstrated focal lymphocytic sialadenitis, confirming pSS according to European League Against Rheumatism (EULAR) criteria. Treatment involved high-dose corticosteroids, leading to complete resolution of symptoms and significant improvement in imaging results. The patient remained stable at follow-up. DAH should be considered a potential presentation of pSS, even in the absence of classical symptoms. Increased awareness of this rare complication can facilitate early diagnosis and improve outcomes.
Therapeutic Evolution and Outcomes in EGPA Complicated by Diffuse Alveolar Hemorrhage: case-based review.
Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare systemic vasculitis characterized by asthma, eosinophilia, and multi-organ involvement. Diffuse alveolar hemorrhage (DAH) is an uncommon but life-threatening pulmonary complication in EGPA. We report a 49-year-old previously healthy woman who presented initially with asthma-like symptoms and later developed fever, hemoptysis, cutaneous purpura, and periorbital edema. Laboratory evaluation revealed marked eosinophilia, anemia, elevated inflammatory markers, and strongly positive MPO-ANCA. Bronchoalveolar lavage fluid (BALF) was hemorrhagic and contained hemosiderin-laden macrophages, indicating DAH. Broad-spectrum antibiotics were empirically initiated but discontinued after metagenomic next-generation sequencing (mNGS) of BALF excluded infection. Bone marrow biopsy showed eosinophilic hyperplasia without clonal mutations. A diagnosis of MPO-ANCA positive EGPA with DAH was established. The patient received pulse methylprednisolone, prednisone, intravenous immunoglobulin, mepolizumab, and rituximab. Clinical symptoms improved rapidly, and radiological signs of alveolar hemorrhage nearly resolved within days. Our case illustrates that integration of rituximab and mepolizumab with corticosteroids can achieve rapid remission and steroid sparing in EGPA-DAH. While evidence remains limited to case reports and small series, targeted biologics may fundamentally improve outcomes in this high-risk subset. Prospective studies are warranted to define optimal treatment strategies.
Diffuse alveolar hemorrhage in systemic lupus erythematosus during the COVID-19 era-a retrospective analysis with mechanistic investigation.
Viral infection plays an important role in driving activity of autoimmune diseases. Preceding SARS-CoV-2 infection has been observed to trigger disease flares in systemic lupus erythematosus (SLE). Besides cytomegalovirus, SARS-CoV-2 can induce pulmonary capillaritis to develop diffuse alveolar hemorrhage (DAH), a respiratory emergency of SLE. Owing to lack of comprehensive studies for SARS-CoV-2-triggered DAH in SLE, we conducted a monocentric analysis with mechanistic investigation. Hospitalized SLE patients were retrospectively analyzed for the DAH manifestation in the COVID-19 era since December 2019, focusing on those with preceding SARS-CoV-2 infection. Peripheral blood (PB) and lung tissues samples and immune/alveolus cell lines were used for mechanistic research. Twenty-five DAH episodes were identified in 21 SLE patients (3% occurrence), all in disease flares with high activity scores. During the domestic omicron variant outbreak, 7 patients (33%) had preceding SARS-CoV-2 infection, 3 to 7 weeks earlier to the onset of DAH, while they had elevated IL-6, nitro-oxidative stress- and cell death-associated molecules levels in PB and lung tissues with enhanced apoptosis formation. IL-6-stimulated alveolus/immune cells exhibited a dose-dependent increase in nitro-oxidative stress- and cell death-associated molecules levels, up-regulated p38MAPK/NF-κB-p65 phosphorylation, raised ROS expression and enhanced apoptosis formation. These findings implicated cell death induced by IL-6-activated nitro-oxidative stress to generate circulating immune complexes with pulmonary deposition as the mechanism for DAH preceded by SARS-CoV-2 infection in SLE. The DAH manifestation preceded by SARS-CoV-2 infection is observed in SLE with disease flares, requiring careful monitor and management of COVID-19 in such patients.
