Beyond Neutropenic Fever: Severe Multiorgan Immune-Related Toxicity Induced by Pembrolizumab.
The use of immune checkpoint inhibitors (ICIs) has revolutionized cancer therapy, introducing a new spectrum of oncologic emergencies associated with immune-related toxicities, which are sometimes severe and difficult to recognize. We report the case of a 49-year-old woman with early-stage triple-negative breast cancer undergoing neoadjuvant chemotherapy with carboplatin, paclitaxel, and pembrolizumab, who presented to the emergency department with abdominal pain, diarrhea, nausea, and vomiting. On admission, she was in shock, with pancytopenia, acute kidney injury, hyponatremia, and low serum cortisol levels. After exclusion of infectious causes, she was diagnosed with pembrolizumab-induced multiorgan immune-related toxicity, manifesting as acute adrenal insufficiency with shock (grade 3), colitis (grade 3), pancytopenia (grade 3), and hypothyroidism (grade 2). Treatment with methylprednisolone 500 mg/day for three days, followed by tapering, filgrastim, and levothyroxine adjustment, led to rapid clinical and laboratory improvement. Pembrolizumab was permanently discontinued. This case highlights the importance of early recognition and aggressive management of potentially fatal immune-related toxicities associated with ICIs.
A case of the endoscopic removal of a gastric bezoar made of sodium zirconium cyclosilicate hydrate.
Sodium zirconium cyclosilicate hydrate (SZC) is a novel potassium binder that can be administered either orally or via nasogastric tubes. We report the case of a 77-year-old woman who developed a 37-mm gastric bezoar after nasogastric SZC administration during intensive care for acute adrenal insufficiency. Endoscopic removal was performed, and the analysis confirmed that the SZC was the primary constituent. Impaired gastric motility, acidic gastric environment, and limited hydration are likely contributors. This case highlights the potential for SZC-induced gastric bezoar formation in critically ill patients, and underscores the importance of imaging monitoring, careful gastrointestinal management, and the awareness of this potential complication during SZC therapy.
Acute adrenal insufficiency-induced hypotension following spinal surgery in a pediatric patient with Charcot-Marie-Tooth disease and severe scoliosis: a case report.
Acute adrenocortical insufficiency (adrenal crisis) as a cause of postoperative hypotension is an uncommon and underreported condition in pediatric patients. Charcot-Marie-Tooth (CMT) disease, a hereditary motor and sensory neuropathy, frequently coexists with scoliosis. Surgical correction of severe scoliosis involves significant stress due to considerable surgical trauma and prolonged operative time. A 13-year-old female diagnosed with CMT and severe scoliosis (thoracic curvature Cobb angle 92°) underwent posterior osteotomy and corrective surgery. On postoperative Day 1, the patient experienced persistent hypotension (systolic blood pressure < 90 mmHg) that did not respond to fluid resuscitation or vasopressor support with dopamine. On postoperative Day 4, Adrenocorticotropic Hormone (ACTH) stimulation testing revealed a cortisol level of 323.1 nmol/L which was low in the context of physiological stress, along with a markedly reduced ACTH concentration of 4.72 pg/ml, confirming a diagnosis of acute adrenocortical insufficiency. Hydrocortisone treatment resulted in a gradual normalization of blood pressure, which returned to preoperative levels by postoperative Day 8. Steroid therapy was discontinued following normalization of adrenal hormone levels. In pediatric patients undergoing surgery for severe scoliosis, unexplained postoperative hypotension warrants thorough evaluation for adrenal crisis, especially in the context of underlying chronic illness or malnutrition.
Case Report: Type II Bartter syndrome with a novel KCNJ1 variant in a premature neonate presenting with features of salt-wasting congenital adrenal crisis and pseudo-hypoaldosteronism.
Bartter syndrome (BS) is a rare group of inherited renal tubulopathies. Diagnosis of BS type II is challenging in the neonatal period as its clinical findings and biochemical features may mimic that of adrenal crisis and pseudo-hypoaldosteronism (PHA) initially. Treatment should be instituted immediately for acute adrenal insufficiency as it is a medical emergency, then modified according to available investigation results and treatment response. We describe a premature female neonate with an antenatal history of severe unexplained polyhydramnios, presented with features of adrenal crisis managed with hydrocortisone and fludrocortisone. Initial endocrine investigations excluded salt-wasting congenital adrenal hyperplasia (SW-CAH) and pointed to the diagnosis of PHA with hyperreninemic hyperaldosteronism. Hydrocortisone was gradually weaned off while fludrocortisone was continued for sodium retention effect. Hyperkalemia quickly transited into hypokalemia requiring high potassium requirement. Clinical and biochemical features of BS gradually evolved with polyuria, excessive weight loss, hypochloremic metabolic alkalosis and hypercalciuria at 1 week of age. Urgent trio whole exome sequencing (WES) subsequently confirmed the diagnosis of BS type II where compound heterozygous missense variants were identified in the KCNJ1 gene, one of which was a novel variant. Fludrocortisone was stopped and indomethacin was started with favorable outcomes. Though hypokalemia is the key feature of BS, transient hyperkalemia can occur in the early neonatal period in BS type II. Antenatal history should be enquired thoroughly to look for presence of severe unexplained polyhydramnios. The diagnosis of BS type II should be considered if other biochemical features are present. Genetic tests are important to provide a definite diagnosis and guide subsequent management and genetic counselling.
Bilateral Adrenal Hemorrhage and Acute Adrenal Insufficiency Secondary to Heparin-Induced Thrombocytopenia After Coronary Artery Bypass Graft Surgery.
Adrenal insufficiency (AI) secondary to bilateral adrenal hemorrhage (BAH) from heparin-induced thrombocytopenia (HIT) is rare; only 3 instances of such cases were reported after coronary artery bypass graft surgery (CABG). We report a case of AI due to BAH from HIT after an elective CABG. A 56-year-old male with a history of diabetes and multivessel coronary artery disease underwent CABG a week ago presented with abdominal pain. Labs showed leukocytosis, hyperglycemia, elevated anion gap, and positive serum ketones. Intravenous insulin with empirical antibiotics was started. Platelet decline was noticed from 154 000/uL to 39 000/uL on the following days, along with fever, hypotension, altered mentation, hyponatremia, and abdominal pain. A computed tomography scan of the abdomen showed BAH. Primary AI was confirmed with a cosyntropin stimulation test, and HIT was diagnosed with Heparin-PF4 IgG assay and serotonin release assay. His symptoms improved after hydrocortisone treatment. AI secondary to BAH from HIT is a rare complication, with most cases reported after orthopedic surgeries. Symptoms of AI are vague and similar to sepsis. If hyponatremia, and gastrointestinal symptoms are noticed, adrenal insufficiency should be suspected, and early stress dose steroid treatment should be considered. Steroid treatment in such cases has shown to improve survival. The symptoms of adrenal insufficiency are nonspecific and could be similar to sepsis. Electrolyte abnormalities and gastrointestinal symptoms could be hints for adrenal insufficiency, especially if risks for adrenal hemorrhage are present. Early stress dose treatment in such cases has shown a survival benefit.
